A Novel Tool for Evaluation of Mild Traumatic Brain Injury Patients in the Emergency Department: Does Robotic Assessment of Neuromotor Performance Following Injury Predict the Presence of Postconcussion Symptoms at Follow-up?

OBJECTIVES: Postconcussion symptoms (PCS) are a common complication of mild traumatic brain injury (TBI). Currently, there is no validated clinically available method to reliably predict at the time of injury who will subsequently develop PCS. The purpose of this study was to determine if PCS following mild TBI can be predicted during the initial presentation to an emergency department (ED) using a novel robotic-assisted assessment of neurologic function. METHODS: All patients presenting to an urban ED with a chief complaint of head injury within the preceding 24 hours were screened for inclu

Complex impairment of IA muscle proprioceptors following traumatic or neurotoxic injury.

The health of primary sensory afferents supplying muscle has to be a first consideration in assessing deficits in proprioception and related motor functions. Here we discuss the role of a particular proprioceptor, the IA muscle spindle proprioceptor in causing movement disorders in response to either regeneration of a sectioned peripheral nerve or damage from neurotoxic chemotherapy. For each condition, there is a single preferred and widely repeated explanation for disability of movements associated with proprioceptive function. We present a mix of published and preliminary findings from our …

Neural substrate for the effects of passive training on sensorimotor cortical representation: a study with functional magnetic resonance imaging in healthy subjects.

Repetitive passive movements are part of most rehabilitation procedures, especially in patients with stroke and motor deficit. However, little is known about the consequences of repeated proprioceptive stimulations on the intracerebral sensorimotor network in humans. Twelve healthy subjects were enrolled, and all underwent two functional magnetic resonance imaging (fMRI) sessions separated by a 1-month interval. Passive daily movement training was performed in six subjects during the time between the two fMRI sessions. The other six subjects had no training and were considered as the control g

Central nervous system modifications in patients with lesion of the anterior cruciate ligament of the knee.

Patients with traumatic lesion of the anterior cruciate ligament often experience knee instability, which, recent studies suggest, is probably due to reduced knee proprioception. We studied knee proprioception and somatosensory evoked potentials (SEPs) after stimulation of the common peroneal nerve at the knee above the articular branches subserving the sensory innervation of the anterior cruciate ligament, in 19 patients with traumatic anterior cruciate ligament lesion. Ten patients showed decreased position sense of the knee, and of these, seven presented loss of cortical P27 potential while…

The role of the neuromuscular mechanism in craniomandibular pain was discussed. Proprioception reflex activity forms the basis for muscle length, mandibular positional sense, as well as masticatory function. Occlusal disharmonies increase noxious input to the neuromuscular system, as well as stress-induced clenching, causing increased muscle …

Sources of afferent fibers from the tongue muscles.

The afferent impulses from the tongue muscles are carried by the hypoglossal nerve, trigeminal nerve, and some of them by glossopharyngeal nerve. The main exit of afferent fibers constitutes the hypoglossal nerve. The location of the ganglion cells responsible for proprioceptive impulses are mainly in the spinal ganglia of the C1-C3 spinal nerves.

A Modern Approach to Brown-Séquard Syndrome: A Case Report.

In this case, we discuss an 82-year-old male patient with a combination of motor weakness and loss of proprioception on one side of the body, together with reduced pain and temperature sensation on the opposite side, which is characteristic of Brown-Séquard syndrome (BSS). This case highlights the differential diagnoses of those presenting with BSS as well as the diagnostic workup in the modern day for BSS. This case gives the reader the opportunity to go through tailored investigations and treatment for the various BBS differentials.

Anterior spinal artery syndrome presenting as cervical myelopathy in a patient with subclavian steal syndrome.

The anterior spinal artery syndrome (ASAS), described by Preobraschenski in 1908,1 is characterized by bilateral spastic paraparesis and dissociated sensory deficits involving bilateral loss of temperature and pain sensation but preserved proprioception and touch. The ASAS typically develops acutely, over minutes to hours. While initially linked to infections, in particular syphilis, many other etiologies are now recognized, including aortic atheromatous disease and thromboembolic phenomenon. We report a case of ASAS in a patient with a steal phenomenon secondary to a right-sided aortic arch w…

[Rehabilitation patients after fractures of upper limb].

Rehabilitation after fractures of upper limb is very complicated and should be coordinated with orthopedic procedures. A choice of method and schema is depended on general health status of patient and used orthopedic treatment. This process should start immediately after surgeon's intervention. An aim of rehabilitation is improvement of range movement, straight of muscles and acceleration of callus formation. It is very important to restore proprioception in a region of trauma. An intensity and velocity of rehabilitation is the greatest after extra bone consolidation and the slowest after cons…

Biomechanics of functional electrical stimulation.

