Primary Extradural Meningioma of Posterior Fossa Associated with Acquired Chiari Malformation: A Short Review.

Generally, meningioma is considered intracranial lesion occurring in the intradural compartment. However, meningioma can also occur and usua … uch as temporal, frontal, and parietal bone and orbit but extremely uncommonly in the occipital and sphenoid bones. Authors did detailed Pubmed search for posterior fossa, occipital bone extradural, ectopic intraosseous meningioma which yielded only four publications in the form of isolated case report analyzing only five case of PEM. Authors report a rare case …

The manifestation of cocaine-induced midline destructive lesion in bone tissue and its identification in human skeletal remains.

Cocaine-induced midline destructive lesion (CIMDL) is a condition that may arise in response to chronic insufflation ("snorting") of cocaine. It is clinically diagnosed when the nasal septum, lateral nasal walls, and/or hard palate show signs of destruction in association with cocaine use. Although its true incidence is unknown, CIMDL is not an uncommon clinical finding amongst intranasal cocaine abusers and is likely to be encountered by forensic anthropologists and medical examiners working worldwide. Given the preponderance of drug abusers amongst the subjects of forensic casework, the abil

Surgical navigation in cranio-maxillofacial surgery: an evaluation on a child with a cranio-facio-orbital tumour.

Surgical navigation, though well-established, is often associated with extra effort for both patient and surgeon, and with additional exposure to radiation as a result of the necessary extra imaging. Osteoblastoma is a benign bone tumour, malignant transformation of which is rare but possible, and so resection is indicated. The orbit with surrounding structures is a challenging region for accurate three-dimensional reconstruction. A virtual plan based on the patient's mirrored anatomy realised by intraoperative navigation can assist in achieving perfect results. An 8-year-old boy presented wit

Extended orbitozygomatic approach to the skull base to improve access to the cavernous sinus and optic chiasm.

BACKGROUND: The orbitozygomatic approach to the skull base has evolved over the past century, with a surge in development during the past 20 years. We describe an extension of this technique involving removal of the most inferior portion of the temporal bone to the level of the internal carotid artery as it enters the carotid canal, to further facilitate exposure of tumors in this region. METHODS AND MATERIALS: We performed the extended orbitozygomatic approach in a series of 17 patients with a variety of neoplastic lesions. Our case series was reviewed in a retrospective fashion, and our sur

A novel technique for skull base reconstruction in craniofacial fibrous dysplasia surgery using three-dimensional printing and a dental alginate mold: illustrative case.

BACKGROUND: Fibrous dysplasia is a rare and benign skeletal lesion characterized by fibrous tissue proliferation due to an abnormal osteogenesis replacing normal bone. OBSERVATIONS: An 18-year-old male with fibrous dysplasia of the left sphenoid, ethmoid, orbit, and frontal bones was managed with excision and skull base reconstruction. After complete removal of the tumor, skull base reconstruction was commenced by making a reverse temporalis flap and placing it over the opened paranasal sinuses for a robust vascularized graft, followed by an abdominal fat graft, and then a pedicled pericranal

The Comparison of Piriform Aperture, Paranasal Sinuses, and Cranial Dimensions.

OBJECTIVE: Piriform aperture, paranasal sinuses, and the cranium dimensions were compared with each other and we investigated the alterations depending on the age and gender in these structures. Before the endoscopic sinus surgery, anatomic variations in sinusoidal region and the occurring differences should be considered. The detection of these variations plays important roles in the prevention of complications which may happen in surgery or in obtaining a successful surgical result. MATERIALS AND METHODS: Piriform aperture maximum width, frontal sinuses, sphenoidal sinuses, maxillary sinuse

Investigation of the relationship between facial injuries and traumatic brain injuries using a realistic subject-specific finite element head model.

In spite of anatomic proximity of the facial skeleton and cranium, there is lack of information in the literature regarding the relationship between facial and brain injuries. This study aims to correlate brain injuries with facial injuries using finite element method (FEM). Nine common impact scenarios of facial injuries are simulated with their individual stress wave propagation paths in the facial skeleton and the intracranial brain. Fractures of cranio-facial bones and intracranial injuries are evaluated based on the tolerance limits of the biomechanical parameters. General trend of maximu

[The parasellar osteo-dural chamber and the vascular and neural elements that traverse it. An anatomical concept that would replace the cavernous sinus of classical anatomy].

On each side of the sella turcica, because of the existence of vascular and nervous elements, the two aspects of the parasellar segment of the tentorium cerebelli diverge and with the wall of the sphenoid bone form an extradural lodge. In this space are included a venous pathway (the cavernous sinus of the classics), the internal carotid artery and the three oculo-motor cranial nerves. The venous pathway is a plexus of small sized veins, mainly draining the orbital blood, and has neighborhood relations with the carotid. The morphology of the region and the relations of the internal carotid art…

Repair of encephalocele and cerebrospinal fluid leak with the use of bone morphogenetic protein: A case report.

