Post-tuberculosis (TB) Unilateral Left Lung Damage: TB Misdiagnosis and Mismanagement.

The extent of lung damage post-tuberculosis (TB) infection varies greatly, resulting in impairment of pulmonary structure and/or function. We present a case of TB misdiagnosis and mismanagement in a 53-year-old black African male living with HIV on anti-retroviral therapy (ART) with post-TB unilateral left lung damage. He presented with a one-month history of intermittent symptoms of fever, night sweats, right-sided chest pain, exertion intolerance, and hemoptysis, and was commenced on anti-TB treatment (ATT) at a local clinic despite the two negative TB infection laboratory results. Further e

High long-term survival of bulk femoral head autograft for acetabular reconstruction in cementless THA for developmental hip dysplasia.

BACKGROUND: Deficient acetabula associated with acetabular dysplasia cause difficulty achieving adequate coverage of the acetabular component during THA. Autografting with the removed femoral head has been used for several decades to achieve better coverage, but the long-term benefits of this technique remain controversial, with some series reporting high rates of graft resorption and collapse. QUESTIONS/PURPOSES: We evaluated the fate of bulk femoral head autograft for acetabular reconstruction in cementless THA for developmental hip dysplasia. PATIENTS AND METHODS: We retrospectively revie

Morphology-mechanics coupling in intraluminal thrombus from abdominal aortic aneurysm visualized by synchrotron imaging.

Abdominal aortic aneurysm (AAA) rupture is a life-threatening event, yet current rupture risk assessment based on aneurysm size and growth rate fails to fully predict outcomes. Intraluminal thrombus (ILT), present in most AAAs, may influence wall mechanics and rupture potential, but the relationship between its microstructure and mechanical behavior remains poorly understood. This study first analyzed the human ILT morphology using synchrotron radiation-based phase-contrast micro-computed tomography (SR-PhC-μCT). Then for the first time stress-relaxation compression was applied in-situ to ILT,

Biomechanical design of titanium-PEEK combined fusion cage based on PLIF surgical model.

Fusion devices play a critical role in lumbar fusion surgery. Titanium alloy fusion devices offer good biocompatibility and stability, but their mechanical properties far exceed those of bone, leading to stress shielding effects after implantation, which can reduce spinal fusion rates and cause endplate collapse. On the other hand, fusion devices made of polyether ether ketone (PEEK), which has a lower elastic modulus, are not conducive to bone ingrowth and fusion stability due to their material properties. Personalized fusion devices that can precisely adapt to a patient's physiological condi

A 3.0-Angstrom Resolution Cryo-Electron Microscopy Structure and Antigenic Sites of Coxsackievirus A6-Like Particles.

Coxsackievirus A6 (CVA6) has recently emerged as one of the predominant causative agents of hand, foot, and mouth disease (HFMD). The structure of the CVA6 mature viral particle has not been solved thus far. Our previous work shows that recombinant virus-like particles (VLPs) of CVA6 represent a promising CVA6 vaccine candidate. Here, we report the first cryo-electron microscopy (cryo-EM) structure of the CVA6 VLP at 3.0-Å resolution. The CVA6 VLP exhibits the characteristic features of enteroviruses but presents an open channel at the 2-fold axis and an empty, collapsed VP1 pocket, which is b

Reed-inspired dual-siloxane engineering of cellulose nanofibril/alginate aerogels for wet-dry-tolerant thermal insulation.

Cellulose-based aerogels are promising sustainable thermal insulators, but their hydrophilic skeletons absorb moisture, causing capillary-shrinkage-induced collapse and insulation loss. Meanwhile, rigid hydrogen-bonded or ionically cross-linked networks fatigue under cyclic deformation. Inspired by reed structures, which integrate a water-repellent exterior with a lightweight porous interior, a two-step methyltrimethoxysilane (MTMS)/polydimethylsiloxane (PDMS) dual-siloxane strategy was developed to regulate wettability and junction flexibility in cellulose nanofibril/sodium alginate aerogels,

Phthalate Esters in Aquatic Ecosystems: A Multiscale Threat from Molecular Disruption to Ecological Risks.

