Victor Horsley, John Marshall, nerve stretching, and the nervi nervorum.

In his early professional life, Victor Horsley was registrar and assistant to Mr. John Marshall, anatomist, surgeon, and president of the Royal College of Surgeons. In helping with the research for the Bradshaw Lecture on Nerve Stretching given by Marshall in 1883, Horsley demonstrated changes in nerve fibers due to mechanical stretching of the sciatic nerve, and he also demonstrated small nerve fibers in the sheaths of peripheral nerves--the so-called nervi nervorum. Marshall attributed the benefits of nerve stretching in sciatica to the interference with these nervi nervorum hitherto conside

Educational quality of ocular and orbital anatomy videos on YouTube: a student- and patient-centered evaluation.

BACKGROUND: The YouTube platform is increasingly used by medical students and patients as a supplementary learning tool. However, the quality and educational value of YouTube videos remain underexplored. This study aimed to evaluate the quality, reliability, and educational utility of YouTube videos on eye and orbit anatomy. METHODS: A systematic YouTube search was conducted using keywords related to eye and orbit anatomy. Out of 270 videos initially identified, only 100 met the inclusion criteria. Two anatomists and two ophthalmologists independently evaluated the videos using validated tools…

Blood supply of the internal female genital tracts with special attention paid to the uterine cervix - an overview.

Blood supply of the uterus raised interest of the scientists since early 19th century. Fundamental works of anatomists from German circle brought not only solutions but also new questions. Nowadays knowledge on the vasculature of the female internag genital organs plays leading role in many procedures, mostly endovascular, i.e. embolisation techniques used for treatment of uterine fibroids, ectopic pregnancies, bleedings etc. This paper brings a short summary of our knowledge on the vascularization of the uterus with special respect to the uterine cervix. It shows the path which led the scient…

John Hunter and the Descent of the Testis.

The descent of the testis and the development of an inguinal hernia were the earliest published scientific work by John Hunter, the Scottish surgeon and anatomist who is acknowledged as the father of scientific surgery. Hunter's anatomic descriptions are the ones we use today to describe the prenatal descent of the testis and to explain the pathogenesis of an undescended testis and inguinal hernia in infancy. His work appeared in print in 1762, not as a formal publication but as an addendum to a screed written by his older brother William publicly accusing Percival Pott of pirating John's obse

A Left-sided Azygos Vein in a Cadaver: Anatomical and Surgical Considerations.

Despite the wide-spread knowledge among anatomists and surgeons that the azygos vein lies on the right side of the vertebral column, various scientific works have been conducted which suggest the existence of left-sided azygos veins. The displacement of the vessel seems to be related with aging, due to crossover veins and the development of osteophytes on the thoracic vertebrae. The current case report confirms the variation of the azygos vein's course, highlights the awareness of the relatively unusual left-sided location of the azygos vein for the surgeon of the region, and underlines the cl…

Reconciling the clinical practice guidelines on Bell's palsy from the AAO-HNSF and the AAN.

Bell's palsy, named after the Scottish anatomist, Sir Charles Bell, is the most common acute mononeuropathy, or disorder affecting a single nerve, and is the most common diagnosis associated with facial nerve weakness/paralysis. In the past 2 years, both the American Academy of Neurology (AAN) and the American Academy of Otolaryngology-Head and Neck Surgery Foundation (AAO-HNSF) have published clinical practice guidelines aimed to improve the quality of care and outcomes for patients diagnosed with Bell's palsy. This commentary aims to address the similarities and differences in the scope and

Descriptions of cervical dystonia by Sir Charles Bell.

Sir Charles Bell is better known among neurologists for his descriptions of the clinical consequences of facial nerve lesions. However, as an accomplished physician, anatomist, and artist, he made many other contributions to the fields of neurology and neuroscience. Among those, his descriptions of patients with what we now know as cervical dystonia have not received much attention. In this report, Bell's depictions of patients presenting with a syndrome consistent with cervical dystonia will be discussed, including the identification of many of the different clinical features we currently use

Esophageal porto-caval anasthomosis. Critical view of the hypothesis of F.C. Carvalho.

The apparition and the breakage of esophageal varices et the level of cardioesophageal junction in the portal hypertension still represents a problem of pathological surgery. The presence of these varicose dilatations in this region is due to an important zone of porto-caval anastomoses. Angioarhitectonical study of lower abdominal esophagus represents a disputed problem in the specialty literature. The hypothesis of south American anatomist F. C. Carvalho (1966) looks to be the best model that could explain the appear of cardioesophageal varices. Studies realized in the past within the Anatom

What is the appropriate management of acute uncomplicated appendicitis in contemporary South Africa?