BACKGROUND: Encephaloceles are rare phenomena which occur when brain parenchyma herniates through a skull defect which, if left untreated, may lead to significant issues such as cerebrospinal fluid (CSF) fistulas, meningitis, and intractable seizures. Due to the rarity and variety in size and location of encephaloceles, no standard technique has been established for the resultant defect. Herein, we demonstrate the safe and effective use of bone morphogenetic protein (BMP) in the repair of CSF leak caused by encephalocele. CASE DESCRIPTION: A retrospective chart review was conducted on a 50-ye

"The pen is mightier than the sword" - suicidal trans-orbital intracranial penetrating injury from a pencil.

A 40-year-old drug addict, who was being treated with methadone and occupational therapy, committed suicide by striking a wooden pencil into his right eye socket. While still conscious, he hit his head hard against a table, jamming the pencil even deeper into his head. The autopsy showed that the pencil missed the globe and lodged in the inner part of the right eye socket. It pierced the orbital part of the right ethmoid bone, the right ethmoid cells, and the right superior nasal concha, then passed through the body of the sphenoid bone and the clivus of the occipital bone before stopping in t…

Inferior encephalocele: transpalatal repair using paired costal bone grafts with a 14-year follow-up.

A 2-year-old girl was referred to our hospital because of a pulsating mass in the roof of the mouth. On examination, a mass measuring 4 × 5 cm was found in the roof of the mouth and nose with a secondary palatal cleft. She had hypertelorism, a bifid nose, and a visible cleft over the dorsum and skin of the nose. In 1 stage, the mass was opened, reduced, and repositioned into the cranial cavity, and the defect was repaired with 2 parallel bridges of split costal bone grafts. The bone grafts were placed between 2 layers of soft tissue and the mucosa repaired over it. Palatal cleft was repaired w

Facial-zygomatic triangle: a relationship between the extracranial portion of facial nerve and the zygomatic arch.

BACKGROUND: This study was conducted to clarify the relationships between the extracranial portion of the facial nerve (EFN) and the zygomatic arch (ZA). METHOD: Four cadaveric heads (8 parotid regions), examined under 3-40x magnification, were dissected from lateral to medial to expose the EFN. FINDINGS: In a vertical plane just anterior to the tragus, the distance from the superior edge of the ZA to the facial nerve (FN) is, on average, 26.88 mm. The FN then courses superiorly and anteriorly, crossing the ZA 18.65 mm anterior to the tragus on average. Thus, three points can be used to depi

Posterior fossa morphometry in symptomatic pediatric and adult Chiari I malformation.

The linear dimensions and volume of the posterior fossa, and the length of the supra-occiput and the clivus in children with Chiari I malformations (CMI) were studied. A statistical relationship between patient demographics, radiological features, posterior fossa and occipital bone morphometry in the study and control groups was investigated. The results of 21 pediatric patients was compared to those of a matched control group. The posterior fossa volume (PFV) of an adult CMI group was also studied. Linear measurements were used to calculate the length of the occipital bone, spherical PFV and

Ectopic sphenoid sinus pituitary adenoma (ESSPA) with normal anterior pituitary gland: a clinicopathologic and immunophenotypic study of 32 cases with a comprehensive review of the english literature.

Ectopic sphenoid sinus pituitary adenoma (ESSPA) may arise from a remnant of Rathke's pouch. These tumors are frequently misdiagnosed as other neuroendocrine or epithelial neoplasms which may develop in this site (olfactory neuroblastoma, neuroendocrine carcinoma, sinonasal undifferentiated carcinoma, paraganglioma, melanoma). Thirty-two patients with ESSPA identified in patients with normal pituitary glands (intact sella turcica) were retrospectively retrieved from the consultation files of the authors' institutions. Clinical records were reviewed with follow-up obtained. An immunohistochemic

Sphenoid Temporal Meningioma With Granular Cell Features and Extracranial Infratemporal Extension.

Meningiomas usually arise from arachnoid cap cells and present as intracranial, dural-based lesions. Extracranial extension into the temporalis muscle, infratemporal fossa, orbit, and skull base foramina is uncommon and can mimic primary soft-tissue or peripheral nerve sheath tumors. We report a 54-year-old female who presented with a slowly progressive right-sided facial and temporal swelling over two years, associated with intermittent pain and mild proptosis but no focal neurological deficit. Computed tomography demonstrated a small extra-axial minimally enhancing right anterior temporal le

Internal Carotid Artery Injury During the Endoscopic Transsphenoidal Surgery of Pituitary Adenoma: Case Illustration, Introspection, and Systematic Review.

Advances in endoscopic technology have made the endoscopic transsphenoidal approach the preferred approach for most surgeries of pituitary adenoma. The goal of these surgeries is to achieve cure, efficacy, and safety. Ample research has deliberated on the complications of cerebrospinal fluid (CSF) leak, meningitis, visual deterioration and nasal crusting after endoscopic transsphenoidal surgery. Among these, injury to the internal carotid artery (ICA) is not common in transsphenoidal pituitary surgery and has an incidence that ranges from 0.1% to 1%. Though it is rare, the effects are devastat
