BACKGROUND: Stability of the spine and intervertebral disc (IVD) integrity is enabled by the highly organized fibrocartilaginous annulus fibrosus (AF). The shear properties of the AF are important in maintaining IVD integrity. AF shear mechanics in degenerative disc (DD) remain underexplored, especially in comparing minimally degenerative (non-DD) and symptomatic DD individuals. This study measured tissue mechanical properties (AF simple shear modulus and dynamic shear properties) and examined structure (with optical coherence tomography (OCT)) in surgical DD and non-DD control individuals.

Chronic painful intervertebral disc (IVD) degeneration (i.e., discogenic pain) is a major source of global disability needing improved knowledge on multiple-tissue interactions and how they progress in order improve treatment strategies. This study used an in vivo rat annulus fibrosus (AF) injury-driven discogenic pain model to investigate the acute and chronic changes in IVD degeneration and spinal inflammation, as well as sensitization, inflammation, and remodeling in dorsal root ganglion (DRG) and spinal cord (SC) dorsal horn. AF injury induced moderate IVD degeneration with acute and broad

Mesenchymal stem/stromal cell (MSC)-based therapies for low back pain and intervertebral disc (IVD) degeneration have been emerging, despite the poor knowledge of their full mechanism of action. As failure of the annulus fibrosus (AF) is often associated with IVD herniation and inflammation, the objective of the present study was to investigate the impact of the MSC secretome on human AF cells exposed to mechanical loading and a pro-inflammatory environment. Human AF cells isolated from IVD biopsies from patients with adolescent idiopathic scoliosis (AIS) or disc degeneration (DD) were exposed

BACKGROUND: Previous studies on dynamic impingement of nerve root in cervical spondylotic radiculopathy (CSR) have focused on effect of cervical spine motion (CSM) on dimensional changes of intervertebral foramen. However, there are few studies to investigate effect of CSM on displacement of posterolateral intervertebral disc until now. The present study aimed to investigate effect of CSM on displacement of posterolateral annulus fibrosus (AF) in CSR with contained posterolateral disc herniation.
METHODS: A C5-C6 CSR finite element model with unilateral contained posterolateral disc herniatio

Intervertebral disc degeneration is a common condition that may lead to low back pain and radiculopathy. Understanding the pathophysiology and cellular and molecular events of degenerative disc disease has resulted in the proposal of a gene therapy approach to halt and reverse disc degeneration. We explored the feasibility of reversing intervertebral disc degeneration using human vascular endothelial growth factor165 (hVEGF165) and transforming growth factor-β1 (TGF-β1) gene therapy. hVEGF165 complementary DNA was obtained from pcDNA3(+)-hVEGF165 and cloned into adeno-associated virus (AAV)-pS

Understanding the biomechanical behavior of the intervertebral disc is crucial for studying disease mechanisms and developing tissue engineering strategies for managing disc degeneration. We used synchrotron small-angle X-ray scattering to investigate how changes to collagen behavior contribute to alterations in the disc's ability to resist compression. Coccygeal motion segments from 6-month-old lean Sprague-Dawley rats ( n=7) and diabetic obese University of California Davis type 2 diabetes mellitus (UCD-T2DM) rats ( n=6, diabetic for 68±7 days) were compressed during simultaneous synchrotron

The present study aimed to investigate the expression and roles of interleukin (IL)-17 and tumor necrosis factor (TNF)-α in intervertebral disc degeneration (IDD) and to identify the association between the effects of IL-17 and TNF-α in IDD. This may increase understanding of the pathogenic mechanism underlying IDD, and aid the development of alternative therapies. The experimental group consisted of 40 samples of nucleus pulposus tissue obtained from the intervertebral discs (IVDs) of patients with IDD by surgical intervention, and was further divided into an annulus fibrosus disrupted group,

1. The maintenance of the nucleus pulposus matrix in the adult human disc is dependent on the functional integrity of the cartilage end plate cells. 2. Cartilage end plate senescence is followed by compensatory cartilaginous metaplasia of annulus fibrosus cells. 3. It is proposed that disc narrowing and collapse are related to metabolic failure of matrix production by end plate and annulus fibrosus cells. 4. Calcium pyrophosphate dihydrate crystal deposits, a visible manifestation of a metabolic abnormality, are found frequently in the annulus fibrosus and cartilage end plates of elderly patie

PURPOSE: Intervertebral disc degeneration (IDD) is a common cause of lower back pain. However, the role of annulus fibrosus rupture in altering the immune microenvironment of nucleus pulposus remains unclear. This study elucidated the impact of annulus fibrosus rupture on the immune response within the nucleus pulposus in IDD. METHODS: The study included patients diagnosed with intervertebral disc herniation who underwent surgical treatment at the Orthopedics Department of the Second Hospital of Soochow University from September 2020 to July 2021. Based on the integrity of the annulus fibrosus

