[Surgical treatment of posterior shoulder dislocation: importance of arthroscopy].

OBJECTIVE: Posterior dislocations of the shoulder represent a rare injury with frequently occurring soft tissue and/or bony concomitant lesions such as the reverse Hill-Sachs lesion and the dorsal labrum tear. For the combination of these injuries, no evidence-based therapeutic recommendations exist. AIM OF STUDY: Reflecting on two clinical cases and the current literature data, options for the treatment of combined osseous and soft tissue injuries due to posterior dislocation of the shoulder are presented. METHODS: We report two cases of fresh traumatic first-time posterior dislocations tha

A Magnetized Brace Designed to Elevate the Extremity after Hand Trauma and Surgery.

Hand elevation is a common recommendation for the treatment of hand injuries to combat development of painful swelling. However, noncompliance with elevation after hand procedures is common, and this is due in part to the lack of acceptable orthotics to assist in this elevation. Conventional slings are often not recommended due to potential shoulder morbidity. Additionally, the standard sling design allows for the hand to fall in a dependent position below the level of the heart. Herein described is a brace that uses magnetic technology designed to provide support for hand elevation that is in

Bilateral luxatio erecta humeri and bilateral knee dislocations in the same patient.

Bilateral knee dislocations seldom occur, and fewer than a dozen cases of bilateral luxatio erecta humeri (erect dislocation of the shoulder) have been reported. Here we present the case of a patient who sustained bilateral knee dislocations and bilateral luxatio erecta humeri in a motorcycle accident. Transient neurologic compromise resolved with closed reduction of the bilateral shoulder and left knee dislocations. To our knowledge, this is the first case report of a patient with this combination of injuries.

Neurologic injuries in baseball players.

In baseball pitchers, injuries to the throwing arm are common due to the extreme stresses placed on the elbow and shoulder joints. These result in peripheral nerve syndromes including ulnar neuropathy at the elbow and suprascapular neuropathy at the shoulder. Recurrent trauma to the axillary artery causing aneurysm and thrombus formation may lead to distal ischemia and stroke. Careful evaluation is required to identify musculoskeletal, neurologic, and vascular causes of upper extremity symptoms in the throwing athlete.

Recurrent posterior shoulder instability.

Recurrent posterior shoulder instability is an uncommon condition. It is often unrecognized, leading to incorrect diagnoses, delays in diagnosis, and even missed diagnoses. Posterior instability encompasses a wide spectrum of pathology, ranging from unidirectional posterior subluxation to multidirectional instability to locked posterior dislocations. Nonsurgical treatment of posterior shoulder instability is successful in most cases; however, surgical intervention is indicated when conservative treatment fails. For optimal results, the surgeon must accurately define the pattern of instability

Ergonomics in Dermatologic Procedures: Mobility Exercises to Incorporate In and Out of the Office.

Dermatologists-particularly dermatologic surgeons-commonly experience neck, shoulder, and back pain due to prolonged static positions and repetitive motions inherent in their practice. Musculoskeletal injuries not only can impact career longevity and satisfaction but also can affect the quality of patient care provided. We highlight mobility techniques and exercises that dermatologic surgeons can incorporate both in and outside the procedure room to reduce pain and prevent future injuries.

Upper extremity injuries in adolescent tennis players.

The majority of adolescent tennis players will have their playing temporarily interrupted by an injury, with lower extremity problems predominating. Although upper extremity injuries occur less frequently, they tend to be more troublesome. Injuries discussed in this article include "King Kong arm," slipped capital humeral epiphysis, "Osgood-Schlatter disease" of the shoulder, tennis elbow, stress synovitis and elbow flexion contracture, and friction burns and collagen stress fractures of the rotator cuff.

Radiology in Sportsmedicine.

In brief: A group of radiologists and orthopedic surgeons discusses the use of x-rays, CAT scans, bone scans, arthrograms, and xeroradiograms in diagnosing sports injuries. Head, spine, knee, and ankle injuries, spleen ruptures, shoulder separations, and stress fractures are covered. The panelists agree that radiologists and sports physicians should maintain good working relationships so that examinations can be tailored to each athlete using the least number of films possible.

A Case of Simultaneous Bilateral Anterior Shoulder Dislocation.

INTRODUCTION: Anterior dislocation of shoulder is commonest dislocation one encounters in day to day Orthopaedic practice. But bilateral shoulder dislocations are relatively uncommon frequently posterior and secondary to violent muscle contraction. Simultaneous bilateral anterior dislocations of shoulder following trauma is rare occurrence. CASE REPORT: 35 year old male presented to emergency department with history fall by tripping on a stone (fall on outstretched hand). He complained of pain and difficulty in moving both the shoulders. On clinical examination, patient's both upper limbs wer

The Latarjet Procedure For Anterior Shoulder Instability : A Consecutive Prospective Series Of 50 Cases.

The Latarjet is a successful primary and revision option for anterior shoulder instability. However, recent reports have highlighted varying complication rates. Our study prospectively collected clinical, functional and radiological outcomes of patients undergoing the procedure. Forty-eight consecutive patients (fifty shoulders) underwent the Latarjet procedure in a single UK centre. Clinical, radiological and functional follow-up was performed. Mean clinical follow-up was 32 months and radiological follow-up 20 months. 95% shoulders were subjectively graded "excellent" or "good" and 5% "fair"

The Hill-Sachs lesion: diagnosis, classification, and management.

