From 1986 to 1996, 557 patients are treated for acute ulcer bleeding. The sex ratio is five to one for men, the average age is 40 years old. The duodenal ulcer is revelated by haemorrhage in 25.8%. 181 patients underwent a surgical operation (32%). Measurement of gastric pepsin and acid secretion was performed before, during and after selective vagotomy with antrectomy (SVA) and Billroth I gastrectomy (B-I) in 20 patients with a duodenal ulcer. Preoperative pepsin concentration increased during operation and decreased gradually 72 hours after SVA and 1 to 3 months after B-I. Simmondsin, 2-(cyanomethylene)-3 hydroxy 4,5 dimethoxy cyclohexyl beta-D-glucoside, from jojoba meal reduces food intake in rats. We investigated the mechanism of action simmondsin, by studying the effects of fasting or of vagotomy on the food intake reduction. The food intake reduction was significantly less in fasted rats than in non-fasted rats. The reduction of food intake was also significantly diminished after vagotomy. The results of the present experiments suggest that simmondsin reduces intake of food in rats through the augmentation of satiety, in part vagally mediated.

Operations were carried out on 258 patients with perforating ulcer and on 256 patients with acute bleeding from the ulcer. Perforating ulcer was managed by its closure with sutures in 226 patients (87.6%), by its excision with pyloroplasty and vagotomy in 4 (1.5%), and by primary resection of the stomach in 28 patients (10.9%).
