Double-Screw Osteosynthesis in an Unstable Scaphoid Fracture Model: A Biomechanical Comparison of Two Screw Configurations.

PURPOSE: Although there is evidence that a single headless compression screw is sufficient for fixation of most scaphoid fractures, double-screw osteosynthesis has been shown to result in higher failure strength and stiffness than a single screw. However, the biomechanical effect of different screw configurations has not been determined. METHODS: A standardized unstable fracture model was produced in 28 cadaveric scaphoids. Specimens were randomly allocated to 1 of 2 fixation groups using 2 internal compression screws positioned in either the sagittal or coronal plane. A specimen-specific 3-d

Surgical Management of Scaphotrapeziotrapezoid Arthritis.

Scaphotrapeziotrapezoid (STT) arthritis occurs commonly with basal joint arthritis, but can also occur in isolation or in conjunction with other patterns of wrist arthritis, such as scapholunate advanced collapse. Surgical options depend on the specific clinical scenario encountered. Isolated STT arthritis was classically managed with arthrodesis, but is now often addressed with distal scaphoid resection (open or arthroscopic), trapeziectomy (partial or complete) and partial trapezoid resection, or implant arthroplasty. Development of postoperative dorsal intercalary segment instability is a n

Conservative treatment of scaphoid fractures: a follow up study.

BACKGROUND AND AIMS: This investigation was made to elucidate the outcome of conservatively treated scaphoid fractures. MATERIAL AND METHODS: 63 patients with a scaphoid fracture were treated initially conservatively and examined retrospectively. The mean follow up time was 54 months. RESULTS: In this group bone union was achieved in 56 cases (89%). Three of the patients ended up to operation because of delayed union soon after conservative treatment and three nonunions were found in the follow up appointment. One fracture did not heal because of dislocation during cast treatment and was tre

Acute rotary dislocation of the scaphoid: a new technique of repair using Herbert screw fixation across the scapho-lunate joint.

Acute rotary dislocation of the scaphoid is commonly misdiagnosed and, if untreated, leads to progressive osteoarthritis of the wrist. The diagnosis requires a high degree of awareness and an understanding of the pathomechanics of the injury. To date, no single method of treatment has been shown to produce consistently satisfactory results. This paper presents a new surgical technique involving open reduction and reattachment of the ligament to the bone, combined with Herbert screw fixation across the scapho-lunate joint. The screw is normally left in situ for 12-18 months, allowing sufficient

Inter- and Intraobserver Reliability for the Computed Tomography Scan Assessment of Union After Surgery for Scaphoid Fractures and Nonunion.

PURPOSE: Assessment of scaphoid union after operative treatment might be associated with uncertainty, disagreement, and misinterpretation. Union is defined as consolidation on three of four X-ray views. Computed tomography (CT) scans are increasingly used to evaluate union of scaphoid fractures and nonunion. It allows multiplanar assessment of the scaphoid. Reliability in CT scan assessment of union after operative intervention is limited. We hypothesized that inter- and intraobserver reliability of the CT scan assessment of union after operative treatment for scaphoid fracture and nonunion ar

Intercarpal degenerative arthritis in adulthood as a late consequence of unilateral congenital aplasia of the scaphoid: a case report.

We report a case of unilateral congenital aplasia of the carpal scaphoid bone discovered in adulthood, which probably caused a severe painful intercarpal degenerative arthritis. This case was not associated with hypoplasia or aplasia of thenar and forearm muscles, abnormality of the skeleton of the thumb ray, absence of the sesamoid bones, hypoplasia of the forearm bones, as in cases reported in the literature; the only additional anatomic abnormality was a mild hypoplasia of the radial styloid. A surgical treatment (4-bone arthrodesis) has been performed to treat the advanced carpal collapse …

Risk Factors That Cause Delayed Union after Surgery for Acute Scaphoid Fracture.

Background: It is important to prevent delayed union after surgical treatment for acute scaphoid fractures; however, risk factors remain to be elucidated. We investigated the risk factors for delayed union in cases of scaphoid fractures with surgical treatment at our hospital. Methods: We enrolled 29 patients with scaphoid fracture who underwent percutaneous screw fixation at Juntendo University Hospital between January 2015 and February 2024. Patients were divided into a U group consisting of those who achieved bone union at 6 months after surgery, and a D group consisting of those who had de

[Does low intensity, pulsed ultrasound speed healing of scaphoid fractures?].

Since pulsed low-intensity ultrasound (frequency: 1.5 MHz, pulsed by 1 kHz, signal burst width: 200 microseconds, intensity: 30 mW/cm2) has been proven to stimulate fracture healing both clinically and experimentally, our question was whether this therapy also accelerates healing of fresh stable scaphoid fractures. Addressing this question, we did the following prospective randomized clinical trial. Regarding the results of former clinical fresh fracture studies by Heckman and Kristiansen, we postulated that low intensity ultrasound accelerates healing by about 30%. Based on this thesis, we ca

Diagnostic value of ultrasound in scaphoid fractures.

Diagnostic ultrasound was used in 103 patients with a clinically suspected fracture of the scaphoid bone in order to achieve an early diagnosis. There were 48 women and 55 men aged 10-75 years (mean 31.4 years) tested with ultrasound. Of the patients, 72 were tested within 3 days after injury the remainder between 4 and 42 days. Fracture was confirmed radiologically in 27. We found that the ultrasound test, applied with a frequency of 1 MHz and intensity of 0.5 W/cm2 and 2.0 W/cm2 for 30s, had a sensitivity of 37 per cent and a specificity of 61 per cent. We thus conclude that ultrasound is no…

Radiographic Scapholunate Interval in the Pediatric Population Decreases in Size as Age Increases.

BACKGROUND: Widening of the scapholunate (SL) interval greater than 2 mm is diagnostic of an injury in adults; however, this absolute number cannot be used for skeletally immature individuals due to the ossification of the carpal bones. The purpose of this study was to determine age-appropriate normative values for the radiographic SL interval in the skeletally immature population. METHODS: Normal pediatric wrist radiographs were identified. The radiographic SL interval was measured as the distance between the scaphoid and the lunate at the mid-joint space between the scaphoid and the lunate,

The role of surgical timing in 124 scaphoid nonunion surgical procedures.

A total of 124 scaphoid nonunion surgical procedures performed between 2002 and 2020 were analyzed to investigate the potential impact of delayed diagnosis on time to union. The primary outcome was the correlation between the time from injury to surgery and the time to union. Secondary outcomes included identifying risk factors for delayed union and persistent nonunion following scaphoid nonunion surgery. The analysis revealed that time to union was similar for surgeries performed between 6 months and over 2 years post-injury. However, nonunion persisted in 12 cases, five of which involved sur

Volar Vascularized Strut Graft for Avascular Scaphoid Nonunion Using the 1,2 Intercompartmental Supraretinacular Artery.

In this retrospective study, we report the preliminary results of a novel technique for volar vascularized strut grafting to treat avascular scaphoid nonunion by using the 1,2 intercompartmental supraretinacular artery through a single incision. Forty-three of 45 patients with avascular scaphoid nonunion healed at a mean of 13 weeks (range, 3 to 10 mo). Complications consisted of 1 pin tract infection that resolved with oral antibiotics and 4 cases of transient dysesthesia of the radial sensory nerve. In 4 patients with equivocal radiographs, computed tomography scans confirmed bony union. The