Phthalate esters (PAEs), ubiquitous plastic additives, have emerged as persistent contaminants in aquatic ecosystems, yet their propagation from molecular initiating events to ecosystem-level collapse remains poorly integrated. This review synthesizes current knowledge on the source-to-sink dynamics of PAEs, revealing a critical paradox in their bioaccumulation patterns: unlike classical persistent organic pollutants, high molecular weight PAEs exhibit distinct trophic dilution rather than biomagnification along food webs, driven by metabolic biotransformation in higher trophic organisms. Desp

Structural, proteomic, and metabolomic insights into the antibacterial efficacy of nalidixic acid against Corynebacterium pseudotuberculosis.

Corynebacterium pseudotuberculosis is a facultative intracellular pathogen that causes caseous lymphadenitis, a disease responsible for substantial economic losses in the livestock industry. Although nalidixic acid (NA) has been shown to inhibit C. pseudotuberculosis biofilm formation, the molecular mechanisms underlying its efficacy remain poorly characterized. This study aimed to characterize the structural, proteomic, and metabolomic alterations induced by NA in C. pseudotuberculosis at effective concentrations. The minimum inhibitory concentration (MIC) and minimum bactericidal concentrati

Gel Foam Loaded with Oxygen-Consuming Bacterial Communities for the Coupled Inhibition of Lignite Spontaneous Combustion.

To improve the durability of microbial inhibition of coal spontaneous combustion, oxygen-consuming bacterial communities from mining environments were immobilized in gel foam to construct a bioactive carrier for lignite protection. Two previously screened aerobic communities, D2 and D3, were loaded into four candidate gel-foam systems and evaluated in terms of carrier stability, water retention, bacterial compatibility, and functional retention. The sodium alginate-based Y1 formulation exhibited the best overall carrier performance and maintained sustained microbial oxygen consumption. Importa

Stereospecific recognition of S-fluoxetine enabled by HOF-101-based multimodal probe.

BACKGROUND: Given the distinct pharmacological activities and toxicological profiles of fluoxetine enantiomers, highly selective analytical tools are essential to ensure medication safety and advance the accuracy of chiral recognition. The hydrogen-bonded organic framework HOF-101 possesses well defined porous channels, abundant directional hydrogen-bonding sites, and intrinsic fluorescence, and can achieve chiral recognition through differential cavity confinement and stereospecific hydrogen-bonding interactions. Based on this, we have developed a multimodal chiral fluorescent probe based on

Geometric multidimensional representation of omic signatures.

INTRODUCTION: Multi-omic signatures are widely used in biomarker discovery, precision oncology, and systems biology, yet they are typically treated as vectors or composite scores that collapse intrinsically multidimensional biological organization into one-dimensional summaries. As a result, their internal structure, contextual dependencies, and functional coherence remain largely inaccessible. METHODS: Here, we introduce a geometric framework that reconceptualizes omic signatures as multidimensional informational entities whose biological meaning arises from structural organization rather th

Ruptured sinus of Valsalva aneurysm mimicking infective endocarditis in a patient with Down syndrome: case report.

BACKGROUND: Sinus of Valsalva aneurysm is a rare structural anomaly that could be congenital or acquired, usually silent until the moment of rupture. Its diagnosis can be challenging as the turbulent jet flow may mimic vegetations or abscesses associated with infective endocarditis (IE). CASE SUMMARY: A 31-year-old woman with Down syndrome presented with signs of severe right heart failure and a history of recurrent fevers. Initial echocardiography revealed a massive left-to-right shunt and an echogenic structure in the aortic root, raising suspicion of a paravalvular abscess-patient was trea

Computational fluid dynamics simulation of upper airway flow and soft tissue deformation in a mouth-breathing patient: a case study.

