[Recurrence of pseudomyxoma peritonei with liver and splenic metastases: about a case and literature review].

Pseudomyxoma peritonei (PMP) is a clinicopathologic syndrome characterized by mucinous ascites and pools of mucin resulting in neoplastic mucinous epithelium in the peritoneal cavity. PMP is uncommon and it is characterized by clinical and unusual pathologic manifestations posing diagnostic and therapeutic problems. Involvement of abdominal viscera and lymph node metastases are rare and sporadic cases have been reported in the literature. We here report the case of a 56-year old patient who had undergone two operations for PMP of appendicular origin presenting with progressive abdominal pain f…

Congenital diaphragmatic hernia in the adult.

Bochdalek hernia is a rare surgical condition primarily diagnosed in infants. Morgagni hernia is more commonly identified in adults. A subset of patients with a Bochdalek hernia, especially those with a right-sided defect, may also be diagnosed during adulthood. Both adult forms of CDH may be discovered incidentally or diagnosed as part of the investigation of nonspecific gastrointestinal or respiratory symptoms. It is recommended that all adult CDH patients undergo surgical repair to prevent incarceration and strangulation of abdominal viscera. Currently, many reports have demonstrated the sa…

Health status of a population of nutria (Myocastor coypus) living in a protected area in Italy.

Ninety trapped nutria (Myocastor coypus) from a protected area of Piedmont (Italy), including the Po river, were examined for the prevalence for lesions in major viscera, selected serum antibodies and enteric bacteria. Coccidial lesions in the liver included cholangitis, calcification and necrosis. Renal lesions were nonsuppurative interstitial nephritis and a single case of renal adenocarcinoma. The lungs had a 41.1% prevalence of nonsuppurative interstitial pneumonia. Ten of 87 sera (11.5%) had antibodies against Leptospira bratislava, 3 of 87 (3.4%) against Leptospira ichterohaemorrhagiae, …

Fine-needle aspiration cytology of pulmonary rheumatoid nodule: case report and review of the major cytologic features.

Patients with rheumatoid disease may develop extra-articular lesions, affecting viscera and soft tissues. Pulmonary rheumatoid nodules show morphologic features reminiscent of a necrotizing inflammation of rheumatoid synovitis and differ from subcutaneous rheumatoid nodules only by their location, extension, and size. Although cytologic studies on pleural effusions in rheumatoid disease have long been performed, there are no more than three reports concerning the fine-needle aspiration (FNA) diagnosis of pulmonary rheumatoid nodules. The authors report on a case of a 62-yr-old woman with rheum…

Teratogenic effects of dilantin on thoraco-abdominal organs of developing chick embryos.

Congenital anomalies on some viscera like heart, liver and kidney have been investigated in chick embryos after a single injection of dilantin (3 mg/egg), a known antiepileptic drug, on 4th day of incubation. On 19th day of incubation, chick embryos were collected to observe the gross malformations and histological changes in heart, liver and kidney. On gross examination, visceroptosis (29%), thin anterior abdominal wall (28%), ectopia cordis (10%) and dextrocardia (1%) were observed. Histological examination of the kidney revealed glomerular degeneration in kidney while in liver, dilated cent…

Hernia repair with expanded polytetrafluoroethylene.

Prostheses of expanded polytetrafluoroethylene (e-PTFE) were used to repair 89 abdominal wall defects in which primary closure would produce undue tension on tissue. Over a 52-month period (median follow-up: 24 months), we observed three wound infections, one in a clean wound, and four hernia recurrences. No other complications were noted. Twenty-one e-PTFE grafts were placed directly over intraperitoneal viscera without clinical evidence of adhesions leading to intestinal obstruction. In grafts inspected postoperatively, no apparent evidence of fatigue or fragmentation was observed. Histologi…

What the practicing nurse should know about thiamine.

Thiamine (vitamin B1) is an essential nutritional component that acts as a coenzyme in the oxidative decarboxylation of alpha-keto acids. It … al antithiamine factors are also found in common foods. For example, a thermal labile factor in the viscera of fresh water fish and tea leaves antagonizes thiamine.