Patients with hemiplegia frequently have difficulty in walking due to lack of eversion and dorsiflexion capability of the foot. One method of treating these patients utilizes functional electrical stimulation (FES). The effect of FES on locomotion, co-ordination, proprioception and balance sense was assessed using instrumented gait analysis and a postural sway test. In general patients treated with FES showed either a marked improvement or very little change. Any improvement was reflected in postural sway and ankle control during locomotion. Changes in hip and knee control were insignificant.

A comparison of three vibrators in static posturography: the effect of vibration amplitude on body sway.

In static posturography, proprioception is often disturbed using vibrators applied bilaterally to the calf muscles. The effect of vibrator amplitude on body sway was compared in static posturography using bilateral vibrators on the calf muscles of 30 healthy male military conscripts at frequencies of 50 and 90 Hz. Postural stability was measured in terms of BSV (body sway velocity), and maximal displacements of the centre of force (MAXY, MAXX) in the anterio-posterior and lateral directions. In comparing the effects of vibration to base stance without vibration, BSV seemed to be the most sensi…

Dynamic platform sway measurement in Menière's disease.

Fourteen patients in the latent phase of Menière's disease and 14 control subjects were tested on a moving platform apparatus as part of a protocol of investigations for Menière's disease. Subjects were tested under 3 different visual conditions (eyes open, eyes closed, sway feedback) and 2 proprioceptive conditions (fixed platform and ankle stabilization). There was a significant interaction between subject group and visual condition, Menière's patients had a larger RMS sway amplitude than controls with their eyes open and closed, but this difference was abolished by sway feedback. Role of Cervical Spinal Magnetic Stimulation in Improving Posture and Functional Ambulation of Patients with Relapsing Remitting Multiple Sclerosis.

Balance impairment is one of the hallmarks of early MS. Proprioceptive deficit was found to be one of the main causes of this imbalance. The cervical enlargement has a strong proprioceptive system, with its projections to the reticular formation and the central pattern generators, helping in rhythmic pattern generation and alternate leg movements. Repetitive trans-spinal magnetic stimulation (rTSMS) is a noninvasive technique, which can trigger massive proprioceptive afferents. Therefore, it has the potential of improving proprioceptive deficits and motor control. Objective. To determine the e

Intentional Head-Motion Assisted Locomotion for Reducing Cybersickness.

We present an efficient locomotion technique that can reduce cybersickness through aligning the visual and vestibular induced self-motion illusion. Our locomotion technique stimulates proprioception consistent with the visual sense by intentional head motion, which includes both the head's translational movement and yaw rotation. A locomotion event is triggered by the hand-held controller together with an intended physical head motion simultaneously. Based on our method, we further explore the connections between the level of cybersickness and the velocity of self motion through a series of ex…

Transformation from arm joint coordinates to hand external coordinates explains non-uniform precision of hand position sense in horizontal workspace.

People perceive hand position in horizontal workspace more precisely in radial than in azimuth directions and closer to the body than farther away. Current explanations for this position sense non-uniformity include spatial asymmetry in arm proprioceptive activities and/or cortex maps, experience-dependent learning, arm posture, and others. Here we investigated contributions to this non-uniformity of a posture-dependent transformation from arm joint angles, sensed by arm proprioceptors, to hand position. We measured precision of hand position sense in a bimanual hand mirror-position matching t

Posture and posturology, anatomical and physiological profiles: overview and current state of art.

BACKGROUND AND AIM OF WORK: posture is the position of the body in the space, and is controlled by a set of anatomical structures. The maintenance and the control of posture are a set of interactions between muscle-skeletal, visual, vestibular, and skin system. Lately there are numerous studies that correlate the muscle-skeletal and the maintenance of posture. In particular, the correction of defects and obstruction of temporomandibular disorders, seem to have an impact on posture. The aim of this work is to collect information in literature on posture and the influence of the stomatognathic s

Vestibular-Somatosensory Interactions: A Mechanism in Search of a Function?

No unimodal vestibular cortex has been identified in the human brain. Rather, vestibular inputs are strongly integrated with signals from other sensory modalities, such as vision, touch and proprioception. This convergence could reflect an important mechanism for maintaining a perception of the body, including individual body parts, relative to the rest of the environment. Neuroimaging, electrophysiological and psychophysical studies showed evidence for multisensory interactions between vestibular and somatosensory signals. However, no convincing overall theoretical framework has been proposed…

Somatosensory processes subserving perception and action.

The functions of the somatosensory system are multiple. We use tactile input to localize and experience the various qualities of touch, and proprioceptive information to determine the position of different parts of the body with respect to each other, which provides fundamental information for action. Further, tactile exploration of the characteristics of external objects can result in conscious perceptual experience and stimulus or object recognition. Neuroanatomical studies suggest parallel processing as well as serial processing within the cerebral somatosensory system that reflect these se
