INTRODUCTION: Introduction: Acromioclavicular (AC) joint dislocations represent a frequent injury among young- and middle-aged adults, often resulting from high-energy trauma such as road traffic accidents and falls. While surgical intervention is standard for high-grade (Rockwood III-VI) injuries, the optimal technique remains debated. This study evaluates the clinical and functional outcomes of the Endobutton loop system in the management of acute AC joint dislocations.
MATERIALS AND METHODS: This prospective observational study was conducted at the Department of Orthopaedics, Gandhi Medica

BACKGROUND: The acromioclavicular (AC) and coracoclavicular (CC) joints provide anteroposterior and superoinferior stability, respectively, of the scapula in relation to the lateral clavicle. Chronic or high-grade (≥IIIB) AC dislocations should be treated with a structured rehabilitation program, and if symptoms persist, they typically require reconstruction to restore alignment, relieve pain, and improve scapular function.
INDICATIONS: This technique is indicated for chronic AC joint dislocations that are Rockwood grade IIIB-VI injuries that have failed conservative management. Patients shou

OBJECTIVES: Management of acromioclavicular (AC) joint injuries remains controversial regarding ideal surgical indications and technique. While loss of reduction following AC reconstruction is common, its relationship to clinical outcomes is uncertain. The purpose of this study was to evaluate outcomes and potential predictors of suboptimal results in patients with structural failure following AC reconstruction.
METHODS: Patients with structural failure following AC reconstruction from 2013-2019 were identified, defined as 50% loss of coronal AC joint reduction between immediate and final pos

Acromioclavicular joint dislocation, particularly Rockwood type III, is a common injury and a topic of ongoing debate regarding surgical treatment options. Numerous surgical techniques have been proposed, but it is crucial to focus on methods that minimize invasiveness and promote faster clinical and functional recovery. This article presents an arthroscopic technique for repairing the dislocation using only 2 arthroscopic portals. The approach emphasizes reducing the complexity of the procedure while ensuring efficient results. Additionally, the article analyzes the benefits of this minimally…

Anatomical reconstruction of the coracoclavicular ligaments involves drilling the base of the coracoid or looping a graft around it, placing the axillary nerve at risk for injury. Rockwood type V acromioclavicular (AC) separation injuries involve disruption of the AC joint capsule and coracoclavicular ligaments, resulting in inferomedial displacement of the scapulohumeral complex and alteration of the normal anatomical relations of the shoulder girdle structures. This study evaluated the effect of Rockwood type V AC separation on the anatomical relation of the axillary nerve to the coracoid ba

BACKGROUND: An anatomic double-tunnel (DT) reconstruction technique has been widely adopted to reconstruct the ruptured coracoclavicular (CC) ligaments seen with high-grade acromioclavicular (AC) joint injuries. However, the anatomic DT reconstruction has been associated with the risk of clavicle fractures, which may be problematic, particularly for contact athletes. Purpose/Hypothesis: The purpose was to compare a single-tunnel (ST) CC ligament reconstruction for AC joint injuries with the native state as well as with the more established anatomic DT CC ligament reconstruction. The hypothesis

OBJECTIVE: Injury to the sternoclavicular joint is a rare phenomenon that has implications for the shoulder complex. Limited literature exists on optimal physical therapy rehabilitation after a sternoclavicular ligament sprain. This case report details the physical therapy management and outcomes of a patient with a posterior sternoclavicular joint sprain.
CASE DESCRIPTION: The patient was a 34-year-old female who sustained a grade I posteriorly directed sternoclavicular sprain during a motor vehicle accident. She received a combination of acromioclavicular joint mobilizations and therapeutic…

BACKGROUND: Understanding the biomechanical roles of the acromioclavicular and coracoclavicular ligaments is essential for optimizing surgical treatment of acromioclavicular joint injuries. This systematic review and meta-analysis investigates the tensile characteristics of the acromioclavicular joint to identify the ligamentous structures contributing to stability.
METHODS: A comprehensive search of the Cochrane, Scopus, and PubMed databases was conducted through January 2025 to identify cadaveric studies reporting tensile characteristics of the intact acromioclavicular joint ligaments; stud…

Acromioclavicular joint injuries are a common shoulder injury encountered by orthopaedic surgeons. Many different surgical techniques have been described for the operative treatment of these injuries with no single, clear gold standard technique on which surgeons agree. Among the most common complications after surgical management of acromioclavicular injuries are loss of reduction, infection, fracture of clavicle or coracoid, and need for reoperation. We propose an arthroscopic-assisted, tunnel-free surgical technique using a tibialis anterior allograft combined with a FiberTape Cerclage (Art…

BACKGROUND: Injuries of the acromioclavicular joint (ACJ) are common shoulder injuries that often lead to pain and dysfunction of the affected shoulder. Regardless of operative or nonoperative treatment, a relatively large number of patients remain symptomatic and experience pain. However, the specific source of persistent pain in the ACJ remains ambiguous.
PURPOSE: To investigate the presence of sensory nerve fibers or pain-generating neurotransmitters within the intra-articular disk of the ACJ to determine its potential role as an independent pain generator in ACJ disorders.

PURPOSE: The aim of this study was to evaluate the outcome of a minimally invasive surgical technique for the treatment of patients with acromioclavicular joint dislocation.
METHODS: Sixteen patients with complete acromioclavicular joint dislocation were enrolled in this study. All patients were asked to follow the less active rehabilitation protocol post-operatively. Computed tomography with 3-D reconstruction of the injured shoulder was performed on each patient post operatively for the assessment of the accuracy of the suture anchor placement in the coracoid process and the reduction of th…

Although calcium pyrophosphate dehydrate (CPPD) deposition disease, including pseudogout, is a relatively common disease affecting the major joints, especially the knee, pseudogout in the acromioclavicular (AC) joint is very uncommon. We describe two cases of acute pseudogout attacks of the AC joint in elderly patients. Radiological findings showed chondrocalcinosis in the AC joints. Microscopically, aspirated articular synovial fluid demonstrated CPPD crystals. Conservative therapy with non-steroidal anti-inflammatory drugs completely resolved all symptoms within 1 week.

We report a modified surgical technique for reconstruction of coracoclavicular and acromioclavicular ligaments after acute dislocation of acromioclavicular joint using suture anchors. We have repaired 3 consecutive type III acromioclavicular dislocations with good results. This technique is simple and safe and allows anatomical reconstruction of the ligaments in acute dislocations.

BACKGROUND: Over 100 techniques for acromioclavicular joint (ACJ) reconstruction have been described. Most of these techniques are invasive and are associated with a high complication rate. We therefore developed a new minimally invasive arthroscopic technique for reconstruction of the ACJ.
METHODS: The new operation technique is described in detail. We operated 13 patients with Rockwood IV or V dislocations of the ACJ using this new technique. Intra- and postoperative complications were recorded. Each patient was evaluated after 3, 6, and 9 months using the Constant score for shoulder functi…

Treatment of AC joint injuries and SC joint injuries continues to evolve. The risk of complications of both the operative and nonoperative management of these injuries can be minimized by the treating physician if the physician thoroughly evaluates and understands the problem. Making an accurate diagnosis of the underlying pathology and then selecting the appropriate treatment for this will minimize the risk of an associated complication. Paying attention to detail and using the appropriate technique before any operative intervention is chosen will decrease the risk of failure and complication