An immunohistochemical study of canine disseminated aspergillosis.

An immunohistochemical study of 25 lesions from 7 dogs with disseminated aspergillosis (Aspergillus terreus) is presented. All had multiple fungal granulomas in many viscera, with centres of necrotic tissue and hyphal elements surrounded by a mixed infiltrate of predominantly mononuclear cells. Within these lesions, hyphae coated with immunoglobulin (IgG, IgM, IgA) and complement (C3, C4) were identified, together with peri-lesional mononuclear cells that reacted with antisera directed towards either IgG, IgM, IgA or a T lymphocyte marker (MUII). A conspicuous feature was the prominent hyphal …

Effective peritoneal dialysis in an infant with extensive resection of the small intestine.

Acute renal failure and pulmonary edema developed in a 1-month-old baby with necrotizing enterocolitis and hypovolemic shock. Following resection of 93% of her small intestine and mesentery, peritoneal dialysis was started; it was terminated 96 h later with complete recovery of renal and pulmonary functions. Two weeks later, while on total parenteral nutrition, the infant developed fulminant septicemia and died. Postmortem examination showed normal kidneys. Surface measurement of the abdominal wall and viscera revealed that the actual peritoneal surface area of the patient was 671 cm2, only 44…

[Rare abdominal and peritoneal localizations of hydatid cysts. Report on two cases (author's transl)].

Rarely observed isolated peritoneal hydatid cysts, without initial hepatic lesions or later development in other viscera, were detected in two patients. The diagnosis of hydatidosis confined to the mesocolon was established pre-operatively in the first case, enabling mebendazole treatment to be instituted. In the second case, however, pre-operative diagnosis was not possible, demonstrating the sometimes misleading appearances found in this affection. Aetiopathogenic and diagnostic features are briefly discussed. Therapy raises problems, and though good results have recently been obtained with …

Radiographic Identification of Visceroptosis in Patients With Hypermobile Ehlers-Danlos Syndrome With Functional Gastrointestinal Symptoms Compared With Healthy Subjects.

INTRODUCTION: Visceroptosis is a potential cause of gastrointestinal symptoms in hypermobile Ehlers-Danlos syndrome (hEDS). METHODS: We systematically examined the prolapse of abdominal organs below their natural supine position (visceroptosis) during the upright small bowel barium study in healthy and hEDS subjects with irritable bowel syndrome. RESULTS: Comparison of age- and sex-matched healthy (n = 20) and hEDS (n = 10) subjects did not show any significant difference in dynamic movement of the viscera. Subgroup analysis did not demonstrate any correlation between the degree of prolapse,…

Variant anatomy of the iliac veins and presence of two venous rings around the arteries in the pelvis.

Internal iliac vein drains the pelvic viscera, gluteal region, and the perineal region. Knowledge of its variations is of importance to radiologists, gynecologists, and orthopedic surgeons. We found one of the rare variations of the internal iliac vein during our cadaveric dissections. The right internal iliac vein had anterior and posterior trunks. The anterior trunk joined the external iliac vein to form the common iliac vein. The posterior trunk terminated into the common iliac vein. There was a broad communication between anterior and posterior trunks. Further, there were two venous rings …

Severe vaginal evisceration at a military treatment facility.

Vaginal evisceration is a rare extrusion of pelvic and/or abdominal viscera through a defect or rupture of the vaginal wall, and it is a true surgical emergency. With less than 100 cases reported in the literature, we present a case of an elderly woman in her 80s who was brought to a military treatment facility with vaginal evisceration after sustaining a ground-level fall. She was activated as a level 1 trauma by the emergency department, which led to efficient evaluation by the trauma team and subsequent emergent surgery to reduce the evisceration and repair the defect. She went on to recove…

Sigmoid Volvulus in the Setting of Type 4 Hiatal Hernia: An Uncommon Presentation and Literature Review.

