Spinal intramedullary tuberculoma requiring surgical treatment--case report.

A 71-year-old male presented with multiple central nervous system tuberculomas including spinal intramedullary tuberculoma manifesting as occipitalgia and left hemiparesis. He had received medical treatment for lung and testis tuberculosis. Magnetic resonance imaging revealed an intramedullary lesion at the C-2 level as well as multiple small extramedullary and intracranial lesions. His neurological symptoms gradually worsened despite intensive antituberculous therapy. The C-2 intramedullary lesion responsible for left hemiparesis was surgically extirpated. Postoperatively, his neurological sy

Early surgical intervention for spinal infection in patients with malignancy requiring chemotherapy: report of two cases and review of the literature.

Advances in chemotherapy for various malignancies have contributed to the increased life expectancy of patients. If such a patient has a concurrent infection, his/her oncologist would hesitate to perform prompt chemotherapy owing to the risk of inducing sepsis. Therefore, the treatment of infection would have priority over initiating chemotherapy for the malignancy. We present a 69-year-old female with malignant lymphoma requiring prompt chemotherapy who also demonstrated spinal infection with Mycobacterium tuberculosis and a 66-year-old male with esophageal cancer who also demonstrated spinal

Extrapulmonary tuberculosis: tuberculous meningitis new developments.

Tuberculosis (TB) can involve any organ system in the body. Extrapulmonary involvement can occur in isolation or along with a pulmonary focus as in the case of patients with disseminated tuberculosis. Tuberculosis meningitis (TBM) is the most severe form of extrapulmonary tuberculosis. TBM a medical emergency, is still a major cause of serious illness in many parts of the world. TBM remains difficult to diagnose, and it is usually due to hematogenous dissemination of the tubercle bacillus. The exact incidence and prevalence are not known. The clinical spectrum is broad and may be non-specific

[Devic's optic neuromyelitis and pulmonary tuberculosis].

INTRODUCTION: Optic neuromyelitis or Devic's syndrome is a very rare disease affecting the optic tracts and the spinal cord. Its association with evolving pulmonary tuberculosis has been noted in a handful of case reports. CASE REPORT: The authors reportthe case of a 40 year old patient with evolving pulmonary tuberculosis who experienced bilateral blindness followed by paraplegia and sphincter disturbance. Clinical examination and investigations excluded direct tuberculous involvement of the nervous system or a reaction to antituberculous therapy and Devic's syndrome was diagnosed, based on

Bone and joint tuberculosis--a 10-year review.

The incidence of bone and joint tuberculosis has declined ever since the advent of medical antituberculous therapy. To evaluate the evolving presentation, we undertook a 10-year review of this form of extrapulmonary tuberculosis in 7 general hospitals in Israel. Thirty-eight patients were identified on the basis of strict criteria. Median age of 63 years in this series was higher than foreseen in this relatively underdeveloped region. Spinal involvement was remarkably low. Involvement of the extremities had a trend towards right side predominance. Overt clinical manifestations were encountered

[Early diagnosis of atlanto-axial tuberculosis].

Spinal tuberculosis at the atlanto-axial level is often overlooked or misdiagnosed for a considerable period of time, until when there has developed subluxation or other obvious complications. In order to make early diagnosis of this lesion, we analysed 30 such patients of our own and found that the positive X-ray film findings, such as thickening and widening of the soft tissue shadow of the retropharyngeal wall could hardly be concerned as an early sign, subluxation of the joint and destruction of bone were definitely quite a late occurrence. While long lasting pain in the upper cervical reg

Epidural spinal infection in intravenous drug abusers.

The incidence of spinal epidural abscess and disk space infection appears to be rising in intravenous drug-using patients. We report 18 cases seen over three years in two municipal hospitals. Staphylococcus aureus was the most common infective agent, but two patients had Mycobacterium tuberculosis infection. Computed tomography of the spine facilitates diagnosis. Early treatment improves outcome but host factors play a role in recovery.

Surgical Management in Elderly Patients with Tuberculous Spondylodiscitis: Ten Year Mortality Audit Study.

STUDY DESIGN: Retrospective study. PURPOSE: To evaluate the factors affecting immediate postoperative mortality in elderly patients with tuberculous spondylodiscitis. OVERVIEW OF LITERATURE: Treatment of spinal tuberculosis in the elderly involves consideration of age and co-morbidities, and often leads to an extended conservative management. Surgical intervention in these patients becomes a complex decision. There are no studies on risk factors of mortality in surgically treated elderly with tuberculous spondylodiscitis. METHODS: Two hundred and seventy-six patients with spondylodiscitis w

[Anterior spinal cord decompression in paraplegia in angular kyphosis. Apropos of 5 cases in children and adolescents].

The authors performed an anterior medullar decompression and a vertebral arthrodesis with bone grafts in 5 children and adolescents suffering neurological troubles caused by an angular kyphosis. 4 patients out of 5 completely or almost completely recovered from their paraplegia in the days or weeks following surgery. The authors stress the infrequency of the indication of such a procedure justified in either recent or incomplete neurological troubles without improvement by immobilization and rest. This procedure, which may raise technical problems, gives way to the rapid recovery of a satisfyi

Clinical features and surgical management of tuberculous arthritis of the sacroiliac joint: a retrospective analysis of 33 patients.

BACKGROUND: We reviewed 3 different types of tuberculous sacroiliitis via anterior and posterior approaches to determine the efficacy and safety of this surgical approach by describing clinical presentation, imaging, and surgical treatment. METHODS: We reviewed 33 patients with 3 different types of severe tuberculous sacroiliitis, of which 16 patients with anterior iliac abscess underwent anterior debridement. 17 patients underwent posterior debridement. Among them, 5 patients with lumbar tuberculosis underwent lesion debridement through fenestration, joint fusion, and interbody fusion and in

[Tuberculous spondylitis syndrome].

The authors conducted a comprehensive study of 482 patients with suspected tuberculous spondylitis. The commonest diagnostic signs of some spinal diseases, similar to the clinical ones, were defined on clinical, X-ray, laboratory, and thermographic findings. This enabled a differentially diagnostic syndrome including 58 signs to be developed. The proposed syndrome is to facilitate the differential diagnosis of tuberculous spondylitis from spinal tumors, degenerative and dystrophic processes, and nonspecific osteomyelitis of the vertebral column.
