Inflammatory, Metabolic, and Coagulation Effects on Medial Arterial Calcification in Patients with Peripheral Arterial Disease.

Calcium deposits in the vessel wall in the form of hydroxyapatite can accumulate in the intimal layer, as in atherosclerotic plaque, but als … vide insight into possible mechanisms by which inflammatory and coagulation factors are involved in vascular calcification processes. A better understanding of the complexity of MAC and the mechanisms involved in its development is essential for the development of potential preventive and therapeutic strategies.

Management of Calcification: Rational and Technical Considerations for Intravascular Lithotripsy.

Arterial calcification (AC) is a common complication among patients with peripheral arterial disease (PAD). AC presents various challenges to PAD treatment including an increased likelihood of vessel rupture and dissection, and by acting as a physical barrier to drug delivery by angioplasty balloons. Intravascular lithotripsy (IVL) is a novel intervention that specifically targets AC by emitting sonic pressure waves that introduce microfractures in target calcified lesions to increase arterial compliance. Preliminary data has demonstrated the safety and efficacy of IVL in PAD treatment. The pr

Association of serum levels of osteopontin and osteoprotegerin with adverse outcomes after endovascular revascularisation in peripheral artery disease.

BACKGROUND: Osteoprotegerin (OPG) and osteopontin (OPN) are vascular calcification inhibitors with a known role in the atherosclerotic and inflammatory process. We investigated their relationship with adverse outcomes (restenosis/adverse cardiovascular events) after endovascular revascularisation of patients with peripheral arterial disease (PAD). METHODS: 203 consecutive patients were enrolled in the PAD group (PADG) and 78 age and sex-matched subjects with less than two cardiovascular risk factors served as control group (COG). PADG underwent standard medical assessment at baseline and 12 m…

Intravascular Lithotripsy in Peripheral Artery Disease.

Peripheral artery disease affects millions of people worldwide, and it is associated with significantly higher morbidity and mortality. In addition, it represents a significant challenge for the interventional operators to appropriately and successfully revascularize heavily calcified stenoses. There are several established atherectomy devices with the risk of procedural complications including dissection and perforation, among others. Intravascular lithotripsy (IVL) is a novel tool with relatively less procedural risk compared to the existing modalities. It is a device that emits high-energy

Treatment of a Heavily Calcified Celiac Artery Ostial Subtotal Occlusion Using Shockwave Lithotripsy: A Novel Approach.

PURPOSE: We present the novel use of adjunctive intravascular lithotripsy (IVL) before definitive intravascular stenting of a heavily calcified celiac artery ostial occlusion. CASE REPORT: A 79-year-old woman presented with chronic post-prandial abdominal pain and weight loss. Selective angiography revealed a sub-totally occluded celiac artery. Percutaneous endovascular intervention of the celiac artery was attempted but was unsuccessful because of heavy calcification. The patient returned for a repeat procedure. A guidewire was successfully advanced across the sub-totally occluded ostium. A

Idiopathic mesenteric phlebosclerosis treated with laparoscopic subtotal colectomy: A case report.

Idiopathic mesenteric phlebosclerosis is a rare entity characterized by chronic intestinal ischemia due to calcification and obstruction of the mesenteric veins. Here, we report a patient with idiopathic mesenteric phlebosclerosis treated with laparoscopic subtotal colectomy after evaluation by imaging studies. The patient was a 68-year-old Japanese woman with recurrent abdominal pain who had taken a Chinese herbal medicine for more than 20 years. Abdominal CT showed wall thickening of the right colon with calcification of branches of the superior mesenteric vein. Colonoscopy showed cyanotic m

S-nitrosylation of AMP-activated protein kinase beta 1 drives atherosclerotic calcification in diabetes.

BACKGROUND: Diabetes constitutes a risk factor for atherosclerotic calcification, which is highly associated with phenotypic switching in vascular smooth muscle cells (VSMCs). Protein cysteine S-nitrosylation plays a crucial role in multiple cardiovascular diseases. The objective of this study is to examine whether diabetic atherosclerotic calcification is regulated by S-nitrosylation of AMP-activated protein kinase (AMPK), a regulator of VSMC phenotype switching. METHODS: The atherosclerotic plaque was induced by feeding Apoe-/- mice a high-fat diet. A low dose of streptozotocin (STZ) was us

Tender, necrotic plaques of the glans penis due to calciphylaxis.

Calciphylaxis, also known as calcific uremic arteriolopathy, is a rare, but often fatal condition involving vascular calcification that can result in tissue ischemia and cutaneous necrosis. It is most often seen in patients with renal failure among many other occasionally reported etiologies. Below, we present a rare and challenging case of calciphylaxis involving the glans penis and right leg in a man with end stage renal disease on hemodialysis.
