Cognitive/behavioral therapy of anxiety in the medically ill: cardiac settings.

Anxiety appears to be a strong risk factor for ischemic heart disease (IHD) and specifically fatal IHD. However, no randomly assigned, controlled, clinical trial targeting anxiety yet exists demonstrating an impact on objective cardiac outcomes. Situational anxiety is frequent in cardiac populations and can diminish quality-of-life by increasing symptoms/disability and result in unnecessary medical system utilization. "Noncardiac" chest pain is common both in patients with objective coronary disease and in patients whose cardiac workup is negative. Both presentations of chest pain respond to c

Psychogenic parasitosis. A case series and literature review.

About 100 years ago, psychogenic parasitosis was first described in the literature. This peculiar symptom may complicate medical and psychiatric conditions or indicate the presence of a delusional disorder (somatic type). By using the authors' case series of 12 patients, which are reported in the article, and a review of the literature, an historical perspective and the authors' clinical orientation to evaluation and treatment of psychogenic parasitosis are presented.

[Psychological sequelae of accidents. Symptoms are seldom recognized].

OBJECTIVE: To present the problems possibly associated with psychological sequelae of accidents and make proposals for appropriate therapy. MAJOR POINTS: Accidents experienced by the victim as threatening, whether associated with immediate physical or mental impairment or not, often trigger multiple psychological reactions, and may lead to psychological disorders of either a temporary or a permanent nature. Accident-related psychological symptoms or problems are almost regularly overlooked during the acute medical treatment phase. Not until psychological symptoms become evident are psycho-dia

The impact of unemployment on psychosomatic symptoms and mental well-being.

The relationship between psychosomatic symptoms and mental well-being among unemployed (N = 132) and employed (N = 187) personnel of two similar wood-processing factories was studied. Mental well-being was measured by means of the 12-item General Health Questionnaire (GHQ-score) and the 13-item Beck Depression Inventory (BDI-score). The 13-item questionnaire assessing psychosomatic symptoms was used (PS-score). The 33 and 66 percentiles (tertiles) were formed on the basis of PS-score of the two groups separately. The highest PS-score percentile was compared with the combined first and second p

[Indications for psychotherapeutic treatment of functional dysphonias].

Functional voice disorders require adequate psychiatric screening and treatment due to the complex etiologies of the psychic elements involved. While the diagnosis of dysphonia increasingly takes into account various emotional factors, considerations of indications for psychotherapy and for differing forms of psychotherapy are missing in the literature. To fill this gap, the aims and principles of a number of psychoanalytically based therapeutic methods are presented. Criteria for general indications for treatment and with respect to patients with functional voice disorders are outlined. Case

Behavior modification of abnormal gait and chronic pain secondary to somatization disorder.

An interdisciplinary team, which included a physiatrist, psychologist, physical therapist, occupational therapist, social worker, and nursing staff, undertook the treatment of a 33-year-old woman with a 16-year history of gait problems and multiple somatic complaints. Previously, she had been followed by a number of physicians and had undergone both invasive and noninvasive diagnostic procedures as well as several surgical procedures. After limited response to such treatment, she was referred to the outpatient PM&R clinic for evaluation. Physical and psychologic study led to a primary diagnosi

Delusional infestation in psychodermatology.

Delusional infestation (DI), also known as delusional parasitosis, consists of a patient's strong belief that he or she is infested with a nonliving substance or living organism despite lack of medical evidence to support this belief. The most commonly reported sources of infestation include insects, fibrous strands, worms, and scabies. DI is predominantly seen in women and older patients. This disorder has a variable course and prognosis in patients. DI can be a primary psychiatric problem, or secondary to underlying medical conditions or other psychiatric disorders. It has been proposed that

Body dysmorphic disorder in patients with acne.

