Surgical fixation with K-wires versus plaster casting in the treatment of dorsally displaced distal radius fractures: protocol for Distal Radius Acute Fracture Fixation Trial 2 (DRAFFT 2).

INTRODUCTION: Optimal management of distal radius fractures in adults remains controversial. Previous evidence and current clinical guidelines tell us that, if a closed reduction of a dorsally displaced fracture is possible, Kirschner wires (K-wires) are the preferred form of surgical fixation. However, the question remains whether there is any need to perform surgical fixation following a successful closed reduction, or is a simple plaster cast as effective? This is the protocol for a randomised controlled trial of manipulation and surgical fixation with K-wires versus manipulation and castin…

Untreated ulnar styloid fractures do not compromise wrist function in patients with distal radius fractures: A systematic review and meta-analysis.

BACKGROUND: Ulnar styloid fractures (USF) often occur in conjunction with distal radius fractures (DRF). This study aims to compare wrist prognosis between DRF patients with untreated USF and those with isolated DRF, and to assess the impact of different USF fracture types. METHODS: A systematic search was conducted in databases such as PubMed, Embase, and the Cochrane Library, up to January 17, 2026. Randomized controlled trials (RCTs) and cohort studies comparing the wrist joint prognosis of patients with DRF combined with USF and those with isolated DRF were included. The primary outcome m…

Treatment with or without internal fixation for ulnar styloid base fractures accompanied by distal radius fractures fixed with volar locking plate.

The purpose of this study was to evaluate the treatment results, with and without internal fixation of ulnar styloid base fractures associated with acute distal radius fractures. A total of 48 patients were enrolled, including 20 patients treated by internal fixation (fixation group) and 28 treated without internal fixation (non-fixation group). The evaluated parameters were postoperative range of motion, grip strength, DASH score, and the presence or absence of ulnar wrist pain over time. The outcome was assessed as "excellent" in 15 patients of the fixation group and 21 patients of the non-f…

Open reduction and internal fixation of unstable distal radius fractures with a low-profile plate: a multicenter study of 73 fractures.

A study of acute, dorsally displaced, unstable (high-energy) fractures of the distal radius was conducted to determine the safety and efficacy of a new low-profile plate for unstable distal radius fractures. Thirteen surgeons in 11 US cities participated in the study. A minimum follow-up period of 1 year was required to be included in the study. Seventy-three fractures in 71 patients met this criterion. Each fracture was treated according to a prospective protocol. An autogenous bone graft was used in 64 fractures. All procedures were completed using a radiolucent sterile traction table. No ex…

Preoperative distal radius CT hounsfield unit and other imaging predictors of postoperative radiocarpal traumatic arthritis after distal radius fracture.

PURPOSE: Postoperative radiocarpal traumatic arthritis (TA) after distal radius fracture (DRF) may cause persistent pain and functional impairment. Early risk stratification may help identify patients at increased risk. This study aims to identify independent predictors of radiocarpal TA after DRF and develop multifactorial predictive models. METHODS: We retrospectively analyzed 226 DRF patients grouped by radiocarpal TA status. Variables were screened using univariate and correlation analyses. Logistic regression (LR) and support vector machine (SVM) models were developed using a 70/30 strati

Surgical Management of Acute on Chronic Distal Radius Fractures: Correcting Malunion Deformities Through the Secondary Fracture Plane.

It is challenging to restore the clinically acceptable alignment of the distal radius after an acute on chronic fracture or after a secondary fracture occurring after malunion of a primary distal radius fracture. In cases of insignificant primary deformity, restoration to the primary deformity may suffice to obtain a successful clinical result. A borderline acceptable primary radial deformity can be unacceptable after the second injury, resulting in functional disability. If surgery is indicated, the surgeon must contend with both primary and secondary deformities to restore proper distal radi

Dorsally displaced distal radius fractures: introduction of Pacetti's line as radiological measurement to predict dorsal fracture displacement.