PurposeTo investigate the biomechanical changes in the proximal adjacent segment with different grades of degeneration after posterior lumbar interbody fusion (PLIF).MethodsWe created three finite element models of the L3-5 with different grades of degeneration (healthy, mild, and moderate) at the L3-4 that were developed by changing the disc height and material properties of the nucleus pulposus. The L4-5 were operated by interbody fusion cages and pedicle screws. All models were loaded with a compressive pre-load of 400 N and a bending moment of 10 N a in three planes to recreate flexion, ex

The risk of accelerating the degeneration of adjacent disc levels after lumbar spinal fusion is a controversial issue. A finite element model consisting of L1 to L5 lumbar spines was used to assess the effect on adjacent disc level after lumbar spinal fusion. We compared intact, L4/5 posterior interbody fusion (PLF), and L4/5 posterior lumbar interbody fusion (PLIF) models. The loading conditions applied were compressive force, compressive force plus flexion moment, and compressive force plus extension moment. Evaluations were made for von Mises stress on each vertebral end-plate, Tresca stres

Pathological mineralization of intervertebral disc is debilitating and painful and linked to disc degeneration in a subset of human patients. An adenosine triphosphate efflux transporter, progressive ankylosis (ANK) is a regulator of extracellular inorganic pyrophosphate levels and plays an important role in tissue mineralization. However, the function of ANK in intervertebral disc has not been fully explored. Herein we analyzed the spinal phenotype of Ank mutant mice (ank/ank) with attenuated ANK function. Micro-computed tomography and histological analysis showed that loss of ANK function re

Atrioventricular (AV) accessory pathways (APs) provide additional electrical connections between the atria and ventricles, resulting in severe electrical disturbances. It is generally accepted that APs originate in the altered annulus fibrosus maturation in the late prenatal and perinatal period. However, current experimental methods cannot address their development in specific locations around the annulus fibrosus because of the inaccessibility of late fetal hearts for electrophysiological investigation under physiological conditions. In this study, we describe an approach for optical mapping

Intervertebral disc degeneration is the main cause of low back pain. In the past 20 years, the injection of mesenchymal stromal cells (MSCs) into the nucleus pulposus of the degenerative disc has become the main approach for the treatment of low back pain. Despite the progress made in this field, there are still many barriers to overcome. First, intervertebral disc is a highly complex load-bearing composite tissue composed of annulus fibrosus, nucleus pulposus and cartilaginous endplates. Any structural damage will change its overall biomechanical function, thereby causing progressive degenera

Specially developed intervertebral and intradiscal pressure transducers are used to map the pressures in the nucleus pulposus and within the annulus fibrosus of the human spinal discs, to the point of fracture of the segmental units. The results show that for normal discs, pressures in the nucleus and in the annulus are linearly related to each other and to the applied compressive loads. Mechanical strains which develop around the periphery of the vertebral body are also linear with the applied compressive loads, when the partial vertebral column is loaded to the point of bony fracture. Over the past decade, single-cell RNA sequencing (scRNA-seq) has revolutionized research on biological mechanisms of diseases. Moreover, this technique has been utilized to identify and characterize unique cell types and subpopulations, thereby illuminating cellular heterogeneity. The true value of scRNA-seq lies in its ability to detect transcriptional alterations or perturbed pathways within specific cell types under pathological conditions. In the context of intervertebral disc degeneration (IVDD), the pathophysiological foundation is largely rooted in inflammation. The primary target cells

To obtain T2* values in histologically evaluated healthy ovine intervertebral discs of the cervical and lumbar spine. Intervertebral discs of nine sheep and nine lambs underwent histological assessment with the modified Boos score for grading of disc degeneration. Discs with a score <10 points (maximum = 40 points) underwent T2* mapping (n = 64). Mid-sagittal T2* values were obtained in five regions: Anterior annulus fibrosus, anterior nucleus pulposus, central nucleus pulposus, posterior nucleus pulposus, and posterior annulus fibrosus. We noted a zonal T2* distribution with high values in th

BACKGROUND AND PURPOSE: Because it diminishes the stiffness of the intervertebral disk, disk degeneration results in abnormal motions of the spine. Therefore, disk degeneration associated with back pain may indicate spinal fusion. The purpose of this study was to correlate the MR appearance and stiffness of lumbar intervertebral disks.
METHODS: Eighty-two lumbar spinal segments were imaged with MR. The intervertebral disks were classified as: 1) normal, 2) having transverse or concentric tears of the annulus fibrosus if MR imaging showed only these changes, 3) having radial tears of the annul

Conventional microdiscectomy treatment for intervertebral disc herniation alleviates pain but does not repair the annulus fibrosus, resulting in a high incidence of recurrent herniation and persistent dysfunction. The lack of repair and the acute inflammation that arise after injury can further compromise the disc and result in disc-wide degeneration in the long term. To address this clinical need, we developed tension-activated repair patches (TARPs) for annulus fibrosus repair and local delivery of the anti-inflammatory factor anakinra (a recombinant interleukin-1 receptor antagonist). TARPs
