Regenesorb and polylactic acid hydroxyapatite anchors are associated with similar osseous integration and rotator cuff healing at 2 years.

BACKGROUND: Commercially available suture anchors for rotator cuff repairs can differ significantly in architecture and material. Clinical data on their osseous integration and its effect on patient-reported outcomes is scarce. Preclinical investigations indicated a higher rate of osseous integration for the open-architecture design of the Healicoil Regenesorb anchor than the closed-threaded design of the Twinfix (Smith & Nephew). The purpose of this study was to investigate these 2 anchors with different architecture and material to determine their effect on osseous integration and clinical o

Superior capsular reconstruction - A systematic review and meta-analysis.

BACKGROUND: Since Mihata's 2012 proposal to arthroscopically reconstruct the superior capsule of patients with massive irreparable cuff tears, many studies have reported the clinical results of this technique using different types of grafts (fascia lata autograft, dermal allograft, porcine dermal xenograft or long head of biceps autograft). PURPOSE: The objective of this meta-analysis was to report the clinical and radiological results of these superior capsule reconstructions. METHODS: Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA) recommendations were used to c

Primary Arthroscopic Repair of a Traumatic Isolated Subscapularis Tendon Rupture in an Adolescent Patient.

Subscapularis tendon tears in the pediatric population are most commonly associated with an avulsion fracture of the lesser tuberosity. Isolated subscapularis tendon ruptures are infrequently reported. According to previous reports, the diagnosis of subscapularis tendon injuries in pediatric patients is often delayed and management is typically conservative. When operative management is indicated, an open deltopectoral approach has been used and may include concomitant open reduction and internal fixation of the lesser tuberosity. The authors report the case of a healthy 15-year-old boy who ha

Human Rotator Cuff Tears Have an Endogenous, Inducible Stem Cell Source Capable of Improving Muscle Quality and Function After Rotator Cuff Repair.

BACKGROUND: The muscle quality of the rotator cuff (RC), measured by atrophy and fatty infiltration (FI), is a key determinant of outcomes in RC injury and repair. The ability to regenerate muscle after repair has been shown to be limited. PURPOSE: To determine if there is a source of resident endogenous stem cells, fibroadipogenic progenitor cells (FAPs), within RC injury patients, and if these cells are capable of adipogenic, fibrogenic, and pro-myogenic differentiation. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 20 patients between the ages of 40 and 75 years with par

Feasibility study: one year fortnightly follow-up of the evolution of supra-spinatus degeneration via text-messages.

BACKGROUND: The clinical follow-up of patients for degeneration of the supraspinatus tendon is limited by the lack of objective assessment of pain evolution over time. We therefore tested a new method to collect follow-up data on patients treated either by surgical cuff repair or rehabilitation. OBJECTIVES: We report the feasibility this method in terms of recruitment of clinicians and patients and their compliance. METHODS: In this multicenter longitudinal observational study, between September 2015 and March 2019, patients consulting either for surgical repair or rehabilitation were examin

Chronicity is associated with the glenohumeral synovitis in patients with a rotator cuff tear.

Synovitis of the glenohumeral joint (GHJ) and subacromial space (SAS) is one of the most common findings during arthroscopic rotator cuff repair (RCR). The purpose of this study is to determine clinical factors associated with the degree of synovitis in patients with a rotator cuff tear and whether macroscopic synovitis affects early clinical outcomes following arthroscopic RCR. Arthroscopic videos of 230 patients treated with arthroscopic RCR were randomly reviewed by two experienced shoulder surgeons. The synovitis scores of the GHJ using Davis's grading system and the SAS using Jo's grading

Evaluating the role of subacromial impingement in rotator cuff tendinopathy: Development and analysis of a novel murine model.

Subacromial impingement of the rotator cuff is understood as a contributing factor in the development of rotator cuff tendinopathy. However, changes that occur in the impinged tendon are poorly understood and warrant further study. To enable further study of rotator cuff tendinopathy, we performed a controlled laboratory study to determine feasibility and baseline characteristics of a new murine model for subacromial impingement. This model involves surgically inserting a microvascular clip into the subacromial space in adult C57Bl/6 mice. Along with a sham surgery arm, 90 study animals were d

A larger critical shoulder angle requires more rotator cuff activity to preserve joint stability.

Shoulders with rotator cuff tears (RCT) tears are associated with significantly larger critical shoulder angles (CSA) (RCT CSA = 38.2°) than shoulders without RCT (CSA = 32.9°). We hypothesized that larger CSAs increase the ratio of glenohumeral joint shear to joint compression forces, requiring substantially increased compensatory supraspinatus loads to stabilize the arm in abduction. A previously established three dimensional (3D) finite element (FE) model was used. Two acromion shapes mimicked the mean CSA of 38.2° found in patients with RCT and that of a normal CSA (32.9°). In a first step

Fracture of a reverse total shoulder arthroplasty retentive liner.