OBJECTIVE: The primary objective of this study was to establish a three-dimensional fluid-structure interaction (FSI) model to compare the biomechanical effects of three distinct breathing modes-nasal (NA), oronasal (ONA), and oral (OA) ventilation-on upper airway dynamics in mouth-breathing patients. A key aim was to investigate a potential biomechanical link between mode-specific hydrodynamic forces and the soft tissue remodeling associated with adenoidal facies. METHODS: Mouth-breathing volunteer was selected, and cone-beam computed tomography (CBCT) and magnetic resonance imaging (MRI) im

Interaction of Sodium Polystyrenesulfonate with Fluorinated Ionic Surfactant of Opposite Charge.

Polyelectrolyte-surfactant systems of opposite charge have been widely studied because of their relevance in applications ranging from pharmaceuticals to advanced materials. However, the role of hydrophobic interactions in such systems remains debated, particularly for sodium poly(styrenesulfonate) (NaPSS) and cationic surfactants. This study investigates the binding behavior of NaPSS with the conventional hydrocarbon surfactant dodecylpyridinium chloride (DPC) or the fluorinated surfactant 1H,1H,2H,2H-perfluorodecylpyridinium chloride (HFDePC), with the objective of elucidating the importance

Economic impact analysis of a minimally invasive temperature-controlled radiofrequency device versus nasal surgery for the treatment of nasal airway obstruction in the United States.

OBJECTIVE: To determine the economic impact of a minimally invasive temperature-controlled radiofrequency (TCRF) device for treating nasal airway obstruction (NAO). METHODS: A budget impact model was developed for two scenarios: a reference scenario of functional rhinoplasty surgery with concomitant septoplasty and inferior turbinate reduction (ITR) performed in the hospital outpatient department where TCRF is not an available treatment option and a new scenario consisting of in-office TCRF treatment of the nasal valve and ITR. A payor perspective was adopted with a hypothetical population pl

Enhancing the water-resistance of MOF-199 film through incorporation of microporous organic networks for solid-phase microextraction of BTEX in aqueous environments with improved efficiency.

BACKGROUND: The practical application of moisture sensitive metal organic frameworks (MOFs) in extraction technology faces challenges related to competitive adsorption and water stability. The target analytes cannot be effectively extracted under humid conditions due to the competitive moisture adsorption and/or framework structure collapse of MOFs. In this study, the microporous organic networks (MONs) were synthesized through Sonogashira coupling reaction to use for hydrophobic modification on the surface of MOF-199. RESULTS: The MOF-199@MON as coating was deposited on stainless steel wires…

The influence of geometry on intervertebral disc stiffness.

Geometry plays an important role in intervertebral disc (IVD) mechanics. Previous computational studies have found a link between IVD geometry and stiffness. However, few experimental studies have investigated this link, possibly due to difficulties in non-destructively quantifying internal geometric features. Recent advances in ultra-high resolution MRI provides the opportunity to visualise IVD features in unprecedented detail. This study aimed to quantify 3D human IVD geometries using 9.4 T MRIs and to investigate correlations between geometric variations and IVD stiffness. Thirty human lumb

In vitro protoscolicidal effects of lithocholic acid on protoscoleces of Echinococcus granulosus and its mechanism.

Surgery has been found to be the best choice of treatment for hydatidosis. However, leakage of cyst contents during surgery is the foremost reason for recurrence of hydatidosis. In this study, we investigated the in vitro efficacy of lithocholic acid (LCA) against Echinococcus granulosus protoscoleces. The protoscoleces were divided into a control group, an albendazole (ABZ) positive control group and LCA intervention groups at concentrations of 0.5, 1, 2, and 3 mmol/L and stained with 0.1% eosin for observation using an inverted microscope; the protoscolecal ultrastructure was examined with S
