Anterior callosal haemorrhage. A partial interhemispheric disconnection syndrome.

The interhemispheric disconnection syndrome secondary to a callosal haemorrhage is exceedingly uncommon. In the present study, 3 patients with haemorrhages restricted to the corpus callosum are presented. All 3 developed a partial anterior interhemispheric disconnection syndrome: unilateral tactile anomia, unilateral agraphia, unilateral apraxia, difficulty in copying drawings, dyscalculia as well as abnormalities of somaesthetic transfer and the 'alien hand' sign. The study of these cases allowed a close examination of the association between deficits in the transfer of specific neuropsycholo…

Homotopic organization of essential language sites in right and bilateral cerebral hemispheric dominance.

OBJECT: Language dominance in the right hemisphere is rare. Therefore, the organization of essential language sites in the dominant right he … es were identified as essential for language function (14 sites for speech arrest/anarthria; 12 for anomia; and 1 for alexia). While significant interindividual variability was observed, the general pattern of language organization was similar to classic descriptions of frontal language production and posterior temporal language integration for the left …

[Language and semantic memory impairment in a patient with motor neuron disease and semantic dementia: a case report].

We report the rare case of a 59-year-old man with motor neuron disease and semantic dementia (SD-MND); SD-MND was in a very early stage, and its clinical progression, especially with regard to language impairment, and abnormalities on neuroimages were evaluated for 3 years. The patient complained only of difficulties in recalling names of acquaintances and in writing kanji characters. After 1 year, he experienced difficulty in describing common objects. He developed two-way anomia only in some words, which varied from day to day. His anomia was not category-specific and was noted even with res…

Bilingual awake craniotomy with English and Polish language mapping in a 15-year-old patient provides evidence for the role of the left superior temporal gyrus in language switching.

The utility of intraoperative mapping in multilingual patients with brain tumours in speech-eloquent locations is evidenced by reports of he … gyrus using a novel test paradigm. Stimulation of the posterior superior temporal gyrus resulted in anomia when switching from Polish to English, in the absence of any stimulation effect on switching from English to Polish or object naming in each individual language.

Combined semantic dementia and apraxia in a patient with frontotemporal lobar degeneration.

In this study we report neuropsychological and brain-imaging findings in a patient with frontotemporal lobar degeneration. Brain imaging using registration of (18)fluorodeoxyglucose (FDG)-PET data to three-dimensional (3-D) magnetic resonance imaging showed atrophy and highly significant hypometabolism of the left temporal lobe and both frontal lobes. Volumetric measurements of the hippocampi/amygdala showed a reduction in volume of 25% on the left compared to right within cortical areas. Neuropsychological testing revealed semantic dementia with severe anomia as well as apraxia with impairmen…

Recent U.S. trends in the age decrement in health satisfaction: preliminary findings.

One of the most salient components of older patients' mental disposition involves satisfaction with health. Low health satisfaction has been observed to be associated with anomia, unhappiness, high body concern, poor adjustment to the environment, low activity, and depression. Yet a number of issues in the area remain under-investigated. One such issue involves change over time in the elderly populace's general level of contentment with health. The present study utilizes data from the National Opinion Research Center at the University of Chicago to investigate this subject. The preliminary fin…

Finger agnosia in Alzheimer disease.

BACKGROUND AND OBJECTIVE: The purpose of this study was to learn if a deficit of finger naming (finger agnosia or anomia) is a sensitive test for Alzheimer disease (AD) and the best means of testing for finger agnosia. METHODS: The subjects were 38 patients with AD and 10 matched normal controls. All subjects were asked to name the thumb, index, and pinky fingers. RESULTS: No control subject had trouble naming any of these fingers, but 37% of the AD subjects did. When AD patients had difficulty with finger naming, they always had trouble naming the index finger. CONCLUSIONS: In the absence …

Semantic dementia and fluent primary progressive aphasia: two sides of the same coin?

Considerable controversy exists regarding the relationship between semantic dementia (SD) and progressive aphasia. SD patients present with anomia and impaired word comprehension. The widely used consensus criteria also include the need for patients to exhibit associative agnosia and/or prosopagnosia: many authors have used the label SD for patients with non-verbal, as well as verbal, semantic deficits on formal testing even if they recognize the objects and people encountered in everyday life; others interpret the criterion of agnosia to require pervasive recognition impairments affecting dai…

Mixed tau and TDP-43 pathology in a patient with unclassifiable primary progressive aphasia.

