OBJECTIVES: To examine the efficacy and safety of abatacept (ABT) in patients with amyloid A (AA) amyloidosis secondary to rheumatoid arthritis (RA), and to speculate about the immunologic association of ABT with AA amyloid deposit regression.
METHODS: We administered ABT to 70- and 65-year-old Japanese women with RA and AA amyloidosis. We quantified serum cytokine concentrations and analysed regulatory T lymphocytes (Treg cells) via flow cytometry. We also studied AA amyloid deposits via histopathology and immunohistochemistry.

We report the successful use of abatacept and sodium thiosulfate in a patient with severe recalcitrant juvenile dermatomyositis complicated by ulcerative skin disease and progressive calcinosis. This combination therapy resulted in significant reductions in muscle and skin inflammation, decreased corticosteroid dependence, and halted the progression of calcinosis.

We experienced a case of Bowenoid actinic keratosis on the left wrist joint in a patient receiving abatacept for rheumatoid arthritis. Simple treatment with imiquimod, initiated with the continuous use of abatacept, did not improve the lesion. Topical imiquimod successfully cured the lesion only after abatacept discontinuation in the patient. Our case suggests that abatacept has depleted the production of tumor necrosis factor-α by macrophages upon contact with imiquimod, which is a toll-like receptor 7 ligand.
