Clinical Microbiome Testing for Urology.

The urine culture is imperfect, and a series of alternative approaches are in development to assist in diagnosis, treatment, and prevention of urinary tract infection (UTI). Culture-independent approaches typically do not distinguish between viable and nonviable bacteria, and are generally not included in current clinical guidance. Next-generation sequencing may play an important future role in precise targeting of antibiotic treatment of asymptomatic bacteriuria prior to endourologic surgery or in pregnancy. Future studies are needed to determine whether microbiota modulation could prevent UT

A critical assessment of antimicrobial treatment in uncomplicated Salmonella enteritis.

The human and veterinary literature on the effect of antimicrobials on the clinical and bacteriological cure in uncomplicated Salmonella gastroenteritis is reviewed. Comparison of data on the efficacy of conventional antimicrobials (chloramphenicol, neomycin, ampicillin, amoxycillin, tetracycline, trimethoprim/sulfonamide combinations) and the newer fluoroquinolones indicate that quinolones may shorten the course of clinical disease in contrast to the conventional antimicrobials. Postconvalescent excretion of Salmonella was not affected by the conventional antimicrobials whereas the data on th

Exploring urinary microbiome: insights into neurogenic bladder and improving management of urinary tract infections.

The traditional view of sterile urine has been challenged by advancements in next-generation sequencing, revealing that the urinary microbiome significantly influences individual health and various urinary system diseases. Urinary tract infections in patients with neurogenic bladder are highly prevalent, recurrent, and lifelong. If frequent urinary tract infections are not adequately managed, they may ultimately lead to chronic renal failure. The excessive use of antibiotics to prevent and treat urinary tract infections may lead to increased bacterial resistance, limiting future therapeutic op

Can We Prevent Recurrent UTIs Without Antibiotics, in Both Those Who Do and Do Not Use Catheters? ICI-RS 2024.

AIMS: Urinary tract infections are one of the most common reasons for antibiotic prescription. The widespread use of antibiotic treatments contributes to the global health problem of antimicrobial resistance development. To slow down the progression of antimicrobial resistance, it is essential that we explore nonantibiotic preventive treatments for this common condition. We aim to report discussions regarding nonantibiotic preventive strategies for recurrent urinary tract infections in both catheterized and non-catheterized patients that took place at the International Consultation on Incontin

Reduction of microbial adhesion on polyurethane by a sub-nanometer covalently-attached surface modifier.

Indwelling urinary catheters are a common medical device used to relieve urinary retention. Many patients who undergo urinary catheterization develop urinary tract infections (UTIs), which can lead to severe medical complications and high cost of subsequent treatment. Recent years have seen a number of attempts at reducing the rate of UTIs in catheterized patients via catheter surface modifications. In this work, a low cost, robust anti-thrombogenic, and sterilizable anti-fouling layer based on a covalently-bound monoethylene glycol hydroxide (MEG-OH) was attached to polyurethane, a polymeric

Review of the literature and individual patients' data meta-analysis on efficacy and tolerance of nitroxoline in the treatment of uncomplicated urinary tract infections.

BACKGROUND: Nitroxoline, a hydroxychinoline derivate, has been used for many years to treat urinary tract infections (UTI). Many uncontrolled, but only few controlled clinical studies have been published. Four so far unpublished, controlled clinical studies were meta-analysed. METHODS: A narrative literature review was performed. In addition the individual patient data (IPD) of 466 females with uncomplicated UTI of four prospective, single blind, randomized, clinical studies with similar protocols using nitroxoline (250 mg tid) versus cotrimoxazole (960 mg bid) or norfloxacin (400 mg bid) as

[Fosfomycin for treatment of uncomplicated lower urinary tract infection].

In this article the current data on using of antibacterial drug Fosfomycin for treatment of lower urinary tract infection are reviewed. The distinct features of pharmacodynamics and pharmacokinetics of fosfomycin, as well as the spectrum of its antimicrobial activity are thoroughly discussed. The current information on the resistance of uropathogens, including those that produce extended-spectrum -lactamase, are presented.

Fighting urinary tract infections with antibiotic and non-antibiotic therapies.

Urinary tract infections (UTIs) place a considerable burden on the patient and are associated with substantial economic cost. Treatment of UTIs is mainly achieved using antibiotics, however, the rise in antibiotic resistance is concerning and the use of non-antimicrobial prophylaxis offers alternative treatment methods.

Intestinal Behcet's syndrome with an unusual complication bladder-intestines fistula and urinary tract infections-A case report.

A 33-year-old male patient with a 17-year Behcet's syndrome history showed abdominal pain and fever symptoms. The abdominal CT was suggestive of an acute ileocecal intestinal perforation. In addition, the symptoms disappeared after the conservative treatment. Some related examinations, including capsule endoscopy, were performed to explain the phenomenon of the food residue urine. These results indicated the intestine-urinary tract fistula formation, supposed to be the outcome of intestinal Behcet's syndrome perforation. This is a rare case of intestinal Behcet's syndrome with abdominal sympto

[Efficacy of Uronext in chronic recurrent bacterial-viral cystitis with multiple resistance to antibacterial drugs].

Chronic recurrent cystitis (CRC) is one of the most common diseases of the lower urinary tract in the female population. Difficulties in diagnosis and treatment, despite the observance of modern clinical recommendations for CRC, are due to the versatility and polyetiology of the pathogenesis of the disease. The most difficult and discussed section of this problem is the diagnosis and treatment of CRC of papillomavirus (PV) etiology. The article presents its own clinical observation - Diagnosis and treatment of a patient with CRC PV etiology. CRC, in addition to bacterial uropathogens, can be c

Bioevaluation of the antibacterial flumequine for urinary tract use.

The antimicrobial activity of flumequine (R-802) was characterized by in vitro and in vivo procedures. Assay of the minimal inhibitory concentrations for 321 recent clinical isolates revealed that 88% of the gram-negative bacteria were inhibited by an R-802 concentration of 6.2 mug/ml or less. Cross-resistance in laboratory-derived mutants of Proteus vulgaris was essentially complete for R-802, nalidixic acid, and oxolinic acid, although quantitative differences were evident. R-802 was more effective than either of these quinolone antibacterials in preventing the development of experimental mu

Drug repurposing as an approach to control biofilm formation and encrustation of urinary catheters: preclinical evidence and future challenges.

Indwelling urethral catheters are the most widely used medical devices across the world, and catheter-associated urinary tract infections (CAUTIs) are the most common type of healthcare acquired infection. For many patients, urinary catheter blockage is a common and recurring problem, which can have considerable negative impact on patient health and well-being. Blockage primarily stems from the formation of crystalline bacterial biofilms on catheter surfaces, which can lead to upper urinary tract infection (UTI) and the onset of serious clinical complications. Potential solutions to this impor

Pharmacological principles in the treatment of urinary tract infections.

The ideal antibacterial drug for urinary tract infections should eliminate most urinary pathogens but not alter the colonic flora. Levels in the urine can be attained with the beta-lactam and aminoglycoside antibiotics which are up to several powers of 10 higher than the minimal inhibitory concentration values of the causative organisms. At these levels even penicillin G eliminates many Gram-negative urinary pathogens. Trimethoprim has a lesser propensity to select resistant organisms than most other antimicrobials. It occurs in vaginal secretions and reduces the number of Enterobacteriaceae s