The Hill-Sachs lesion is an osseous defect of the humeral head that is typically associated with anterior shoulder instability. The incidence of these lesions in the setting of glenohumeral instability is relatively high and approaches 100% in persons with recurrent anterior shoulder instability. Reverse Hill-Sachs lesion has been described in patients with posterior shoulder instability. Glenoid bone loss is typically associated with the Hill-Sachs lesion in patients with recurrent anterior shoulder instability. The lesion is a bipolar injury, and identification of concomitant glenoid bone lo

The impact of upper-extremity injuries on polytrauma patients at a level 1 trauma center.

BACKGROUND: Upper-extremity injuries often lead to long-term problems in function and quality of life in patients. However, not much is known about the effects in polytrauma patients. This study aimed to describe the upper-extremity injuries in polytrauma patients and to compare self-reported disability and quality of life in polytrauma patients with vs. without upper-extremity injuries. METHODS: We performed a retrospective cohort study of adult patients with an Injury Severity Score ≥ 16 admitted to Erasmus MC between January 1, 2007, and December 31, 2016. Patients were asked to complete t

Epidemiology of acromioclavicular joint injuries in professional baseball: analysis from the Major League Baseball Health and Injury Tracking System.

BACKGROUND: Shoulder injuries account for a large portion of all recorded injuries in professional baseball. Much is known about other shoulder pathologies in the overhead athlete, but the incidence and impact of acromioclavicular (AC) joint injuries in this population are unknown. We examined the epidemiology of AC joint injuries in Major League Baseball (MLB) and Minor League Baseball (MiLB) players and determined the impact on time missed. METHODS: The MLB Health and Injury Tracking System was used to compile records of all MLB and MiLB players from 2011 to 2017 with documented AC joint in…

Open repair of retracted latissimus dorsi tendon avulsion.

Latissimus dorsi avulsion injuries are rarely reported in the literature and are managed with a variety of strategies. Primary anatomical repair of tendon to bone may offer athletes the best chance for successful return to sports. In this article, we describe a surgical technique for safely repairing an acute or chronic, retracted, avulsed latissimus tendon back to its insertion on the medial aspect of the bicipital groove of the proximal humerus. Using 1 low anterior axillary incision and 1 posterior axillary incision for tendon retrieval when retraction is more than 5 cm, this technique allo

Type 4 Acromioclavicular Joint Separation Reconstruction With Suspensory Fixation, Allograft, and Suture Augmentation.

BACKGROUND: This is a case of surgical treatment of a type 4 acromioclavicular (AC) joint separation via an arthroscopically assisted approach and reconstruction with combined suspensory fixation, allograft ligament reconstruction, and suture augmentation. AC joint injuries are relatively common, representing approximately 9% of shoulder injuries and approximately 50% of sport-related shoulder injuries. Most AC joint injuries do well when treated conservatively. However, the literature supports surgical treatment for high-grade injuries, failed conservative treatment, and open injuries or thos…

Vascular trauma associated with fractures and dislocations.

Vascular trauma occurs relatively infrequently in association with general orthopedic trauma but may be seen more often in injuries involving joint dislocations and areas in which vascular structures are tethered at the fracture site. The mechanisms of vascular trauma are identified, and the general principles in managing these injuries, including the operative approach to the injury itself and the options in repairing both arterial and venous injuries, are discussed. The role of fasciotomy and primary amputation are also reviewed. Several specific injuries, including pelvic fracture, knee dis

Clinical and functional outcomes of open reduction and internal fixation for scapular fractures using the modified Judet approach.

OBJECTIVE: Displaced scapular fractures are uncommon injuries, and the optimal surgical approach remains controversial. Muscle-preserving techniques may improve post-operative shoulder function by minimizing soft-tissue damage. The purpose of the study was to evaluate the clinical and functional outcomes of open reduction and internal fixation (ORIF) using a muscle-preserving modified Judet approach in displaced scapular fractures and to assess the effect of surgical timing on post-operative shoulder function. METHODS: This retrospective study included 12 adult patients who underwent ORIF for

Prevalence of HAGL lesions and associated abnormalities on shoulder MR examination.

OBJECTIVE: Humeral avulsion of the glenohumeral ligament (HAGL) is an uncommon shoulder injury. We report the prevalence of HAGL lesions and other associated shoulder injuries in a large series of shoulder MR examinations. All results were correlated with surgery. MATERIALS AND METHODS: MR reports of 1,000 consecutive conventional shoulder MR exams performed on patients with shoulder pain were reviewed in our information system for the word HAGL. A total of 743 patients went on to surgery. There were 23 HAGL lesions reported at surgery. Those 23 examinations were reviewed retrospectively in c…

Axillary nerve injury after anterior glenohumeral dislocation: MR findings in three patients.

OBJECTIVE: We performed this study to demonstrate the MR imaging findings that indicate an anterior dislocation has caused an axillary nerve injury. CONCLUSION: MR images of the shoulder can show findings indicating that an axillary nerve injury has been caused by an anterior shoulder dislocation. All MR examinations should be evaluated for these findings, particularly if the patient has a history of anterior shoulder dislocation.
