Epidemiology of paroxysmal positioning vertigo: correlation with seasons, climate, and pollution.

The aim of this article was to evaluate the time course of paroxysmal positioning vertigo (PPV) and to investigate correlations with environmental and seasonal factors through a retrospective statistical analysis spanning 4 years (2001-2004). Applying rigorous diagnostic criteria, we selected 575 patients (429 women and 146 men; age range, 17-94 years; mean age, 55 years for men and 56 years for women). Statistical analysis included events per month and per year. We conducted a descriptive statistical analysis to investigate the correlation between vertigo events and main environmental factors…

Three-dimensional visualization of vertebrobasilar system aneurysms in a vertigo patient.

Cerebral artery fenestration refers to a division in the lumen of an artery leading to two distinct endothelium-lined channels. At the site of turbulent flow, fenestration may predispose to an aneurysm formation. In this study, a 45-year-old woman suffered an acute vertiginous attack after cervical manipulation. The MRI scan demonstrated basilar artery duplication and tortuous left distal vertebral artery impinging on the cisternal segment of the cochleovestibular nerve. Subsequent three-dimensional (3D) reconstruction by MR imaging showed vertebrobasilar artery fenestrations with aneurysms. U

Efficacy of the barbecue manoeuvre in benign paroxysmal vertigo of the horizontal canal.

UNLABELLED: Five to ten percent of benign paroxysmal positional vertigo are caused by the horizontal semi-circular variant (h-BPPV). In this study, we reviewed the efficacy of the Barbecue repositioning manoeuvre in h-BPPV, and we assessed the possible effect of different factors on the outcome. Barbecue manoeuvre consists in turning the supine patient around his longitudinal axis toward the unaffected side until 360 degrees are accomplished. After every 90 degrees step the patient is maintained in the new position for 30 s. We reviewed 46 patients with h-BPPV, treated by barbecue rotation fro…

The effects of caloric vestibular stimulation on EEGs in patients with central vertigo.

The EEGs of 30 patients with central vertigo were examined by caloric vestibular stimulation (CVS). The control group consisted of 30 persons without vertigo disturbances. In the standard procedure (and after hyperventilation), there was no statistically significant difference between the two groups of patients. In comparison to the EEGs of healthy persons, CVS produced statistically significant differences ( P<0.001) in the EEGs of patients with vertigo.

[The European Evaluation of Vertigo (EEV) scale: a clinical validation study].

Up until now, the questionnaires used to evaluate vertigo have been self-administered questionnaires that rate either symptoms and/or their consequences. In contrast, the European Evaluation of Vertigo scale (EEV) is a physician-administered questionnaire that only assesses symptoms of the vestibular syndrome: illusion of movement, duration of illusion, motion intolerance, neurovegetative signs, and instability. Twenty-six ear, nose, and throat (ENT) specialists participated in this validation study conducted among 123 patients suffering from different types of vertigo. The reliability, respon…

Electronystagmographic findings in patients with acute cerebral vascular disease and vertigo.

A neurootologist examined 233 successive cases of acute disabling vertigo from the Clinic of Neurology of Turku University Central Hospital. On magnetic resonance imaging (MRI), 24 patients had evidence of acute cerebral or cerebellar brain infarction. The examination scheme included a case history analysis, clinical neurootological examination and electronystagmography (ENG). The audiological examination included pure-tone thresholds and speech audiometry. The ENG analyses were done by two neurootologists who had no information about the aetiology of the diseases. A central disturbance in ENG…

Treatment of benign paroxysmal positional vertigo: no need for postmaneuver restrictions.

The liberatory maneuver of Semont is an effective physical treatment for benign paroxysmal positional vertigo. It works because it causes otoconia to move out the posterior canal. The effectiveness of the maneuver is thought to be indicated by the appearance of a liberatory nystagmus. After the maneuver, patients are usually instructed to keep their heads erect for several days and not to lie on the pathologic side for about a week. Here we investigated the prognostic value of liberatory nystagmus and whether restrictions are necessary after treatment. Fifty-six patients with posterior canal b…

[Diagnosis of common causes of vertigo using a structured clinical history].

The structured clinical history is the most sensitive test for diagnosing vertigo. Its diagnostic effectiveness on the first visit was analyzed and key signs and symptoms with high predictive value for common causes of vertigo were identified. One hundred outpatients who complained of dizziness or loss of balance were evaluated using a structured clinical interview. Each questionnaire was examined independently by three blinded investigators, who assigned a diagnosis and identified the elements of the history that figured most prominently in the diagnosis. The gold standard was defined as inde…

Posterior fossa vestibular nerve section for the management of peripheral vertigo.

Patients with vertigo resistant to conservative treatment require surgical management. Between March 1991 and August 1996, vestibular nerve sections were performed in 108 patients with peripheral vertigo not responding to conservative treatment. The diagnoses were classic Ménière's disease in 96 patients and recurrent vestibulopathy in 12 patients. Combined retrosigmoid retrolabyrinthine (n = 106) and retrolabyrinthine (n = 2) approaches were used. Patients were grouped according to follow-ups of less than 2 years and more than 2 years. In the former group (n = 49), hearing preservation and ve…

Click evoked myogenic potentials in the differential diagnosis of acute vertigo.

OBJECTIVE: In response to loud clicks, a vestibular evoked potential can be recorded from sternocleidomastoid muscles, called "click evoked myogenic potential" (CEMP). This paper reports on the usefulness of CEMP in the differential diagnosis of acute vertigo of presumed vestibular origin. METHODS: CEMP was examined in 40 patients with acute vertigo of vestibular origin (26 with acute peripheral vestibulopathy, five with Ménière's disease, three with benign paroxysmal positioning vertigo (BPPV), six with psychogenic vertigo) and the results compared with standard caloric reaction (CR). For CE…

Vertigo secondary to hypotension: the relationship between therapeutic effects and hearing.

Patients with vertebrobasilar insufficiency tend to develop transient, repeated vertigo. However, diagnostic criteria by which to definitively diagnose circulatory disturbance of the vertebrobasilar system have yet to be established. Thus, in order to determine such criteria the relationship between the therapeutic effects on clinical symptoms and hearing in 40 patients with vertigo secondary to hypotension were investigated in the present study. In most patients, vertigo was perceived as a floating or spinning sensation which occurred spontaneously or during positional changes and lasted for …

Vestibular neuronitis: a review of a common cause of vertigo in general practice.

Vestibular neuronitis is an interesting condition characterized by the acute onset of vertigo, nausea and vomiting, in the absence of hearing loss or tinnitus. There is often evidence of a recent or concurrent upper respiratory tract infection. The disease follows a benign course of between two days and six weeks. It often occurs in epidemics. Following the acute attack, mild transitory episodes of dizziness may recur over a period of 12 to 18 months. Clinical and histopathological evidence suggests that it is caused by an isolated lesion of the vestibular nerve, although the exact aetiology r…