Hiatal hernias occur when intra-abdominal contents protrude into the diaphragmatic opening. Of the four classifications, Type 4 hiatal hernias are the most rare and severe. They develop from herniation of the gastroesophageal junction and abdominal viscera other than the stomach into the thoracic cavity. The resulting increase in intrathoracic pressure can cause a wide variety of symptoms on presentation and potentially lead to misdiagnosis. We present a rare case in which a 78-year-old woman presented with nonspecific symptoms and was diagnosed with incarcerated Type 4 hiatal hernia with sigm…

Jejunal Undifferentiated Spindle Cell Sarcoma with Intussusception Revealed by 18F-FDG PET/CT.

Spindle cell sarcoma is a malignant tumor with low incidence. They can occur in the soft tissue, bone, or viscera. The characteristics of morphology, density, and metabolism of spindle cell sarcoma are related to the location of the lesion. A 61-year-old woman presented with vomiting after eating for 2 weeks. Signs of peritoneal irritation were involved, but no response for symptomatic treatment included antiemetic and antispasmodic therapy. Abdominal computed tomography (CT) indicated a mass in the intestinal tract in the pelvic cavity. Then, 18F-fluorodeoxyglucose (18F-FDG) positron emission…

Isolated diaphragmatic hydatid cyst: a rare entity in the paediatric population.

Isolated primary diaphragmatic hydatid disease (HD) occurs in approximately 1% of adult cases. However, this unique presentation of a pediatric diaphragmatic cystic mass has not been previously described in the literature. This study reports a rare case of a 12-year-old girl who was diagnosed with a diaphragmatic hydatid cyst. Surgical exploration via subcostal incision revealed an isolated cyst, free from the thoracic and abdominal viscera. Cystotomy, removal of daughter cysts, and excision of the pericyst cavity were performed, followed by diaphragmatic repair. Histopathological examination …

Nontraumatic right diaphragmatic hernia with malrotated left liver lobe incarceration: An unusual case report with literature review.

Diaphragmatic hernia is an unusual finding, especially in emergency settings and in the absence of trauma. Imaging plays a crucial role, with various CT signs of diaphragmatic rupture having been described, including the "dangling diaphragm," "absent diaphragm," "collar sign," "hump sign," "fascia sign," and "dependent viscera sign". We report an unusual case of a 53-year-old woman who presented with exertional dyspnea and asthenia. A CT scan was performed, revealing a large right diaphragmatic hernia involving an abnormally rotated liver and several other organs. The association between a mal…

Uniportal left middle lobectomy in a patient with situs inversus totalis: a case report.

BACKGROUND: Situs inversus totalis (SIT), a rare recessive autosomal disease, involves the complete transposition of the thoracic and abdominal viscera in the left-right axis. Patients with SIT combined with lung cancer are extremely uncommon. CASE PRESENTATION: We present a case of a 57-year-old woman with SIT who underwent uniportal video-assisted thoracoscopic left middle lobectomy for adenocarcinoma of the lung. The procedure was performed safely with adequate anatomical identification and careful intraoperative manipulation based on the preoperative three-dimensional-computed tomography …

Recurrent Groin Hernia Containing the Ovary and Fallopian Tube.

A 46-year-old female presented to the emergency room with complaints of right groin pain. A palpable mass was found inferior to the right inguinal ligament. Computed tomography showed evidence of a viscera-containing hernia sac within the femoral canal. The patient was taken to the operating room for exploration of the hernia where a well-perfused right fallopian tube and right ovary were identified within the sac. These contents were reduced, and the facial defect repaired primarily. The patient was discharged and has since been seen in clinic with no residual pain or recurrence of her hernia…

A Rare Case of Left-Sided Gastroschisis in a Human Museum Specimen.

Gastroschisis is a congenital defect in the anterior abdominal wall resulting in herniation of the abdominal viscera without any fetal membrane covering it. It usually occurs to the right of a normally inserted umbilical cord. The anomaly is associated with intrauterine growth retardation, stillbirth, and preterm delivery. We found a preserved specimen of a 17- to 20-week-old male human fetus presenting with gastroschisis in the Departmental Museum of Anatomy of the Institute of Medical Sciences and SUM Hospital, Bhubaneshwar, a medical college in Eastern India. The fetus showed a hiatus on th…
