Glanzmann's thrombasthenia during pregnancy complicated by large subchorionic hematoma managed with antifibrinolytics, human leukocyte antigen-matched platelet transfusion, and primary cesarean delivery: a case report.

A 29-year-old primipara with Glanzmann's thrombasthenia presented for prenatal care at 8 weeks of gestation. Pregnancy remained uncomplicated until 22 weeks of gestation when a subchorionic hematoma, measuring 5.8 × 4.1 × 6.7 cm, was diagnosed and managed outpatient. At 28 weeks of gestation, the subchorionic hematoma was significantly expanding to 11 × 13 × 3.7 cm (∼30% of the placental surface). The patient was admitted for antepartum surveillance and steroid treatment. Fetal and maternal status were reassuring. At 36 weeks of gestation, there was active extravasation from the subchorionic h

2010 Young Investigator Award winner: Therapeutic aprotinin stimulates osteoblast proliferation but inhibits differentiation and bone matrix mineralization.

STUDY DESIGN: Analysis of the effect of antifibrinolytics on in vitro bone formation. OBJECTIVE: As the direct effect of antifibrinolytics on bone formation is unknown, we examined whether antifibrinolytics routinely used in spine surgery, namely, aprotinin and aminocaproic acid, affect osteoblast function in vitro. SUMMARY OF BACKGROUND DATA: Antifibrinolytics are used in spine surgery to prevent intraoperative blood loss and decrease the need for transfusion. They are either delivered systemically or included as a component of most tissue sealants. Although the role of the fibrinolytic sys

Hyperglycemic effect of amino compounds structurally related to caproate in rats.

The chronic feeding of small amounts (0.3-3% of diet weight) of certain amino derivatives of caproate resulted in hyperglycemia, an elevated glucose tolerance curve and, occasionally, glucosuria. Effective compounds included norleucine, norvaline, glutamate, epsilon-aminocaproate, methionine, and leucine.

Aprotinin attenuates genomic expression variability following cardiac surgery.

BACKGROUND: Aprotinin was the subject of recent controversy regarding adverse clinical outcomes following cardiac surgery. We compared the role of Aprotinin and epsilon-aminocaproic acid on clinical outcomes and the attenuation of the postcardiopulmonary bypass (CPB) response at the genomic expression and cytokine (protein) level. METHODS: Thirty-nine low-risk patients undergoing coronary revascularization (CABG) and/or valve procedures using cardiopulmonary CPB were enrolled into a prospective cohort study. Aprotinin or epsilon-aminocaproic acid was administered to patients. Gene expression

Zinc acexamate inhibits gastric acid and pepsinogen secretion in the rat.

Pretreatment with zinc acexamate (25-100 mg kg-1 i.p.) inhibited acid and pepsinogen secretion in the pylorus-ligated rat. Zinc acexamate (5-50 mg kg-1 p.o.) also inhibited the increases in acid secretion induced by carbachol (10 micrograms kg-1) and 2-deoxy-D-glucose (200 mg kg-1) in the perfused stomach of the anaesthetized rat. A delayed antisecretory effect was observed with this drug on histamine induced responses. High concentrations of zinc acexamate (10(-5) - 10(-2) M) did not modify the in-vitro activity of pepsin. Administration of zinc acexamate resulted in an increase in the presen

Low-temperature depolymerization of nylon-6 and other polyamides mediated by boron-based Lewis acids.

An efficient depolymerization of nylon-6 to 6-aminocaproic acid hydrochloride (6ACA·HCl) in >99% yield is reported. The reaction proceeds under mild conditions and generates no detectable organic by-products, thereby eliminating the need for tedious purification. The obtained 6-ACA·HCl is a commercially valuable compound that can be converted to ε-caprolactam in 78% yield.

[D(--)alpha-aminobenzylpenicilline and amino acid pattern in urine (author's transl)].

Three cases of patients with D(--)alpha-aminobenzylpenicillinuria are reported. On the quantitative, urinary amino acid chromatogram we found a high ninhydrinpostitive peak between the xi-aminocaproic acid and the arginine peak. Column chromatographic and thin-layer chromatographic studies with test substances permitted to identify the unknown peak as D(--)alpha-aminobenzylpenicillin.

Antifibrinolytic agents reduce blood loss during pediatric vertebral column resection procedures.

STUDY DESIGN: Retrospective multicenter review. OBJECTIVE: To evaluate the effect of intraoperative antifibrinolytic (AF) agents on blood loss associated with vertebral column resection (VCR) procedures for pediatric patients. SUMMARY OF BACKGROUND DATA: VCR procedures may be associated with substantial blood loss. METHODS: A multicenter review of 147 patients (aged <21 yr) who underwent VCR as part of their spinal deformity correction was conducted. Estimated blood loss (EBL) was calculated as percentage of blood volume (BV) (EBL/BV × 100), which was normalized on the basis of the number o

[Fulminant and gradual evolution of the clinical manifestations of lumbar osteochondrosis].

A vertebroneurological examination of 187 patients with lumbar osteochondritis revealed the presence of two variants of its exacerbation: fulminant (occurring within 1-3 days) and gradual (with a longer period of progress). The fulminant variant was associated with tension of the cellular immunity expressed in a decreased index of leukocyte migration inhibition and the corresponding therapeutic effect of epsilon-aminocaproic acid.

Safety of antifibrinolytics in 6583 pediatric patients having craniosynostosis surgery: A decade of data reported from the multicenter Pediatric Craniofacial Collaborative Group.

BACKGROUND: Antifibrinolytics such as tranexamic acid and epsilon-aminocaproic acid are effective at reducing blood loss and transfusion in pediatric patients having craniofacial surgery. The Pediatric Craniofacial Collaborative Group has previously reported low rates of seizures and thromboembolic events (equal to no antifibrinolytic given) in open craniofacial surgery. AIMS: To query the Pediatric Craniofacial Collaborative Group database to provide an updated antifibrinolytic safety profile in children given that antifibrinolytics have become recommended standard of care in this surgical p

Subarachnoid hemorrhage in the African-American population: a cooperative study.

The clinical outcome of patients following subarachnoid hemorrhage is complicated by delayed cerebral ischemia and contributing factors such as hypertension. To observe the impact of hypertension and delayed cerebral ischemia on the outcome of a predominantly African-American cohort following subarachnoid hemorrhage, both retrospective (n = 42) and prospective (n = 21) studies were conducted. In the total pool (n = 63), the mean age was 49.7 years (range: 17 to 80) with a preponderance of female patients (70%). Aneurysm formation was significant in the region of the posterior communicating art
