Segmental organization of vestibulospinal inputs to spinal interneurons mediating crossed activation of thoracolumbar motoneurons in the neonatal mouse.

Vestibulospinal pathways activate contralateral motoneurons (MNs) in the thoracolumbar spinal cord of the neonatal mouse exclusively via axons descending ipsilaterally from the vestibular nuclei via the lateral vestibulospinal tract (LVST; Kasumacic et al., 2010). Here we investigate how transmission from the LVST to contralateral MNs is mediated by descending commissural interneurons (dCINs) in different spinal segments. We test the polysynaptic nature of this crossed projection by assessing LVST-mediated ventral root (VR) response latencies, manipulating synaptic responses pharmacologically,

Contralateral suppression of distortion-product otoacoustic emissions: a potential diagnostic tool to evaluate the vestibular nerve.

The amplitude of distortion-product otoacoustic emission (DPOAE) is suppressed in one ear when the contralateral ear is subjected to sound stimulation. Contralateral suppression of DPOAE is the phenomenon resulted by the efferent cochlear innervation on the outer hair cells via medial olivocochlear bundle (MOCB) and inferior vestibular nerve. We assumed that DPOAE would not be suppressed by contralateral sound stimulation in patients with vestibular nerve lesion as long as the specific pathway conveying that efferent innervation is affected. To test this hypothesis, we compared the amount of D

Botulinum Neurotoxin for Postoperative Facial Nerve Paralysis: A Technical Note.

Excision of a vestibular schwannoma or any lesion in close proximity to the seventh cranial nerve (facial nerve) can compromise the nerve due to traction or, less commonly, neuropraxia. We retrospectively evaluated the effect of botulinum neurotoxin (BoNT) on preventing exposure keratitis in patients with postoperative facial nerve paralysis. We included 18 patients with postoperative lower motor neuron (LMN) facial nerve paralysis. All these patients had received BoNT injections into the upper eyelid in the postoperative period as an alternative to tarsorrhaphy. BoNT was effective in inducing

Neural response telemetry and auditory/nonauditory sensations in 15 recipients of auditory brainstem implants.

Auditory brainstem implants (ABIs) provide a means of restoring some hearing sensations to individuals with neurofibromatosis type 2 (NF2) who are deaf after vestibular schwannoma removal. In this study, neural response telemetry (NRT) was used to record electrically evoked neuronal activity near the ABI electrode array in 15 such subjects. Our interest was to investigate whether NRT recordings from the brainstem might be useful in implanting or programming ABIs. We therefore sought relationships between postoperative NRT recordings and the sensations reported by the subjects in response to th

Sporadic case resembling autosomal-dominant motor system degeneration (Azorean disease complex).

We describe a case of an adult-onset progressive dystonia with external ophthalmoplegia, occurring in a black man without a family history of neurologic disorders. Neuropathologic examination demonstrated neuronal loss and gliosis in the anterior horn and Clarke's column in the spinal cord, nuclei of cranial nerves III, VI, X, and XII, vestibular complex, lateral cuneate nucleus, lower pontine tegmentum, red nucleus, substantia nigra, and dentate nucleus. The cerebral cortex, corpus striatum, basis pontis, inferior olives, and cerebellum were spared. The clinical and pathologic findings closel

Thiamine-deficient encephalopathy in rats: effects of deficiencies of thiamine and magnesium.

Thiamine- and vitamin B-deficient encephalopathy was produced in adult Wistar rats and the effect of magnesium deprivation and/or the administration of guanidine, a magnesium antagonist, was studied. These encephalopathic rats had symmetrical lesions, edema, exudation of fibrin, spongy changes, petechial hemorrhages, neuronal degeneration and gliosis in the pontine tegmentum, mainly the lateral and medial vestibular nuclear areas. Mild pathological changes were seen in the purely thiamine- or vitamin B-deficient rats, whereas severe pathological changes occurred in thiamine- or vitamin B-defic

Familial, congenital paralysis of horizontal gaze.

Eye movements were studied in a sister and brother with familial, congenital paralysis of horizontal gaze. Horizontal and vertical eye movements were recorded with DC electro-oculography and analyzed with a laboratory computer. All horizontal, conjugate eye movements were absent (saccades, pursuit, optokinetic nystagmus, vestibulo-ocular response, and visual-vestibular responses). Voluntary vergence eye movements were preserved and were used to track visual targets. An involuntary, horizontal, pendular nystagmus was found to represent disconjugate, smooth, vergence eye movements. Vertical sacc

Temporal bone studies in anencephaly.

The principal anatomical and histological features of the temporal bones of three anencephalics are described, one a twin-headed monster, with four temporal bones. These may be summarized as follows: Malformation of ossicles. Mondini type malformation of the cochlea. Occasionally cochlear hydrops or collapse of Reissner's membrane, following apparently on its rupture. Distension of the vestibular labyrinth and oedema of the perilymphatic spaces and membranes. Rupture of the saccular wall with 'sealing off' and repair of the tears and gaps by coagulated lymph material. Fibrous obliteration of t

Cognitive function assessment in patients with uncompensated vestibular neuritis.

BACKGROUND: The vestibular system is essential for spatial orientation and is increasingly implicated in higher-order cognitive processes. OBJECTIVE: To investigate specific cognitive domains affected by unilateral vestibular neuritis (VN). METHODS: In a cross-sectional comparative study, 30 patients with uncompensated unilateral VN and 30 age- and sex-matched healthy controls were assessed. Participants completed a cognitive test battery including reaction time tasks, visuospatial memory (Corsi Block-Tapping Test), processing speed and response inhibition (Visual Stroop Task), verbal fluenc

Discordant vestibulo-ocular reflex function according to the frequency and mode of stimulation.