Reverse shoulder arthroplasty may be indicated in shoulders that cannot be effectively managed by conventional reconstruction procedures. Reported complications include instability, infection, hematoma, scapular notching, and premature component loosening. To our knowledge, however, fracture of a humeral component polyethylene inlay has not been described. A 72-year-old man presented with unstable, painful shoulders after undergoing unsuccessful open repairs and acromioplasties at an outside institution for bilateral massive rotator cuff tears. We performed a left reversed shoulder arthroplast

Biomechanical characteristics of single-row repair in comparison to double-row repair with consideration of the suture configuration and suture material.

The aim of the study was to evaluate the time zero mechanical properties of single- versus double-row configuration for rotator cuff repair in an animal model with consideration of the stitch technique and suture material. Thirty-two fresh-frozen sheep shoulders were randomly assigned to four repair groups: suture anchor single-row repair coupled with (1) braided, nonabsorbable polyester suture sized USP No. 2 (SRAE) or (2) braided polyblend polyethylene suture sized No. 2 (SRAH). The double-row repair was coupled with (3) USP No. 2 (DRAE) or (4) braided polyblend polyethylene suture No. 2 (DR

Rotator cuff repair: an analysis of utility scores and cost-effectiveness.

More than 75,000 rotator cuff repairs are performed annually, yet cost-effectiveness data are unavailable. This study examines the cost utility of rotator cuff repair by relating surgical costs to increase in quality-adjusted life-years (QALYs). Eighty-seven patients were followed up prospectively for 1 year, during which cost and quality of life data were collected. Patient-based utility measures of quality of life (European Quality-of-Life measure [EuroQoL] and Health Utility Index [HUI]) were administered. Changes in these measures generated net QALYs. Finally, life expectancies were applie

Irreparable rotator cuff tears: what to do and when to do it; the surgeon's dilemma.

Irreparable rotator cuff tears have been defined as those tears that because of their size and retraction cannot be repaired primarily. Patients with an irreparable tear present with a variety of symptoms and physical findings, and their management depends on the clinical situation and the specific location of the tear. Most of these tears occur in the older, less active patient and many of these patients are best treated without surgery. For those in whom surgery is indicated, the best procedure should be tailored to the tear.

Isolated arthroscopic biceps tenotomy or tenodesis improves symptoms in patients with massive irreparable rotator cuff tears.

BACKGROUND: Lesions of the long head of the biceps tendon are often associated with massive rotator cuff tears and may be responsible for shoulder pain and dysfunction. The purpose of this study was to evaluate the clinical and radiographic outcomes of isolated arthroscopic biceps tenotomy or tenodesis as treatment for persistent shoulder pain and dysfunction due to an irreparable rotator cuff tear associated with a biceps lesion. METHODS: We conducted a retrospective study of sixty-eight consecutive patients (mean age [and standard deviation], 68 +/- 6 years) in whom a total of seventy-two i

Humeral osteochondroma causing a subscapularis tear: a rare source of shoulder dysfunction.

The proximal humerus is a well-known site of predilection for the solitary osteochondroma-the most common benign bone tumor. Subscapularis tears were underreported in the pre-arthroscopic era. Literature about rotator cuff impairment caused by osteochondromas is almost non-existent. We will present the case of a young man, who had a complete non-traumatic subscapularis tear, where the tendon had been peeled off the lesser tuberosity by an osteochondroma. Diagnostic features such as MRI and arthroscopy are emphasized. The patient was successfully treated by open resection of the tumor and reatt

Measurement properties of the Western Ontario rotator cuff outcome measure: a preliminary report.

The purpose of this prospective repeated-measures outcomes study was to investigate the construct validity and sensitivity to change of a newly developed outcome measure, the Western Ontario Rotator Cuff Index (WORC) in a population of patients seen at a tertiary shoulder center. A total of 154 subjects (66 women and 88 men; mean age, 48 years [SD, 14.80 years]) agreed to participate in the study. Of these, 50 patients (16 women and 34 men; mean age, 50 years [SD, 14.36 years]) met the criteria for surgery. The Pearson correlation coefficients between the initial scores and the change scores a

Results of surgery after failed attempt at repair of irreparable rotator cuff tear.

Twelve patients who had surgery to repair a rotator cuff tear, which could not be repaired because the residual tendon tissue was of poor quality, insufficient, or both, were followed up for a mean of 33 months. At surgery, the deltoid was detached from the acromion for 4 to 6 cm and an acromioplasty was done. Preoperatively, all patients except one had mild to severe pain. The mean active forward flexion, abduction, and external rotation were 75 degrees, 61 degrees, and 22 degrees, respectively. After surgery, of the 11 patients who had shoulder pain preoperatively, 10 reported partial relief

Clinical outcome after structural failure of rotator cuff repairs.

BACKGROUND: The clinical outcome for patients with documented rerupture after open repair of one or more rotator cuff tendons is not well known. The purpose of this study was to evaluate the clinical outcomes of a consecutive series of rotator cuff reruptures after repair and to provide information concerning the advisability of rotator cuff repair in situations in which there may be a high probability of rerupture. METHODS: During prospective follow-up after rotator cuff repairs, we detected, with magnetic resonance imaging, structural failure of the repair in twenty patients, who had a mean
