Ankylosing spondylitis of the craniovertebral junction: a single surgeon's experience.

OBJECT: The objective of this study was to describe a single surgeon's experience managing craniovertebral junction (CVJ) disease due to ankylosing spondylitis. METHODS: The authors undertook a retrospective review of the records of patients with CVJ disease due to ankylosing spondylitis who were evaluated and treated by the senior author. Charts were reviewed for symptoms and signs at presentation, radiography results, treatment, and outcome. In addition, some of the patients had pathology reports available for review. RESULTS: Eight patients with CVJ disease due to ankylosing spondylitis w…

Splenic tuberculosis in a patient with ankylosing spondylitis treated with adalimumab.

We present a rare case of splenic tuberculosis in a 42-year old man with long-standing ankylosing spondylitis treated with adalimumab. We review the association between antitumor necrosis factor therapy and splenic tuberculosis. Our case, like many other reported cases, illustrates that the index of suspicion of tuberculosis in patients treated with anti TNF therapies must be high and emphasizes that this rare infection may occur even with negative tuberculosis screening before the initiation of therapy.

Coexistence of ankylosing spondylitis and rheumatoid arthritis in a female patient.

Ankylosing spondylitis (AS) and rheumatoid arthritis (RA) are two distinguished representatives of inflammatory rheumatic diseases. The two diseases differ significantly in their etiology, pathology, clinical signs, and in the nature of articular manifestations. Their association has been a rarity in the literature. Here, authors describe a case of a 55-year-old female patient with AS associated with RA. Her spinal symptoms started in 1979, and the diagnosis of AS was established based on the typical clinical picture and X-ray. She developed severe spinal deformity during the next decades. In …

Membranous glomerulonephritis in a patient with ankylosing spondylitis: a rare association.

Ankylosing spondylitis is a chronic inflammatory disease of the vertebral joints and soft tissues. Renal involvement, apart from amyloidosis, is rare in this disorder. Of the various glomerulonephritides reported in association with ankylosing spondylitis, IgA nephropathy is the most common. Membranous glomerulonephritis occurs very rarely in patients with ankylosing spondylitis, and only four such cases have been reported in the available English literature. Due to the rarity of this association, membranous glomerulonephritis may not initially be considered in patients with ankylosing spondyl…

Discovertebral (Andersson) lesions of the spine in ankylosing spondylitis revisited.

A well-known complication in patients with ankylosing spondylitis (AS) is the development of localised vertebral or discovertebral lesions of the spine, which was first described by Andersson in 1937. Since then, many different terms are used in literature to refer to these localised lesions of the spine, including the eponym 'Andersson lesion' (AL). The use of different terms reflects an ongoing debate on the exact aetiology of the AL. In the current study, we performed an extensive review of the literature in order to align communication on aetiology, diagnosis and management between treatin…

Vitiligo improvement in a patient with ankylosing spondylitis treated with infliximab.

Ankylosing spondylitis (AS) is a disabling disease affecting the enthesis and joints of the spine and peripheral sites. Tumour necrosis factor alpha (TNF-alpha) is a proinflammatory mediator which plays a significant role in the pathogenesis of AS. Infliximab (a chimeric monoclonal antibody which blocks the activity of TNF-alpha) is one of the most effective therapies of AS thus far. Vitiligo is a depigmenting disorder of the skin of unknown aetiology affecting 0.5-1% of the population. Here we describe the effect of infliximab on vitiligo in a patient with AS.

A case of post-pregnancy osteoporosis combined with ankylosing spondylitis.

Post-pregnancy osteoporosis is not a common disease and is hard to diagnosis because their specific situation is post-partum and lactation. It commonly occurs on lumbar spine within a few months after the birth of a patient's first child and it could lead to be fracture after minor trauma. Although its etiology is not clear, it would not be of sufficient magnitude to cause fractures unless the woman already had a substantial decrease in bone mass. Also, it is rare to be combined with ankylosing spondylitis. Ankylosing spondylitis has a higher risk of osteoporosis and vertebral fracture which i…

Lumboperitoneal shunt for treatment of dural ectasia in ankylosing spondylitis.

Neurological complications of ankylosing spondylitis (AS) are reported in 2.1% of patients. Cauda equina syndrome (CES) is rare and occurs at the ankylosing stage. MRI and CT of the lumbar spine show a cauda equina deformation with dural ectasia and bony erosion. We report three patients with AS presenting with progressive CES. These patients underwent lumboperitoneal shunting (LPS) surgery. The motor deficit improved in all cases. We suggest that CES develops from arterial pulsation of the CSF on a dural sac with reduced elasticity and that LPS reduces these intradural pressure shock waves. A…

[Early diagnosis of ankylosing spondylitis and spondyolarthritis and predictors of outome].

Ankylosing spondylitis (AS) is a relevant differential diagnosis in patients with chronic back pain. Inflammatory back pain is the leading symptom in AS and also in undifferentiated spondyloarthritis. The diagnosis of AS is often delayed by 5 to 10 years, mainly because radiographs do not reveal definite radiographic sacroiliitis during the initial stage of the disease. Diagnostic algorithms based on Bayesian models have been developed to facilitate making an early diagnosis in daily practice. Moreover, screening parameters for early referral to the rheumatologist of patients seen in primary c…

Pathologic alterations of the heart and the kidney in patients with ankylosing spondylitis.