This study aimed to determine the incidence, pattern, and etiology of dissociated vestibulo-ocular reflex (VOR) function according to the stimulus frequency in dizzy patients. We retrospectively evaluated the results of bithermal caloric tests and video-head impulse tests (vHITs) in 1022 patients with dizziness or vertigo between July 2016 and April 2021. Patients were classified into concordant group (normal or abnormal results on both tests) and discordant group (dissociated results between two tests). Of 1022 patients, discordant group had 159 (16%), comprising abnormal horizontal vHITs wit

[Idiopathic cranial nerve failure].

Cranial nerve lesions require a thorough diagnostic work-up and known etiologies have to be excluded before the term idiopathic can be considered. The focus of the present review is on idiopathic peripheral facial nerve paralysis (Bell's palsy) for which this terminology has been established. For all other cranial nerve lesions the typical clinical signs, established etiologies and possible diagnostic pitfalls are discussed.

The differential effects of acute right- vs. left-sided vestibular failure on brain metabolism.

The human vestibular system is represented in the brain bilaterally, but it has functional asymmetries, i.e., a dominance of ipsilateral pathways and of the right hemisphere in right-handers. To determine if acute right- or left-sided unilateral vestibular neuritis (VN) is associated with differential patterns of brain metabolism in areas representing the vestibular network and the visual-vestibular interaction, patients with acute VN (right n = 9; left n = 13) underwent resting state (18)F-FDG PET once in the acute phase and once 3 months later after central vestibular compensation. The contr

Intraobserver reliability of posturography in patients with vestibular neuritis.

The aim of the study was to establish the intraobserver reliability of a posturographic method in patients (n = 34) with vestibular neuritis. Intraclass correlation coefficients (relative reliability) for all parameters and test positions (ALL(mean)) ranged from 0.71 (95% CI: 0.41-0.85) to 0.92 (95% CI: 0.84-0.96). Absolute reliability (coefficient of variation) ranged between 3.1% (95% CI: 2.60-8.67) and 42.3% (95% CI: 40.7-74.5). Reliability of single test positions is much lower. The posturographic system showed good relative and satisfactory absolute intraobserver reliability for ALL(mean)

Three-dimensional video-oculography in patients with vestibular neuritis.

CONCLUSIONS: The nystagmus recorded without visual fixation contained all three components in most patients with vestibular neuritis (VN). The nystagmus was unidirectional, strongly visually suppressed with the loss of vertical component in most patients, and it obeyed Alexander's law. Video-oculography (VOG) is a non-invasive tool for detailed analysis of nystagmus, and hence may improve differential diagnostics in dizzy patients. OBJECTIVE: To characterize three-dimensional nystagmus findings in patients with acute VN using the VOG technique. PATIENTS AND METHODS: A prospective study in 16

Head-shaking nystagmus depends on gravity.

In acute unilateral peripheral vestibular deficit, horizontal spontaneous nystagmus (SN) increases when patients lie on their affected ear. This phenomenon indicates an ipsilesional reduction of otolith function that normally suppresses asymmetric semicircular canal signals. We asked whether head-shaking nystagmus (HSN) in patients with chronic unilateral vestibular deficit following vestibular neuritis is influenced by gravity in the same way as SN in acute patients. Using a three-dimensional (3-D) turntable, patients (N = 7) were placed in different whole-body positions along the roll plane

The vestibulothalamic projections in the cat studied by retrograde axonal transport of horseradish peroxidase.

Horseradish peroxidase (HRP) was injected or iontophoretically ejected in various thalamic nuclei in 63 adult cats. In 11 other animals HRP was deposited outside the thalamic territory. The number and distribution of labelled cells within the vestibular nuclear complex (VC) were mapped in each case. To a varying degree all subgroups of VC appear to contribute to the vestibulothalamic projections. Such fibres are distributed to several thalamic areas. From the present investigation it appears that generally speaking, there exist three distinct vestibulothalamic pathways with regard to origin as

CANVAS an update: clinical presentation, investigation and management.

BACKGROUND: Cerebellar Ataxia with Neuropathy and bilateral Vestibular Areflexia Syndrome (CANVAS) is a multi-system ataxia which results in cerebellar ataxia, a bilateral vestibulopathy and a somatosensory deficit. This sensory deficit has recently been shown to be a neuronopathy, with marked dorsal root ganglia neuronal loss. The characteristic oculomotor clinical sign is an abnormal visually enhanced vestibulo-ocular reflex. OBJECTIVE: To outline the expanding understanding of the pathology in this condition, as well as diagnostic and management issues encountered in clinical practice. ME

Azorean disease of the nervous system.

We studied a family of Portuguese ancestry from the Azores who suffered a progressive neurologic disease characterized by gait ataxia, features similar to Parkinson's disease in some patients, limitation of eye movements, widespread fasciculations of muscles, loss of reflexes in the lower limbs, followed by nystagmus, mild cerebellar tremor and extensor plantar responses. Two post-mortem examinations revealed loss of neurons and gliosis in the substantia nigra, nuclei pontis (and in the putamen in one case) as well as in the nuclei of the vestibular and other cranial nerves, columns of Clarke