BACKGROUND AND AIM OF THE WORK: In the best of our knowledge there is not yet in the literature a measurement able to assess post reduction stability of distal radius fractures. AIM: to study the relationship between our newly introduced Pacetti's line, anatomical reduction of DRFs and post-reduction stability of fractures. METHODS: Patients/Participants: 230 patients (122men, 108women) who sustained a dorsally displaced distal radius fracture. Close reduction procedures attempted; below elbow cast applied. FOLLOW-UP: Pacetti's line used on true AP and lateral view xrays after reduction and…

[The MEU classification system for distal radius fractures: Prognostic and therapeutic value of an independent assessment of various fracture parameters].

Distal radius fractures (DRF) are often complex injuries that can impact the radial metaphysis (M), the radial epiphysis (E) and the distal ulna (U). Each of these parameters can influence the outcome. In a given injury, these three DRF components are involved to a varying degree and are variably associated. The MEU classification independently analyzes the three main bone components of the fracture; thus, all possible combinations and each specific injury can be described. It accurately depicts the type and severity of the DRF. Our results show that this classification is useful for both prog…

A comparison of outcomes after K-wire fixation of distal radius fractures with and without associated ulnar styloid fracture.

PURPOSE: A prospective study was carried out to compare outcomes after K-wire fixation of distal radius fractures without and with associated ulnar styloid fracture, but without fixation of the styloid fracture. PATIENTS AND METHODS: 70 patients, 60 women (86%) and 10 men (14%) with a mean age of 63 years were enrolled. 35 patients had an isolated fracture of the distal radius and 35 had an associated fracture of the ulnar styloid. All patients underwent percutaneous, "augmented" K-wire fixation of the distal radius fracture; the ulnar styloid fracture was left untreated. The patients were fo…

Distal radius fracture with concomitant ulnar styloid fracture: does distal radioulnar joint stability depend on the location of the ulnar styloid fracture?

INTRODUCTION: Studies are conflicting regarding the relationship between ulnar styloid fracture (USF) location and distal radioulnar joint (DRUJ) instability in patients with distal radius fracture (DRF) and concomitant USF. The objective of this study was to determine the association of USF location with TFCC foveal tear and factors associated with DRUJ instability in patients with both DRF and USF. MATERIALS AND METHODS: Fifty-four patients with both DRF and USF who had wrist MRI examination before surgery were analyzed. USF location (tip or base) and TFCC foveal insertion status (intact, p

Distal Radius Fracture Treated with a Modified Kocher-style Dorsolateral Approach, Distal Radius T-plate, and Cannulated Compression Screw Fixation.

Distal radius fractures are common injuries and displaced or unstable patterns frequently require operative stabilization. Volar locking plating remains the predominant method for many unstable distal radius fractures; however, selected radial column or radial styloid-dominant patterns may be amenable to fragment-specific fixation. We report a displaced distal radius fracture treated using a modified Kocher-style dorsolateral wrist exposure with cannulated compression screw fixation augmented by a distal radius T-plate. A 45-year-old, right-hand-dominant, male manual laborer presented after a …

An Analysis of the Impact of Timing and Technique on Outcomes after Surgery for Distal Radius Fractures: The Wrist and Radius Injury Surgical Trial Collaborative Study.

BACKGROUND: Surgical treatment of closed distal radius fractures varies based on treatment, surgeon schedule, and patient preferences. The authors examined how timing and technique impact surgeon-perceived procedural difficulty and quality of reduction, outcomes, and complications. METHODS: This was a retrospective study of participants in the randomized, multicenter Wrist and Radius Injury Surgical Trial with isolated unstable distal radius fractures. Participants were randomized to treatment with a volar locking plate system, closed reduction and percutaneous pinning, or external fixation. …

Older Patient Preferences for Internal Fixation after a Distal Radius Fracture: A Qualitative Study from the Wrist and Radius Injury Surgical Trial.

BACKGROUND: Distal radius fracture treatments provide similar functional outcomes. It has been hypothesized that the use of internal fixation is increasing because of physician preferences. The multisite randomized Wrist and Radius Injury Surgical Trial provides a unique opportunity to examine patient preferences in the absence of surgeon influence. The authors' objective was to investigate patient preference for internal fixation even after being informed of the equipoise among treatments. METHODS: The authors performed 30 semistructured interviews with older individuals, all older than 60 y
