The pituitary-adrenal axis activity was evaluated in 43 patients, treated for congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency, by measuring plasma ACTH, 17-hydroxyprogesterone (17-OHP), testosterone, and aldosterone. Dynamic studies were performed by injecting 250 micrograms synthetic ACTH im and collecting blood samples 1 h later for steroid analysis. The present study reports steroid metabolism by corpora lutea (CL) obtained from skunks with diapausing embryos ('delay' CL) and with activated embryos (activated CL). CL from both reproductive periods were incubated with various radioactive precursors. INTRODUCTION: One to six percent of women with hirsutism have 21-hydroxylase deficiency. Beyond these classical congenital forms, the most frequent cases present non-specific signs of hyperandrogenism. Late-onset congenital adrenal hyperplasia is a mild genetic defect in steroidogenesis that presents with hirsutism and menstrual irregularities and responds to specific treatment with dexamethasone sodium phosphate. Dexamethasone (DXM) has been used to reduce clinical and/or biological hyperandrogenism in 18 adolescent patients treated by hydrocortisone and fludrocortisone for congenital virilizing adrenal hyperplasia (CAH). The doses and duration of treatment did vary among these patients. Androgen excess has been suppressed in all cases, even treated with low doses. The endocrine studies revealed a slight elevation of the plasma progesterone, 17 alpha-hydroxyprogesterone, testosterone and oestrone levels, and an increased urinary oestrogen excretion. Fifteen transsexuals were prepared for surgery with estrogen treatment. The response to human chorionic gonadotropin (hCG) in the untreated state was similar to that of normal subjects when testosterone (T), estradiol-17 beta (E2), 17 alpha-hydroxyprogesterone (17 alpha-OHP), progesterone (P), 4-androstenedione (delta 4A), and dehydroepiandrosterone (DHA) were used as indices.
