BACKGROUND: Bacterial infections occurring during labour, childbirth, and the puerperium may be associated with considerable maternal and perinatal morbidity and mortality. Antibiotic prophylaxis might reduce wound infection incidence after an episiotomy, particularly in situations associated with a higher risk of postpartum perineal infection, such as midline episiotomy, extension of the incision, or in settings where the baseline risk of infection after vaginal birth is high. However, available evidence is unclear concerning the role of prophylactic antibiotics in preventing infections after…

Primary vesicoureteral reflux (VUR) is a dynamic disease with several factors influencing its course. Therapeutic options range from watchful waiting to open surgery. Multiple risk factors for the development of pyelonephritis are identified, which then determine the decision for the appropriate treatment option. They include age, gender, history of pyelonephritis and renal scarring, bladder and bowel dysfunction (BBD), circumcision status and parental preference. The use of continuous antibiotic prophylaxis (CAP) is becoming increasingly controversial with antibiotic resistance being a major …

INTRODUCTION: The rate of surgical site infection is a good indicator of the quality of care in surgical departments. Antibiotic prophylaxis represents a measure of proven efficiency in preventing the infection in a surgical site, and its fulfilment is a main goal in management contracts of health services. The efficiency of this preventive measure requires the existence of updated protocols and performance evaluation.
OBJECTIVE: To evaluate the compliance of this antibiotic protocol in our Urology Department with the global analysis of the causes of inadequate prophylaxis and by procedures.

The practice of giving prophylactic antibiotics to patients at the time of urinary catheter insertion, change or removal is variable since g … iotics. This article describes an audit conducted in one trust to establish the current practice of antibiotic prophylaxis for urinary catheter procedures. The audit confirmed that in 60% of the recorded catheter procedures, patients were given antibiotics, usually gentamicin. Variations in gentamicin prophylaxis were revealed, including differences in the timing of adm …

A randomized prospective study of 30 patients was undertaken to evaluate antibiotic prophylaxis for orthognathic surgery. Perioperative, high-dosage antibiotic administration was compared to an existing prophylactic regimen employing longer intervals and continued postoperative antibiotics for 7 to 10 days. No significant difference was found between the two groups with respect to incidence of infection. Other variables studied included gender and age of patients, type of surgery performed, procedure length, and use of fixation.

A single intravenous dose of tobramycin and lincomycin, given at the start of gastrointestinal operations, significantly reduced the incidence of postoperative wound infection from 34% to 5%. The occurrence of both anaerobic and aerobic bacteria was reduced. Therapeutic concentrations of the antibiotics were maintained throughout the operative period in most cases. No toxic effects of the antibiotics were detected, no anaesthetic complication occurred, and resistant strains of bacteria normally sensitive to the antibiotics were not isolated from wounds.

Four patients with malignant blood disease and neutropenia developed apparently localized infective lesions of the skin or conjunctiva from which Pseudomonas aeruginosa was cultured. In spite of appropriate local antimicrobial treatment all patients developed pseudomonas septicaemia within a short period and three died. It is argued that the isolation of pseudomonads from apparently localized lesions in patients with compromised antibacterial defences is an indication for systemic antibiotic therapy.

BACKGROUND: Surgical site infection is one of the most common complications of septoplasty. The use of antibiotic prophylaxis has been debated for years. Since the practice at our centre has shifted towards a more frequent use of prophylactic antibiotics in septoplasties, our aim was to review its use and effect on the postoperative infection rate.
METHODS: We retrospectively reviewed the medical records of all adult patients who had undergone septoplasty at the Helsinki University Hospital (Helsinki, Finland) during 2019-2023. Patient and surgery characteristics, use of surgical antibiotic p…

Perioperative antibiotic prophylaxis (PAP) is a key component of infection prevention in trauma surgery and orthopedics. The new German S3 guidelines recommend a single-shot PAP dose for most procedures, particularly joint arthroplasty, osteosynthesis of closed fractures and other elective operations. Open fractures require pre-emptive antibiotic treatment for a maximum of 24 h in type I and type II fractures and for a maximum of 72 h in type III open fractures. Patients with an open distal phalanx fracture of the finger should not receive PAP. In knee arthroscopy PAP is recommended when forei…

BACKGROUND: Rheumatic heart disease affects more than 40.5 million people worldwide and results in 306,000 deaths annually. Echocardiographic screening detects rheumatic heart disease at an early, latent stage. Whether secondary antibiotic prophylaxis is effective in preventing progression of latent rheumatic heart disease is unknown.
METHODS: We conducted a randomized, controlled trial of secondary antibiotic prophylaxis in Ugandan children and adolescents 5 to 17 years of age with latent rheumatic heart disease. Participants were randomly assigned to receive either injections of penicillin …