Classifying primary progressive aphasia (PPA) into variants that may predict the underlying pathology is important. However, some PPA patients cannot be classified. A 78-year-old woman had unclassifiable PPA characterized by anomia, dysarthria, and apraxia of speech without agrammatism. Magnetic resonance imaging revealed left mesial temporal atrophy and 18-flourodeoxy-glucose positron emission tomography showed left anterior temporal and posterior frontal (premotor) hypometabolism. Autopsy revealed a mixed tauopathy (argyrophilic grain disease) and transactive response-DNA-binding-protein-43 …

Predicability (ease of predication) as semantic substrate of imageability in reading and retrieval.

It was proposed by Jones (1985) that the apparent influence of a word's imageability upon the probability of the word being read correctly by a deep dyslexic person could be understood in terms of an underlying semantic variable, ease of predication (also termed predicability). In a recent critique, de Mornay Davies and Funnell (2000) claim to have identified a number of problems with the ease of predication proposal. It is shown here, however, that it is the critique itself which is fundamentally flawed. In contrast, the predicability approach continues to identify correctly the semantic subs

Speech-language pathology and dysphagia in multiple sclerosis.

Dysarthria occurs in approximately 40% of all patients with MS. When speech and voice disturbances do occur, they usually present as a spastic-ataxic dysarthria with disorders of voice intensity, voice quality, articulation, and intonation. While language disturbances such as aphasia, auditory agnosia, anomia, dysgraphia, and dyslexia are very rare in MS, cognitive deficits and swallowing disorders are common. Treating dysarthria, dysphagia, and cognitive deficits in MS patients is effective for reestablishing functional daily activities. The types, severity, and rates of deterioration in MS a…

Associative visual agnosia resulting from a disconnection between intact visual memory and semantic systems.

We report the case of a patient (RC) who developed a severe visual agnosia, associated to alexia without agraphia, color anomia and amnesia, following an ischemic stroke in the territory supplied by the left posterior cerebral artery. Based on his proficient performance on tests evaluating analysis of elementary visual features, formation of viewer-centered and object-centered representations of visual stimuli and discrimination between drawings representing real and unreal objects, we concluded that the critical locus of deficit was a disconnection between the normally functioning visual memo…

Creutzfeldt-Jakob disease revealed by a logopenic variant of primary progressive aphasia.

Logopenic aphasia, mainly characterized by word anomia, sentence and phrase comprehension difficulties secondary to phonological loop deficits but relatively preserved single word comprehension and no agrammatism, is one of the 3 main variants of primary progressive aphasia (PPA). We describe the first case of PPA that fulfilled clinical criteria of logopenic aphasia but showed abnormal DWI hyperintensities that were predominant on the left hemisphere and compatible with Creutzfeldt-Jakob disease (CJD). After abnormally long isolated language deficits, the patient rapidly worsened and died. Au…

Intellectual loss in Alzheimer's dementia and WAIS-R intrasubtest scatter.

Patterns of intrasubtest scatter in the WAIS-R protocols of patients (n = 32) with Alzheimer's disease were compared to those of normal elderly controls (n = 32). The Alzheimer's patients showed more randomly dispersed item failures on some subtests, but normal controls showed more intrasubtest variability on other measures. Rates of correct diagnostic classification based on scatter measures were only slightly better than chance despite the presence of prominent anomia, memory impairment, construction apraxia, and significant decline from premorbid intellectual level in demented patients. In …

[Interhemispheric disconnection syndrome with diagonistic dyspraxia in the course of Marchiafava-Bignami disease].

The probable diagnosis of Marchiafava-Bignami disease was made in a chronic alcoholic patient because of the rapid onset of signs of inter-hemisphere disconnection. These inclused left visual and tactile anomia, difficulty of a limb to imitate postures presented in the contralateral visual half-field, constructional apraxia of the right hand, agraphia of the left hand, left auditive extinction in the dichotic test, difficulty in responses of the left limbs to verbal orders, which the authors propose to name left "aphasic apraxia" and a bimanual asynergia, sometimes presenting an aspect of diag…

[18F]THK-5351 PET imaging in early-stage semantic variant primary progressive aphasia: a report of two cases and a literature review.

BACKGROUND: Semantic variant primary progressive aphasia (svPPA) is a subtype of primary progressive aphasia characterized by two-way anomia and disturbance in word comprehension, with focal atrophy in the left temporal lobe. [18F]THK-5351 was originally developed to trace tau protein. However, it has recently been suggested that [18F]THK-5351 binds to monoamine oxidase B in astrocytes, which reflects gliosis. Herein, the authors present two cases involving patients with early-stage svPPA who underwent [18F]THK-5351 positron emission tomography (PET) imaging, and examined whether [18F]THK-5351…
