Management of acoustic schwannoma.

PURPOSE: To discuss the optimal management for patients with acoustic schwannoma. MATERIALS AND METHODS: Review of the pertinent literature. RESULTS: Microsurgery, stereotactic radiosurgery, and fractionated radiotherapy result in cure rates that approximate 90% at 5 years. Depending on tumor extent and surgical approach, the morbidity of microsurgery may exceed that of stereotactic radiosurgery and fractionated radiotherapy. Patients with useful hearing before treatment may have a higher likelihood of hearing preservation after radiotherapy compared with radiosurgery. CONCLUSION: Both micr

[The effects of acoustic overstimulation].

Basic aspects of acoustic trauma are presented. Exposure to loud noise leads to an acoustic traumatization with a temporary threshold shift initially and, with increasing exposure, intensity and duration, a permanent hearing loss. Impulse sound such as hammer blows on metal, gun shots and other detonations reaching peak levels of 160 to 180 dB is particularly hazardous to the inner ear. Playing loud musical instruments such as trumpets or percussion may also lead to hearing damage. Less dangerous than often believed is listening to electronically amplified music with walkmen, at discos or rock

[Treatment of acoustic neuroma].

BACKGROUND: Acoustic neuroma is the most common tumour in the cerebello-pontine angle. MATERIAL AND METHODS: We present the results after surgery in 99 cases, and the natural course of the disease in 82 patients followed for up to 20 years (mean 3 years). RESULTS: During the observation period, 43% of the tumours increased in size. 36% of patients suffered increasing hearing loss. Total removal was accomplished in 92 cases. In 12 cases hearing preservation was attempted; successfully in five cases. 72 patients had normal or near normal postoperative facial nerve function, whereas 19 patients

Treatment of acoustic trauma.

The aim of this study is to determine the actual efficiency of the present medical treatments of acoustic trauma. Untreated and treated groups of guinea pigs are exposed to a traumatic noise exposure, inducing up to 60-dB threshold shift. The recovery is followed for up to 14 days. The first results indicate that in some animals the recovery of the threshold shifts are complete despite the fact that significant areas of hair cells are damaged. The most widely used medical treatments of acoustic trauma (oxygenotherapy, carbogen, hyperbaric oxygen, vasoactive agents, and corticotherapy) have bee

The acoustic properties of snores.

This study was undertaken in an attempt to characterize the acoustic properties of snoring sounds in the time and frequency domains, and to correlate between these properties and the mechanical events underlying their production. Three experimental set-ups were used: 1) Dog model--six mongrel dogs, in which partial upper airway obstruction was created by an implanted supraglottic balloon. Flow, supraglottic pressure, and snoring sounds were recorded during different degrees of obstruction. Fifteen to 20 snores from each dog (total 100 snores) were analysed. 2) Simulated human snores--Six simul

Bilateral acoustic neuromas.

This article reviews 12 patients with bilateral acoustic neuromas. The sex incidence was equal and the mean age at diagnosis was 26.2 years. The family history was positive in nine of the patients. Five patients have had incomplete surgical removal of acoustic neuromas on both sides. Two of them are completely deaf and the other three have severe sensorineural hearing loss in one ear and no hearing in the other ear. In five patients the tumour on one side has been operated on and the other side is being observed with at least short-term preservation of good hearing. The remaining two patients

Acoustic effects of lightning.

The effects of lightning on the audiovestibular apparatus vary with the degree of injury. This depends on whether the individual is struck directly or indirectly by lightning. We reported two cases where lightning has caused trauma to audiovestibular apparatus and have reviewed the available literature. One patient was struck directly by lightning leading to rupture of the tympanic membrane and a conductive hearing loss. The other patient was struck indirectly via telephone cable and had a mixed hearing loss with tympanic membrane intact.

Radiosurgery of acoustic neurinomas.

Eighty-five patients with acoustic neurinomas underwent stereotactic radiosurgery with the gamma unit at the University of Pittsburgh (Pittsburgh, PA) during its first 30 months of operation. Neuroimaging studies performed in 40 patients with more than 1 year follow-up showed that tumors were smaller in 22 (55%), unchanged in 17 (43%), and larger in one (2%). The 2-year actuarial rates for preservation of useful hearing and any hearing were 46% and 62%, respectively. Previously undetected neuropathies of the trigeminal (n = 12) and facial nerves (n = 14) occurred 1 week to 1 year after radiosu
