Anesthesia Mumps With Transient Airway Compromise Following Spinal Anesthesia for Emergency Cesarean Delivery.

The authors present a 44-year-old woman who developed bilateral parotid swelling following spinal anesthesia for an emergency cesarean section. Shortly after surgery, the patient developed acute facial swelling associated with pain, trismus, and transient respiratory distress. Ultrasonographic evaluation was consistent with acute parotitis. Conservative management, including intravenous hydration, corticosteroids, analgesics, and supportive care, resulted in complete clinical recovery within 3 days. Although perioperative parotid swelling is an uncommon complication after spinal anesthesia, th

Periprocedural Polypharmacy in Lumbar Fusions Performed Under Spinal Anesthesia Compared With General Anesthesia.

BACKGROUND: Spinal anesthesia (SA) is a safe and effective alternative to general endotracheal anesthesia (GEA) for lumbar surgery. Foremost among the reasons to avoid GEA is the desire to minimize postoperative cognitive dysfunction (POCD). Although POCD is a complex and multifactorial entity, the risk of its development has been associated with anesthetic modality and perioperative polypharmacy, among others. OBJECTIVE: To determine whether SA reduced polypharmacy compared with GEA in patients undergoing transforaminal lumbar interbody fusion (TLIF). METHODS: Demographic and procedural dat

[Total spinal anesthesia. A complication of lumbar catheter peridural anesthesia for postoperative analgesia].

A case of secondary subarachnoidal dislocation of a lumbar epidural catheter is reported. A 76-year-old female underwent resection of the sigmoid colon. In order to provide postoperative analgesia, an epidural catheter was inserted between the 3rd and 4th lumbar interspaces prior to induction of anaesthesia. Aspiration tests were negative twice and a test dose of 4 ml 0.5% bupivacaine produced no signs of anaesthesia. Thereafter, the operation was performed under balanced general anaesthesia. Prior to the end of surgery a total of 14 ml bupivacaine 0.5% was administered without significant car

Spinal Anesthesia: A Safe and Effective Alternative to General Anesthesia for Infantile DDH Surgery.

BACKGROUND: Operative hip reductions for infantile DDH are traditionally performed under general anesthesia (GA). This introduces potential safety concerns, as risks related to GA are highest in children below 1 year. While surgery can be temporarily delayed to reduce these risks, this decreases the chances of a successful reduction via closed methods alone and also compromises valuable remodeling potential. This pilot study sought to evaluate the safety and efficacy of spinal anesthesia (SA) in infants undergoing operative hip reduction. METHODS: This is a multicenter, retrospective cohort s

Influencing factors of general anesthesia and spinal anesthesia for cesarean section women with PAH.

OBJECTIVE: To compare the clinical efficacy and safety of general anesthesia versus spinal anesthesia for cesarean section in parturients with pulmonary arterial hypertension (PAH), and analyze the influencing factors for postoperative adverse outcomes. METHODS: A retrospective cohort study was conducted on 236 PAH parturients who underwent cesarean section from January 2022 to December 2024. All subjects were grouped via propensity score matching into the general anesthesia group (Group A, n=104) and the spinal anesthesia group (Group B, n=132). Intraoperative hemodynamic parameters, surgica

Ultrasound-guided spinal anesthesia for cesarean section in a parturient with spinal metastases.

Preprocedural spinal ultrasound appears to decrease the failure rate and complications of neuraxial anesthesia compared to the conventional landmark technique. It is especially beneficial in difficult cases where conventional palpation technique may fail. We recently encountered a parturient with multiple lumbar and cervical spinal metastatic lesions presenting for cesarean section in the third trimester. We used spinal ultrasound to define the appropriate intervertebral space and measure the distance to the ligamentum flavum-dura mater complex. This greatly helped in administering a safe spin

The influence of spinal flexion in the lateral decubitus position on the unilaterality of spinal anesthesia.

BACKGROUND: For unilateral spinal block, local anesthetics should affect the spinal nerves of 1 side. With full flexion of the spine, the sunken cauda equina becomes tightened and is suspended in the middle of the subarachnoid space. We performed this study to assess whether spinal flexion facilitates unilateral spinal anesthesia. METHODS: Hyperbaric bupivacaine (8 mg) was administered at the L3-4 interspace through a 25-gauge Quincke needle at a rate of 0.02 mL/s. Patients were randomly allocated to group F (with full spinal flexion) or group N (the hips and back straightened). After maintai

Spinal subarachnoid hematoma following spinal anesthesia in a patient with HELLP syndrome.

A case of subarachnoid hematoma following spinal anesthesia for cesarean section in a patient with HELLP syndrome is reported. A 39-year-old woman underwent cesarean section under spinal anesthesia for worsening preeclampsia with HELLP syndrome. Despite full recovery from the spinal anesthetic, on the second postoperative day she felt numbness on the posterior aspect of her right leg, noticed she was insensitive to bladder fullness and had mild flaccid paraparesis. Magnetic resonance imaging revealed a spinal subarachnoid hematoma with cauda equina compression. With conservative management she

Control of Spinal Anesthesia-Induced Hypotension in Adults.

Spinal anesthesia-induced hypotension (SAIH) occurs frequently, particularly in the elderly and in patients undergoing caesarean section. SAIH is caused by arterial and venous vasodilatation resulting from the sympathetic block along with a paradoxical activation of cardioinhibitory receptors. Bradycardia after spinal anesthesia (SA) must always be treated as a warning sign of an important hemodynamic compromise. Fluid preloading (before initiation of the SA) with colloids such as hydroxyethyl starch (HES) effectively reduces the incidence and severity of arterial hypotension, whereas crystall

Spinal anesthesia in a patient with Schwartz-Jampel syndrome.

BACKGROUND: Schwartz-Jampel syndrome (SJS) is a very rare inherited disorder characterized by multiple skeletal deformities, limited joint mobility, micrognathia, blepharophimosis, myotonia, and growth retardation. SJS is caused by mutations in the gene encoding perlecan (heparan sulfate proteoglycan). Anesthetic management of these patients is challenging. The use of spinal anesthesia in these patients has not been reported. CASE PRESENTATION: A 14-year-old boy was scheduled for inguinal hernia and hydrocele repair. The diagnosis of SJS was based on his dysmorphic features, electromyographic

Obstetrical epidural and spinal anesthesia in multiple sclerosis.

To examine obstetrical epidural and spinal anesthesia use in women with multiple sclerosis (MS) and the relationship with MS clinical factors. This was a retrospective cohort study, linking clinical data from women with MS in the British Columbia (BC) MS database to obstetrical data (1998-2009) from the BC Perinatal Database Registry. We compared epidural use in 431 deliveries to women with MS and 2,959 deliveries from the general population, as well as spinal use in cesarean deliveries (128 to women with MS and 846 in the general population), considering parity and using multivariate models.

Promazine and chlorpromazine for prolonged spinal anesthesia in rats.

Though promazine and chlorpromazine elicited cutaneous anesthesia, no study of spinal anesthesia with chlorpromazine and promazine has been reported. This study was to examine whether chlorpromazine and promazine produce spinal anesthesia. Using a rat model via intrathecal injection, we tested spinal blockades of motor function and nociception by promazine, chlorpromazine or bupivacaine, and so were dose-response studies and durations. We demonstrated that chlorpromazine and promazine elicited dose-dependent spinal blockades in motor function and nociception. On the 50% effective dose (ED(50))
