Functional Evaluation of Levator Scapulae Tendon to Supraspinatus in Adult Brachial Plexus Injuries.

Introduction  Brachial plexus injuries are severe life-altering injuries. The surgical method to restore shoulder abduction in adult upper b … previous surgery where supraspinatus has recovered partially but not functionally significant, this tendon transfer can be considered for the augmentation of the existing shoulder abduction.

Vascularised flexor tendon grafts.

In densely scarred digits, tendon reconstruction fails because of paucity of blood supply. This inevitably leads to adhesion formation and failure of gliding. Staged tendon grafting with silicone spaces can improve results but does not ameliorate the essential problem of reduced vascularity and may indeed worsen it. Tendon grafts can be implanted with their own living sheaths as free or pedicled vascularised transfers from the foot or forearm to either extensor or flexor sites in the hand. These mesenteric tendons with their fascial or synovial sheaths intact and vascularised by adjacent vascu

Excision of aberrant abductor pollicis longus tendon slips for decompression of de Quervain's disease.

Release or excision of the first extensor compartment is a commonly performed surgical procedure to treat de Quervain's disease. This technique can potentially cause palmar subluxation of the extensor tendons. The abductor pollicis longus (APL) tendon has multiple slips which can be used as a resource for tendon transfer without loss of function. The technique described in this paper is decompression of the first extensor compartment by excision of an aberrant APL tendon slip without releasing the first extensor compartment. This technique was used in 24 wrists in 21 patients, and outcome asse…

Long-term results of the Steindler flexorplasty.

Seventy-one consecutive patients were treated with a modified Steindler flexorplasty from 1970 to 1987. The mechanisms of injury and etiologies included 45 motor vehicle accidents, 13 birth injuries, and 13 cases of poliomyelitis. Additional operative procedures included shoulder fusion (45 patients), tendon transfer (20 patients), and wrist tenodesis (3). Follow-up averaged 8.2 years. The outcome was excellent in 32%, good in 47%, fair in 13%, and poor in 8%. Postoperatively, the mean arc of active elbow flexion was 114 degrees; the average elbow extension loss, 28 degrees; the mean active pr…

Transfer of the triceps brachii to the finger and thumb extensors: Anatomical study and report on one case.

Our aims were to study the anatomical feasibility of triceps brachii long head (TBLH) transfer to the extensor digitorum communis (EDC) and extensor pollicis longus (EPL) tendons through a medial route, and to report on its first clinical application. Dissections were conducted on 10 fresh-frozen cadaver specimens. Using a posteromedial approach, the TBLH was separated from the remaining triceps and extended distally with a fascia lata strip. This strip was then tunneled through a medial route and secured distally to the EDC and EPL tendons. The transfer tenodesis effect during elbow extension

[Management of affected flexor tendons in rheumatoid arthritis of the hand].

The affection to the flexor tendons of the patient with rheumatoid arthritis represents a substantial rheumatic change in the hand, which is characterized by restriction of movement, ulnar deviation in the level of the metacarpophalangeal joint and palmar incomplete dislocation. Early treatment by removing any possible constriction areas in synovial proliferations in the area of the flexor pullies or a complete tendosynovectomy makes an extensive restitution possible. In the case of only one ruptured flexor tendon, it can be treated by the transfer of the neighboring superficialis tendon or a

Anatomic variation of the 5th extensor tendon compartment and extensor digiti minimi tendon.

Anatomic variation within the 5th extensor compartment may contribute to the development of tenosynovitis and limit the usefulness of the extensor digiti minimi (EDM) for tendon transfer. The purpose of this study was to assess the anatomic variation of the EDM tendon and its surrounding retinaculum, with particular attention to anatomical variation between specimens. Forty-one fresh cadaver hands were dissected. The length of the 5th compartment retinaculum was noted. The incidence of an intercompartmental septum was noted in each specimen as well as the type of tendinous attachments present …

Musculo-tendinous transfers of the hand and forearm.

The concept of providing a more equitable distribution of forces acting upon a disabled limb is the basis of tendon transfers. The musculo-tendinous transfer is usually performed by moving the insertion, thereby altering the direction of the unit in an attempt to replace a lost function. In order to obtain a successful result basic principles have to be followed. The transferred muscle must have enough strength, should have approximately the same amplitude as the muscle whose function it is replacing and should have a straight line of pull from the origin to the new insertion or distally from …

Arthroscopic Acromioclavicular Joint Stabilization Using Autologous Biceps for Horizontal Neutralization (AutoBAHN) Technique.

The surgical treatment of chronic acromioclavicular joint (ACJ) instability is most commonly performed using tendon grafts combined with synthetic stabilization. However, autografts can be associated with donor site morbidity, while allografts often have inferior tissue quality and a higher risk of graft failure. We present a technique of arthroscopically assisted ACJ stabilization using an autologous, rerouted long head of the biceps (LHB) tendon transfer, augmented with a suture button pulley construct for the treatment of symptomatic bidirectional ACJ instability. The LHB tendon is tenodese…

Arthroscopy-Assisted Transosseous Lower Trapezius Transfer With Peroneus Longus Autograft Using a Reusable Transosseous Repair System for Massive Irreparable Cuff Tears.

Tendon transfers are preferred in patients with massive, irreparable posterosuperior rotator cuff tears without glenohumeral arthritis. Recently, a surge of lower trapezius transfer over latissimus dorsi transfer has been observed due to its biomechanical superiority. We present an arthroscopy-assisted lower trapezius transfer technique using a reusable transosseous system and a peroneus longus autograft. The reusable repair system and autograft make this technique both accessible and affordable. With its potential to improve patient outcomes and reduce health care costs, this lower trapezius …

Biomechanics of lower trapezius and latissimus dorsi transfers in rotator cuff-deficient shoulders.

BACKGROUND: Irreparable posterosuperior rotator cuff tears cause pain and impaired shoulder function. Latissimus dorsi (LD) transfer has been proven to improve shoulder function, but lower trapezius (LT) transfer has recently been proposed as an alternative. This study aimed to compare the biomechanics of LD and LT transfers and how they are affected by different insertion sites. METHODS: The Newcastle shoulder model was used to investigate the biomechanics of these 2 tendon transfers. Computed tomography data sets from 10 healthy subjects were used to customize the model, and virtual LD and …

A previously unreported variation related to the insertion of the tibialis anterior muscle and the superficial fibular (peroneal) nerve.

During dissection classes in 2003 in the Department of Anatomy at Ege University Faculty of Medicine, an unexpected variation of the insertion of the tibialis anterior tendon was determined on the right side of a formalin-fixed cadaver of a 40-year-old man without any trace of scars, adhesions or signs of trauma or operation. The insertion of the tibialis anterior tendon was very abnormal, not on the medial side of the foot but on the lateral side. The tibialis anterior is an important muscle because of its function and use in tendon transfer as a treatment of recurrent congenital clubfoot and…