There is growing evidence that the prevalence of body dysmorphic disorder (BDD) is significantly higher in specially selected populations as compared to the general population. The goal of the current study was to evaluate prevalence of BDD in Turkish patients with mild acne presenting to a dermatologist for treatment. This study was the first empirical investigation of BDD in acne patients in Turkey. One hundred fifty-nine outpatients diagnosed with acne who consulted to the dermatology clinic were included in the study. The diagnosis of BDD was based on DSM-IV criteria and the Structured Cli

[Anxiety disorders in children and adolescents].

Anxiety disorders are among the most common psychiatric disorders in children and adolescents with estimated prevalence rates of between five and ten percent. Comorbidity rates are high--in particular for further anxiety and depressive disorders. The etiology is considered to be multifactorial, with genetic, biological and temperamental factors as well as stressful life events, parental role models and parenting all playing a part. Therapeutic programs combine psychological counseling--including the family--psychotherapy and psychopharmacotherapy, as required. As psychotherapy, cognitive-behav

Psychiatric comorbidity in primary care somatization disorder.

Lifetime prevalence rates were calculated for comorbid psychiatric disorders in 119 patients who were referred from primary care physicians for unexplained somatic complaints and who met DSM III-R criteria for somatization disorder. Comparisons were made with general population norms from the ECA study. Prevalence of nine comorbid conditions was significantly higher than in the general populations. The most prevalent comorbid diagnoses were major depression (54.6%), generalized anxiety disorder (33.6%), and phobic disorders (31.1%). The least common comorbid disorders were mania (4.2%) and dru

Symptomatology of minor psychiatric morbidity: a crosscultural comparison.

The basic symptoms of minor psychiatric morbidity (MPM) reported elsewhere were also found in a community survey in Taiwan. However, differences in the patterns of and manifestations of the symptoms were evident. Contrary to most Western surveys, the prevalence of anxiety (24.7%) was found to be higher than that of depression (8.3%) in Taiwan. Possible explanations based on sociocultural characteristics of the Chinese family were proposed. The notion of somatization as a predominant symptom in Chinese neurotic patients advocated by some research workers was not supported in this study. As a re

Acute and long-term mental and physical sequelae in the aftermath of traumatic exposure - some remarks on "the body keeps the score".

Traumata, by definition, refer to exterior events that expose a person to experiences of overwhelming threat and catastrophe and elicit feelings of death anxiety, panic, horror, helplessness, loss of personal control, and intractability. Most affected persons respond with at least some distressing symptoms of trauma-related memory intrusions, autonomic hyperarousal, dissociation, and depression in the acute aftermath. Fortunately, the majority of traumatized individuals succeed in coping with this major stress quite well during the following weeks and months unless the process of recovery is h

[Rethink the panic disorder].

INTRODUCTION: We propose some reflexions on the validity of the conceptualization of panic disorder, its nosographical place, and its clinical homogeneity, through the study of the frequency of some of its psychiatric comorbidities. BACKGROUND: To define a panic attack, DSM IV requires a number of symptoms which vary from four to 13. However, some patients suffer from panic attacks with less than four symptoms (paucisymptomatic attacks) and which fill the other criteria of panic disorder. These patients would have a biological vulnerability, familial antecedents, and a treatment response whic

The relationship between borderline personality disorder and anxiety disorders.

Eleven "pure" borderlines, ten borderlines with depression, 16 "pure" depressives, and 31 normal subjects were compared on a number of standardized inventories of anxiety. While patient groups experienced more anxiety of all types than did normals, borderlines did not emerge as more anxious than other patient groups. Qualitative differences in the anxiety experienced by borderlines and nonborderlines are discussed.

Does psychotherapy benefit neurotic patients.

Smith, Glass, and Miller examined the benefits of psychotherapy in 475 controlled studies. Their data were reexamined, and only studies of patients coded as having neuroses, true phobias, and emotional-somatic complaints and who had sought treatment were included in the present analysis. The results of 81 controlled trials were integrated statistically using the metaanalytic technique. The findings provide further evidence on the efficacy of psychotherapy. The condition of the typical patient after treatment was better than that of 77% of untreated controls measured at the same time. The rate