Acute appendicitis has afflicted humankind throughout our existence but has been a surgical disease for little over one hundred and fifty years. Leonardo da Vinci first drew the appendix in 1508, and several anatomists described it over the next two centuries. These included Berengarius in 1522, Andreas Vesalius in 1543 and Giovanni Battista Morgagni in 1719.1 Several pathologists began to define and elucidate the pathophysiology of acute appendicitis and a number of authors from the enlightenment onwards began to describe what was termed typhlitis until Fitz, described a large series of acute…

From Galen to Eustachio: Discovering the Anatomy of the Facial Nerve.

OBJECTIVE: To discuss the historical relevance of the original illustrations and descriptions of the facial nerve during the Italian Renaissance (ca. late 14th to early 17th centuries). MATERIAL AND METHODS: Graphic and textual information related to facial nerve discoveries were harvested from the works of the study period's anatomists, with special attention to those of Leonardo da Vinci, Andreas Vesalius, Gabrielle Falloppio, and Bartolomeo Eustachio. The importance of Galen's anatomical works as a guide to these discoveries is discussed. CONCLUSION: The reviewed texts and the illustratio…

Anatomy of the mesentery: Historical development and recent advances.

Recent appraisals of mesenteric anatomy clarify its structure and show a continuous and helical-shaped organ. This new model signifies a departure from the conventional model which described multiple, separate "mesenteries". Renaissance anatomists depicted the mesentery as a continuous structure. Events that led to replacement of a continuous with a fragmented model span several centuries. In effect, the scientific and clinical community has come full circle and back to the Renaissance model. Here we review the historical development of our understanding of the mesentery. We discuss how the fr…

A retrotracheal right subclavian artery in association with a vertebral artery and thyroidea ima.

The retro-oesophageal right subclavian artery is an anatomical abnormality encountered by anatomists and pathologists and, more recently, interventional cardiologists and thoracic surgeons with an incidence of 0.2-2% in the population. We report a case of a retrotracheal right subclavian artery which originated distally along the left aortic arch and coursed between the trachea and the oesophagus. Additionally, the aortic arch gave rise to a common trunk, which subsequently bifurcated to yield to a right vertebral artery and a left thyroidea ima, replacing the left inferior thyroid artery. Con…

Reintroducing Gotthelf Carl, the Other Huber.

Anatomist Dr. Gotthelf Carl Huber was a pioneer in peripheral nerve surgical research. His work was groundbreaking, including confirmation that axon regeneration occurs from the proximal stump as well as establishing the utility of primary and secondary nerve repair for peripheral nerve injury (PNI) at a time when the best treatment for nerve injuries was unclear and contested. He collaborated with the giants of his time and mentored the next generation of greats in PNI. Despite all of this, he is not the individual we think of when we hear the name "Huber." As such, the motivation for this re

Bilateral prehilar multiple branching of renal arteries: a case report and literature review.

Proper knowledge of variations of the arteries supplying the kidney is essential not only to the anatomists but also to the surgeons. In the present paper we are reporting a case of bilateral early and multiple branching of the renal arteries. The origin of the 2 renal arteries was normal but soon after their origin they ended by giving rise to multiple branches. Most of these branches entered the kidney through the hilum. However, on both sides, one of the branches (superior polar artery) passed superolaterally to reach the upper pole of the kidney. The superior polar artery also gave rise to…

Chiari network entanglement and herniation into the left atrium by an atrial septal defect occluder device.

The Chiari network is a fenestrated membrane consisting of threads and strands in the right atrium. First described in 1897 by anatomist Hans Chiari, it is a congenital remnant of embryonic development resulting from incomplete resorption of the right valve of the sinus venosus. Found in 2% to 3% of the population, it is generally not of clinical importance. Rarely, however, the network may be associated with serious complications such as thrombus formation, embolus entrapment, arrhythmia, tumor development, and catheter entrapment. We report the entanglement of an Amplatzer septal occluder de

The Typical Body Segment: An Anatomy Teaching Paradigm Created by Matt Cartmill and Others While Teaching Gross Anatomy at Duke University.

Within allied-health programs, gross anatomy is widely regarded as a foundational and content-dense course challenging instructors and students alike to manage an overwhelming volume of information within an increasingly constrained curriculum. In the late 1960's, the Duke University medical curriculum was redesigned to produce 'scientists-doctors' intensifying the instructional challenges of gross anatomy. In the wake of a series of unsuccessful experiments, comparative anatomists Matt Cartmill and James Shafland were recruited to develop and teach a medical gross anatomy course at Duke. Draw…
