Athletic injuries of the wrist and hand: part II: overuse injuries of the wrist and traumatic injuries to the hand.

Hand and wrist injuries in sports are some of the most common injuries reported. This review discusses common overuse injuries of the wrist including tendon injuries such as de Quervain's syndrome, subluxation of the extensor carpi ulnaris, and the common dorsal carpal impingement syndrome. The main focus of this section is the discussion of traumatic injuries to the hand in the athlete. Included is a discussion and review of fractures of the phalanges and metacarpals, common proximal interphalangeal joint injuries, and thumb carpal metacarpal and metacarpophalangeal joint injuries. Emphasis i…

Tendon transfer to reconstruct wrist extension in children with obstetric brachial plexus palsy.

This study reports on 20 children with obstetric brachial plexus palsy who underwent a tendon transfer to reconstruct wrist extension. The mean age at the time of tendon transfer was 8 years. There were seven patients with Erb's palsy and the remaining 13 had total palsy. The flexor carpi ulnaris was utilized 15 times and the flexor carpi radialis five times. The transferred tendon was sutured to the tendon of the extensor carpi radialis brevis. The result of the transfer was assessed according to a modified Medical Research Council (MRC) muscle grading system. A good result was obtained in 18

Long-Term Functional Outcomes of Posttraumatic Complex Extensor Tendon Reconstruction in Hand and Forearm Using Composite Anterolateral Thigh Free Flaps: A Case Series and Literature Review.

Complex dorsal hand and forearm injuries with extensor tendon and soft tissue loss are challenging to manage. Single-stage reconstruction using composite anterolateral thigh (ALT) fasciocutaneous free flaps with vascularized fascia lata may reduce the need for multiple surgeries and accelerate functional recovery, but clinical evidence is limited. We conducted a retrospective case series of nine patients with complex extensor tendon and soft tissue defects who underwent single-stage reconstruction with composite ALT flaps between 2009 and 2018. Six patients were available for long-term follow-

Distal biceps and triceps injuries in athletes.

Biceps and triceps ruptures are rare, but can cause significant disability. They typically result from a forceful eccentric contraction, resulting in weakness and pain. Biceps and triceps tendon injuries represent the most and least common tendinous injuries about the elbow, respectively. Nonoperative management of these injuries is generally reserved for partial ruptures or patients unfit for surgery. Surgical repair has become the preferred method of treatment for acute, complete ruptures. Anatomy, epidemiology, clinical evaluation, and treatment of these injuries are described in this revie…

Traumatic rupture of the tibialis posterior tendon after ankle fracture: a case report.

Degenerative tears of the posterior tibial tendon associated with chronic disease are well documented in the literature. Traumatic ruptures of this tendon, however, are much less common and consequently have received little attention. An association has been shown between pronation-external rotation ankle fractures and tears of the tendons that cross the medial aspect of the ankle, most commonly the posterior tibial tendon. In the present case report, we share our unique experience of an open-ankle fracture associated with the traumatic rupture of the posterior tibial tendon. This injury illus

Dual-layer aligned-random nanofibrous scaffolds for improving gradient microstructure of tendon-to-bone healing in a rabbit extra-articular model.

BACKGROUND: Tendon/ligament injuries are common sports injuries. Clinically, the repair of a ruptured tendon or ligament to its bony insertion is needed, but the enthesis structure is not well reestablished following surgical repair. Herein, we fabricated dual-layer aligned-random scaffold (ARS) by electrospinning and aimed to investigate the effect of the scaffold on tendon-to-bone healing in vivo. MATERIALS AND METHODS: The random and dual-layer aligned-random silk fbroin poly(L-lactic acid-co-e-caprolactone) (P(LLA-CL)) nanofibrous scaffolds were successfully fabricated by electrospinning

Evolution of the Achilles tendon: The athlete's Achilles heel?

The Achilles tendon is believed to have first developed two million years ago enabling humans to run twice as fast. However if the Achilles … rch has shown that there is a genetic influence on the predisposition a person has towards Achilles tendon injuries. So if this tendon is here to stay in our anatomy, and it probably is due to the slow rate of evolution in humans, research in genetic modification could be used to decrease athletes' predisposition to Achilles tendinopathy.

Repair of Achilles tendon rupture using autologous semitendinosus graft in a kidney transplant recipient.

