Multilevel laminectomy in children has a significant rate of postoperative spinal deformity. To decrease the incidence of this complication, the use of osteoplastic laminotomy is advocated to minimise the risk of spinal deformity by preserving the normal architecture of the spine. In this retrospective study, a 10-year series of a paediatric population undergoing multilevel osteoplastic laminotomy is reviewed to determine the incidence, especially in contrast to laminectomies, and to identify factors that affect the occurrence of spinal column deformity. Seventy patients (mean age 4.2 years) u

Fusion of cervical spine in kyphotic alignment has been proven to produce an acceleration of degenerative changes at adjacent levels. Stand-alone cages are reported to have a relatively high incidence of implant subsidence with secondary kyphotic deformity. This malalignment may theoretically lead to adjacent segment disease in the long term. The prospective study analysed possible risk factors leading to cage subsidence with resulting sagittal malalignment of cervical spine. Radiographic data of 100 consecutive patients with compressive radiculo-/myelopathy due to degenerative disc prolapse o

BACKGROUND CONTEXT: The role of surgical correction of sagittal plane deformity in cases of lytic spondylolisthesis remains controversial. While some early evidence is emerging of the possible short- and long-term benefits of restoring spinal balance, many surgeons have been concerned about the associated risks. The insert-and-rotate posterior lumbar interbody fusion (PLIF) technique, first described by Jaslow in 1946, may enable surgeons to safely and effectively correct sagittal balance through a single posterior approach.
PURPOSE: To determine whether the focal kyphosis and subluxation ass

Screw augmentation with calcium apatite cement (CAC) was used in seven patients with a progressive osteoporotic spinal deformity. Thirty-nine spinal segments (64 screws) were augmented: 15 anteriorly (three patients) and 24 posteriorly (five patients). Dorsally, hemilaminectomy was performed at the level of all augmented screws to rule out CAC leakage. Autogenous bone graft was applied in all patients to induce fusion. Screw augmentation failure occurred in only one patient: 1 of the 16 ventral augmented screws (5.5%) was still loose after the augmentation procedure. In three other patients, 4

The spine is a key component of the skeletal system, with a complex anatomical structure that varies along its length. Its specific structure allows it to fulfill multiple functions, including maintaining proper upright posture. Human posture reflects the coordinated interaction between the kinesthetic sense, muscular system, and nervous system. Various factors influence posture formation, including environmental conditions, lifestyle, physical activity, nutrition, and genetics. Data from Poland indicate that postural defects affect 30% to 60% of children and adolescents, with variations due t

Adolescent Idiopathic Scoliosis (AIS) is a three-dimensional spinal deformity that, if untreated, can impair posture, biomechanics, and quality of life. This case report aims to present the clinical outcomes of an innovative conservative treatment approach that combines the PosturalSpine® D'Amanti Method with the Chêneau brace, in an 8-year-old patient with thoracolumbar rotoscoliosis. This integrative method may represent a novel alternative to conventional bracing or physiotherapy-only protocols. An 8-year-old girl diagnosed with a 32° thoracolumbar rotoscoliosis likely of hereditary origin

BACKGROUND: Adolescent idiopathic scoliosis (AIS), which is the most common type of scoliosis, is a progressive disease that occurs in children aged 10-16 years. Abnormal curvature in AIS provokes spinal asymmetry of the upper body alignment and might deteriorate postural balancing and control ability.
OBJECTIVE: To evaluate the effect of exercise interventions on balance and postural stability in patients with adolescent idiopathic scoliosis.
METHODS: Embase, Scopus, Pubmed (Medline) and Web of Science databases were searched using the terms idiopathic scoliosis, physiotherapy, and balance.

Thoracic kyphosis tends to increase with age. Hyperkyphosis is defined as excessive curvature of the thoracic spine and may be associated with adverse health effects. Hyperkyphosis in isolation or as a component of degenerative kyphoscoliosis has important implications for the surgical management of adult spinal deformity. Our objective was to review the literature on the epidemiology, etiology, natural history, management, and outcomes of thoracic hyperkyphosis. We performed a narrative review of literature on thoracic hyperkyphosis and its implications for adult spinal deformity surgery. Hyp

Human tails have been attributed to a disturbance in fetal tail regression which normally occurs at the gestation age of about 6 weeks. To date, more than 100 cases of human tails have been reported. However, reports of true human tails, which involve the coccygeal vertebrae, are rare. We recently encountered a patient with a true human tail which involved the coccygeal vertebrae and was accompanied by lumbar spinal lipoma and spina bifida. A four-year-old boy was brought to our clinic with complaints primarily of painful mass. The boy had no neurological abnormalities. Physically, a tail bone

Costello syndrome (CS) is a rare genetic syndrome in which, due to the occurrence of a mutation in the HRAS gene on chromosome 11 that causes the manifestation, a set of features such as a characteristic appearance, many congenital defects, intellectual disability and a genetic predisposition to cancer, friendly personality, and others can be identified. CS is very rare, with an incidence of ~1/300,000, but it belongs to one of the largest groups of congenital syndromes, called RASopathies, occurring with an incidence of 1/1000 people. Growth-guidance constructs are an alternative to growing rods for the surgical treatment of early onset scoliosis (EOS). Constructs containing ultra-high molecular weight polyethylene (UHMWPE) sublaminar tape have been proposed as an improvement to the traditional Luque trolley. Ideally, a certain minimum number of levels is instrumented, thus offering the best balance between providing adequate spinal fixation and minimizing surgical exposure and spinal mobility reduction. The objective of the current study was to validate a parametric FE model of the thoracolumbar spine including its ability

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a complex spinal deformity with evidence of contributions from multiple genetic factors, but the molecular basis of gene-gene interactions in its development remains unclear. Filamin B (FLNB) and tetratricopeptide repeat domain 26 (TTC26) have both been implicated in the regulation of primary cilia and extracellular matrix homeostasis. However, whether combined variation in these genes contributes to spinal instability has not been established.
RESULTS: We generated mice carrying heterozygous mutations in both Flnb and Ttc26 and find that t

OBJECTIVE: A new formula containing terms for age and sagittal curvature reported by the International Spine Study Group is ideal lumbar lordosis (iLL) = pelvic incidence - 0.3 thoracic kyphosis - 0.5Age + 10. However, there are no reports of whether proximal junctional failure (PJF) can be predicted using this formula. We assessed the utility of this formula in PJF in patients with adult spinal deformity with global kyphosis using the Roussouly classification.
METHODS: Forty-four patients with adult spinal deformity global kyphosis (mean age 70.0 years) who underwent multiple levels of later

Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal curvature deformity that appears in the adolescent period. In this study, we performed whole-exome sequencing on 11 unrelated Taiwanese patients with a Cobb's angle greater than 40 degrees. Our results identified more than 200 potential pathogenic rare variants, however, most of which were carried only by one individual. By in silico pathogenicity annotation studies, we found that TTN, CLCN1, and SOX8 were the most important genes, as multiple pathogenic variants were within these genes. Furthermore, biological functional anno