BACKGROUND: The occurrence of a variety of pathological lesions of the heart and kidneys have been described in patients with ankylosing spondylitis (AS). The frequency of these alterations and whether they are specific for AS has been discussed controversially. - METHODS: Outpatients with AS were studied to determine the frequency of cardiac and renal alterations and to assess the associated clinical and demographic factors. - RESULTS: A total of 77 patients with AS participated in the study (male 84.4%, mean age 48.3 +/- 1.5 years, mean duration of disease 15.4 +/- 1.2 years). Hypertension…

Is osteoporosis generalized or localized to central skeleton in ankylosing spondylitis?

BACKGROUND: Osteoporosis at the lumbar spine and at the femur is a well-established complication in ankylosing spondylitis (AS), but the exact mechanism and the distribution of osteoporosis are not known absolutely. OBJECTIVE: To determine whether the osteoporosis is generalized or localized to central skeleton and to examine the relation between bone mineral density (BMD) and disease activity and radiologic progression in patients with AS. METHODS: In this study, 26 patients with AS and 33 healthy controls matched for age and sex were recruited to the study. Hip and forearm BMD were measure…

A co-occurrence of sarcoidosis and ankylosing spondylitis: a case report.

Sarcoidosis is an inflammatory multi-system disease, which may cause articular involvement. By causing sacroiliitis, it may mimic spondyloarthropathies. A case with a co-occurrence of sarcoidosis and late-onset of typical ankylosing spondylitis was presented and a possible association between these two entities was discussed.

Iatrogenic fractures in ankylosing spondylitis--a report of two cases.

STUDY DESIGN: Two cases of intraoperative, iatrogenic cervical spine fractures in patients with ankylosing spondylitis are reported. OBJECTIVE: To describe the uncommon complication of iatrogenic cervical spine fractures occurring during spine surgery in patients with ankylosing spondylitis. SUMMARY OF BACKGROUND DATA: To our knowledge, this is the first report on this rare complication. METHODS: A 39-year-old patient (1) with ankylosing spondylitis was operated on for cervical stenosis due to C1/2 anterolisthesis. Fifteen hours postoperatively, he developed acute quadriplegia. MRI revealed…

[Pathogenesis of ankylosing spondylitis-mechanisms of disease manifestation and chronicity].

Despite intensive research during the last three decades, it is still not clear which precise mechanisms determine the interactions between host factors (HLA-B27 and other genes, cytokines, T lymphocytes) and microbial factors leading to the manifestation and chronicity of ankylosing spondylitis (AS). Rheumatologists and histopathologists have focused their interest on decoding the immune-mediated inflammatory processes and on studying new bone formation and ankylosis. Concerning the genetic basis of AS, there is considerable effort in large genome-wide and candidate gene analyses to discover …

New directions in classification and outcome assessment in ankylosing spondylitis.

In 2003, several manuscripts on the classification of patients as having ankylosing spondylitis and the further development of outcome assessment tools have been published. It is obvious that the use of radiographic sacroiliitis is a problematic part of the classification criteria because there is major interobserver variation and is one of the causes of a long delay between onset of symptoms and diagnosis. Especially in patients without human leukocyte antigen B27, the diagnostic delay is long. Existing outcome tools, such as the Bath Ankylosing Spondylitis Disease Activity Index, have been a…

[Use of carbon-carbonic implants in surgery of tuberculous spondylitis].

For surgical treatment of spinal tuberculosis, carbon-carbonic implants were used for anterior spondylosis in 29 patients; in 14 of them bone autografts were additionally applied. Carbon-carbonic implants reliably fixed the operated part of the vertebral column, prevented an increase in kyphotic deformity, and, in combined plastic repair, created favorable conditions for consolidation of bone autografts. A bone-carbon block formed in the late postoperative period.

Surgical options in the treatment of the spinal disorders in ankylosing spondylitis.

Serious complications or unsatisfactory results in the past often have made patients as well as doctors hesitant to decide for surgery in ankylosing spondylitis. Though the techniques of correction for fixed flexion deformity still are demanding, decisive progress has been made toward less complications, less operative trauma and better quality of life after surgery. The present work represents a short review of the evolution of surgical techniques in treatment of spinal disorders encountered in ankylosing spondylitis. Emphasis is given to the benefits of improved instrumentation systems, intr…

Transverse myelitis in a patient with long-standing ankylosing spondylitis.

Ankylosing spondylitis is reported to involve not only the joints but other organs as well. Among these extra-articular involvements, uncommon complications associated with nervous system such as single root lesions, compression of the myelum and cauda equina syndrome have also been documented. Here we present a patient with long-standing ankylosing spondylitis who developed spastic paraparesis. Extensive study to find the cause of a spastic paraparesis failed and therefore led to the conclusion that this patient was suffering from transverse myelitis. Similar reports in the past have been att…