Insertional Achilles tendon injuries can be difficult to treat when minimal tendon tissue remains for anastomosis. Moreover, in the chronic case with tendon shortening, operative repair can be more difficult than acute rupture. It is particularly desirable to reinforce the tendons, in addition to performing primary repair, in patients with renal or systemic diseases because of the accelerated collagen degeneration. Many techniques have been described for the surgical management of Achilles tendon rupture; however, none has shown clear superiority. We report the case of a 50-year-old renal tran…

Analysis of the therapeutic efficacy of arthroscopic repair of the subscapularis tendon combined with coracoplasty.

The aim of this study was to analyze the outcomes of arthroscopic subscapularis tendon repair combined with coracoplasty in the treatment. T … that subcoracoid impingement syndrome could potentially be a significant precursor to subscapularis tendon injuries. It is noteworthy noting that the combined approach of subscapularis tendon repair and coracoplasty showed superior therapeutic efficacy compared to isolated subscapularis tendon repair when subscapularis tendon injuries were associated with subcora …

Isolated vastus lateralis tendon avulsion.

Isolated avulsion of the vastus lateralis tendon is a very rare injury. To our knowledge, only 1 case has been reported in the literature. This tendon is crucial to knee stability and proper patellofemoral tracking. As isolated avulsion of the tendon tends to occur in young, active males, early surgical repair is recommended to allow them to maintain a high level of functional ability. We present the case of a 49-year-old man who sustained an isolated vastus lateralis tendon avulsion injury. The injury was successfully treated with suture anchor repair.

MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

[Traumatic rupture of subscapularis with avulsion of tuberculum minor and subluxation of the biceps tendon in a 13-year-old].

Traumatic ruptures of the subscapularis tendon in children are rare and often missed. In this case report, a 70 kg person landed on a 13-year-old boy with his arm abducted and rotated, causing pain and restricted motion. An MRI showed a total rupture of the subscapularis with avulsion of the minor tubercle and subluxation of the biceps tendon. The avulsion was fixed to the foot print and the biceps tendon in the bicipital groove, by delto-pectoral approach, using three anchors. It is important to avoid missing these injuries. They may cause chronic shoulder pain and decreased motion if left un

Management of bilateral Achilles tendon rupture associated with ciprofloxacin: a review and case presentation.

Fluoroquinolone antibiotics are increasingly being recognized as a cause of Achilles tendinitis and rupture. We report the case of a 62-year old man who developed bilateral Achilles tendon rupture six days following commencement of ciprofloxacin. Tendon exploration and repair was accomplished with the use of a prosthetic substitute (Leeds-Keio ligament) but healing was complicated by left wound breakdown that was successfully repaired with a perforator-based fasciocutaneous flap. A review of the current literature on fluoroquinolone associated achilles ruptures and the various methods of tendo

Flexor hallucis longus dysfunction: an overview.

Whereas acute and chronic injuries of the tibialis posterior, peroneal and Achilles tendon are frequently encountered, disorders of the flexor hallucis longus tendon are often overlooked, which may contribute to chronic pain and disability. Patients with stenosing tenosynovitis of the flexor hallucis longus tendon frequently present with overlapping signs and symptoms of flexor hallucis longus tendinitis, plantar fasciitis and tarsal tunnel syndrome, which the authors collectively refer to as "flexor hallucis longus dysfunction." A keen awareness of the presenting signs and symptoms and use of

Isolated subluxation of the extensor pollicis longus tendon. A cause of 'boutonnière' deformity of the thumb.

Isolated ulnar subluxation of the extensor pollicis longus (EPL) tendon occurs when there is damage to the radial side of the extensor hood on the dorsum of the metacarpophalangeal joint of the thumb. The thumb collapses into a pseudoboutonnière deformity because the EPL tendon subluxes and comes to lie palmar to the centre of rotation of the metacarpophalangeal joint to act as a flexor instead of an extensor. The extensor pollicis brevis tendon which lies in a deeper plane to the EPL is unaffected. We describe four cases and propose conservative treatment as being effective in recent injuries

Pathologic hindfoot conditions in the athlete.

Millions of people run in the United States, both for competitive reasons and for basic conditioning. Unfortunately, many runners develop cumulative (overload) stress syndromes, a number of which occur in the hindfoot. Among the most common are Achilles tendinitis and its associated conditions, plantar fasciitis, and tendinitis of the posterior tibial and flexor hallucis longus tendons. Most of these conditions respond well to conservative treatment, but in some instances surgery is needed. Surgery frequently can help patients who have not responded to conservative therapy to return to active
