[Radiation-induced morphea].

Although the aetiology of morphea often remains unknown, various precipitating causes have been identified. Morphea is a rare development following irradiation. We observed a circumscribed scleroderma 14 years after x-ray-radiotherapy following surgery of breast cancer. Furthermore multiple weeping papules occurred within the radiation area. A relapse of the breast cancer was ruled out by histological examination which displayed a transepidermal elimination of acid mucin. The irradiation had obviously induced different reactions of the dermal fibroblasts (radiodermatitis, morphea, mucin produc

[Eye involvement in circumscript scleroderma--manifestation of borreliosis?].

BACKGROUND: The circumscript cutaneous sclerosis is an inflammato-edematous erythema of the skin, usually leading to a plaque-like sclerosis and cutaneous atrophy. The etiology of this disease remains unknown, but some authors presume a systemic infection with Borrelia burgdorferi as the underlying pathological condition. PATIENT: We present a case of a 66-year-old male patient who suffered from episcleritis of his left eye. Funduscopy showed an unilateral papillary edema. Visual acuity was 1.0. The patient had multiple erythemata of the skin of the body-trunk and the arms. On histological ex

Response to methotrexate in a patient with idiopathic eosinophilic fasciitis, morphea, IgM hypergammaglobulinemia, and renal involvement.

A 35-year-old man with idiopathic eosinophilic fasciitis (EF) and morphea developed renal disease characterized by microscopic hematuria, nephrotic range proteinuria, and rapidly progressing hypertension, an association that has not previously been reported in EF. Initial clinical symptoms of EF began in July 1989; peripheral eosinophilia peaked at 30% in August 1990; an abnormal urinalysis was first observed in March 1992 and subsequently a renal biopsy was performed. Renal biopsy demonstrated focal segmental glomerulosclerosis and a subepithelial immune-type deposit. Partial fasciectomy and

Palmoplantar keratoderma with sclerodactyly (Huriez syndrome).

A syndrome characterized by palmoplantar keratoderma, sclerodactyly, and skin cancer was first described in two families by Huriez et al. The pattern of inheritance was compatible with that of an autosomal dominant disorder. We report a patient with this condition and review the literature.

Diagnosis of Lyme disease based on dermatologic manifestations.

Lyme disease, or Lyme borreliosis, is an infection caused by the spirochete Borrelia burgdorferi, which is most commonly transmitted to humans by a tick bite. Characterized by early and late phases, Lyme disease is a multisystem illness involving the skin, heart, joints, and nervous system. Diagnosis is based predominantly on clinical manifestations, the most specific being dermatologic. Thus, recognizing the dermatologic manifestations of Lyme disease is important for diagnosis and institution of appropriate, effective therapy. Approximately 75% of patients with Lyme disease present with the

Linear scleroderma associated with ptosis and motility disorders.

A case is reported in which an 11-year-old girl developed progressive ptosis and a subsequent motility disorder of the right eye. The diagnosis linear scleroderma en coup de sabre was established. Atrophy of the upper levator palpebral and superior rectus muscle could be shown on CT scan.

[Scleromyxedema therapy with isotretinoin].

We report on a 56-year-old male patient suffering from scleromyxedema, who was successfully treated with isotretinoin (13-cis retinoic acid, Roaccutan). After 10 months therapy, we observed considerable reduction of the cutaneous infiltration and the skin thickening; the papular eruptions had almost completely disappeared. The mobility of the joints, however, could only be slightly improved.

Juvenile dermatomyositis: histological findings and pathogenetic hypothesis for the associated skin changes.

Skin biopsies from patients with scleroderma and juvenile dermatomyositis (DM) share many histologic features. Characteristics common to both diseases are particularly evident in the dermal microvasculature and include endothelial swelling and concentric thickening of the vascular basement membrane. Biopsies performed on 3 patients with the severe vasculitic form of juvenile DM showed these changes as well as dropout of vessels and linear deposition of collagen. The latter findings, seen late in the course of the disease, are indistinguishable from those of advanced scleroderma. A hypothesis i

A case of primary biliary cirrhosis associated with generalized morphea.

A 50-year-old female with primary biliary cirrhosis associated with generalized morphea is reported. She had suffered from multiple painful indurated plaques on the trunk which was diagnosed as generalized morphea; subsequently, primary biliary cirrhosis was diagnosed on the basis of her hepatic dysfunction. Although progressive systemic sclerosis has been reported to occur in patients with primary biliary cirrhosis, this localized form of cutaneous sclerosis has not previously been reported. Since generalized morphea is one of the most common manifestations of chronic graft-versus-host diseas

[Lichen myxedematosus and scleromyxedema].

Review of the clinical picture, dynamics of the disease, associated symptoms and signs, etiopathology as well as treatment and prognosis of lichen myxedematosus and scleromyxedema. Diagnostic criteria are wax-like papules and leather-like skin thickening, an increase in concentration of acid mucopolysaccharides consisting mainly of hyaluronic acid in the dermis, lympho-plasmacytoid infiltrates in the skin and the bone marrow, normal thyroid function, and paraproteinemia of light chain type lambda or kappa. The nosologic connection of both manifestations implying a single disease entity is emph

Diffuse fasciitis with eosinophilia in childhood.

Diffuse fasciitis with eosinophilia is a newly recognized connective tissue disease appearing initially to be either scleroderma or dermatomyositis. It has clinical and histologic features that allow it to be clearly separated from both disorders and has a more favorable prognosis. The majority of reported cases have been in adults. The disease involves the extremities, and at times the trunk, diffusely. One sees firm, often puckered or rivuleted skin, tightly bound to underlying structures; contractures result in several weeks. Early peripheral blood features are eosinophilia and elevated imm

Pterygium inversum unguis-like changes in scleroderma. Report of four cases.

During an 18-month period, four patients with scleroderma were found to have nail findings suggestive or pterygium inversum unguis, a recently described condition. In the same time period, two other examples of the disorder were seen. Pterygium inversum unguis-like change may represent an additional nail sign in scleroderma. There are possible mechanisms for its development in patients with Raynaud's phenomenon, connective tissue diseases, or peripheral vascular disease.

HLA-DR antigens in progressive systemic sclerosis (scleroderma).

Typing at the HLA-DR locus was performed in 125 North American white patients with progressive systemic sclerosis (PSS). We could not confirm associations of PSS with the HLA-DR5 antigen or the HLA-B8/DR3 haplotype. A weak association of the DR1 antigen and PSS with diffuse scleroderma (27.5% vs 11.5% in local controls (corrected p less than 0.05) was observed. HLA-DR1 was significantly associated with the presence of anticentromere antibodies (p less than 0.05), and this combination was found more frequently in individuals with the CREST syndrome variant of PSS. This study failed to identify

[Morphologic study of the microcirculation in connective tissue diseases].

Capillaroscopy is a non-invasive diagnostic test used to study microvascular abnormalities which are present in many disorders, particularly some rheumatic pathologies, such as connective tissue diseases. In systemic sclerosis, capillaroscopy allows detection of pathognomonic microvascular alterations. In other connective tissue diseases, including systemic lupus erythematosus, dermatopolymyositis, undifferentiated connective tissue diseases, and mixed connective tissue disease, the capillaroscopic patterns, although non-specific, can provide a valid support for the diagnosis. In Raynaud's phe
