OBJECTIVE: (a) To explore burden related to caregiving and support received from professionals and social network in relatives of patients with schizophrenia in Northern, Central and Southern Italy; and (b) to test whether a higher level of family burden is associated with a lower level of professional and social network support.
METHOD: Seven hundred and nine patients with schizophrenia and their key-relatives were consecutively recruited in 30 Italian mental health departments. Data were collected on: (a) patients' clinical status and levels of disability; (b) relatives' burden, social and

BACKGROUND: Decreases in length of hospital stay have increased pressure on staff to provide appropriate patient education. A hospitalwide continuous quality improvement (CQI) program was implemented to improve patient education documentation at Raritan Bay Medical Center (RBMC; Perth Amboy, NJ). IMPROVING THE PATIENT EDUCATION DOCUMENTATION PROCESS: In March 1995 the patient education committee was given the responsibility to develop a CQI program to improve documentation of patient education. Patient interviews revealed that assessment of patients' initial knowledge was inadequate and goal s

A case of homosexual erotomania is described in a patient with AIDS-related complex. Direct involvement of the central nervous system was thought an unlikely cause, as specific stressors appeared to have precipitated a reactive psychosis. A psychodynamic understanding of these factors and their interaction with the patient's masochism, fetishism, and personality disorder provided an alternative explanation for the onset of the paranoid illness and its symptom content. The psychodynamic aspects of acute psychotic disorders without evidence of cognitive impairment in patients with HIV infection

Primal repression, long an obscure and unusable concept, has been given a precise place in a recent re-working of the theory of repression (Cohen & Kinston, 1984); and this paper specifically examines its properties and presentation. Primal repression refers to an absence of psychic structure which can be made good in the process of emotional growth. It is a part of the mind where trauma persists; and it has a close connexion to the unrepressed unconscious. Direct emergence of primal repression is a threat to life and its activation is therefore risky. During psychoanalysis, primal repression

OBJECTIVE: Several clinical studies have found that therapists differ in their client outcomes, supporting the notion of "therapist effects" in psychotherapy. However, only a handful of studies have investigated therapist effects in internet-delivered cognitive behavior therapy. This study aimed to examine therapist effects in internet-delivered cognitive behavior therapy treatment of anxiety and depression in routine care.
METHOD: Data of 8,145 clients who were treated by 44 therapists were examined. Generalized linear mixed models were performed to identify the presence of therapist effects

OBJECTIVES: Although improving the average patient experience is at the center of recent efforts to make cancer care more patient centered, extreme experiences may be more informative for quality improvement. Little is known about the most deeply dissatisfying experiences that predispose disengagement and negatively influence patient outcomes. We sought to establish a framework for emotionally adverse patient experiences and identify the range of common causes.
STUDY DESIGN: Qualitative study including in-depth interviews and free-text survey comments.
METHODS: Thematic analysis of 20 open-e

BACKGROUND: While primary care medical clinics have been the most common setting for the delivery of advice about smoking cessation, the hospital emergency department (ED) is a valuable context for counseling medically underserved tobacco users. We conducted a secondary analysis based on a larger audio-recorded study of patient-provider communication about pain and analgesics in the ED. Within a sample of ED patients with back pain, the purpose of this mixed-methods study was to examine how physicians and nurse practitioners capitalize on "teachable moments" for health education to offer spont

BACKGROUND: Patient satisfaction is a determinant of treatment uptake, adherence and retention, and an important health systems outcome. Queues, health worker-patient contact time, staff attitudes, and facility cleanliness may affect patient satisfaction. We quantified dimensions of patient satisfaction among HIV and TB patients in a rural sub-district of KwaZulu-Natal, South Africa, and identified underlying satisfaction factors that explained the data.
METHODS: We conducted patient-exit interviews with 300 HIV and 300 TB patients who were randomly selected using a two-stage cluster random s

Although the alliance-outcome correlation is well established, no published studies to date have separated between therapists' and patients' contributions while controlling for early symptom change. In this study, we examined therapist effects in two trials of CBT for panic disorder with agoraphobia (PDA) and the impact of therapists' and patients' contribution to the alliance on outcome and attrition in one trial. Alliance ratings were obtained from patients and therapists early and late in treatment (n = 133). Data were analyzed using multi-level modeling controlling for early symptom change

BACKGROUND: Little is known regarding which behavioural responses can be expected if an influenza pandemic were to occur.
METHODS: A survey comprising questions based on risk perception theories, in particular PMT, was conducted with a Dutch sample.
RESULTS: Although fear that an influenza pandemic may occur was high, participants do not feel well informed. General practitioners and local health authorities were considered trustworthy sources of information and the information considered most urgent pertained to which protective measures should be taken. Participants reported an intention to

Journal of clinical child and adolescent psychology : the official journal for the Society of Clinical Child and Adolescent Psychology, American Psychological Association, Division 532002eng
Examined how 56 children who had received brief-exposure treatment for specific phobias experienced the treatment. It was also investigated whether there was a relation between the children's reported experiences before, during, and after the treatment and therapy outcome. Results indicated that the treatment was experienced as something positive, and the large majority of the children appreciated the pace and degree of control they had during treatment, as well as the therapist and the treatment outcome. The response patterns did not differ between sexes, diagnostic groups, mode of treatment,

Data from therapists who were treating 26 patients when they committed suicide were utilized to identify signs that warned of a suicide crisis. Three factors were identified as markers of the suicide crisis: a precipitating event; one or more intense affective states other than depression; and at least one of three behavioral patterns: speech or actions suggesting suicide, deterioration in social or occupational functioning, and increased substance abuse. Problems in communication between patient and therapist were identified as factors interfering with crisis recognition. Evaluation of the id

Values of 42 Alaska Native clients, 30 Alaska Native counselors, and 19 non-Native counselors at seven Indian Health Service inpatient alcoholism treatment programs in Alaska are compared. Using the Rokeach Value Survey, differences were revealed on six instrumental values and six terminal values. The primary value disparities were between the Alaska Native groups and non-Native counselors. Specifically, both Alaska Native groups placed greater importance on values that were other-focused while the non-Native counselors placed more importance on values that were self-focused. Minor differences
