Conversational discourse analysis as a method for evaluating progress in aphasia: a case report.

The gains made by a woman with Broca's aphasia as documented by traditional measures were paralleled by changes in conversation, including increased verbal output and efficiency, and changes in conversation repair patterns. Her conversational partner decreased her verbal output, as predicted. The progress documented with conversational discourse analysis was not observable from other test measures. Clinical and theoretical implications are discussed.

Language as a Predictor of Motor Recovery: The Case for a More Global Approach to Stroke Rehabilitation.

Stroke is the third leading cause of death in the developed world and the primary cause of adult disability. The most common site of stroke is the middle cerebral artery (MCA), an artery that supplies a range of areas involved in both language and motor function. As a consequence, many stroke patients experience a combination of language and motor deficits. Indeed, those suffering from Broca's aphasia have an 80% chance of also suffering hemiplegia. Despite the prevalence of multifaceted disability in patients, the current trend in both clinical trials and clinical practice is toward compartme

Fundamental or forgotten? Is Pierre Paul Broca still relevant in modern neuroscience?

The ability to speak is a unique human capacity, but where is it located in our brains? This question is closely connected to the pioneering work of Pierre Paul Broca in the 1860s. Based on post-mortem observations of aphasic patients' brains, Broca located language production in the 3rd convolution of the left frontal lobe and thus reinitiated the localizationist view of brain functions. However, contemporary neuroscience has partially rejected this view in favor of a network-based perspective. This leads to the question, whether Broca's findings are still relevant today. In this mini-review,

Effect of speech therapy intervention program for non-fluent aphasic patients after stroke.

PURPOSE: the objective of this paper is to verify the effect of speech therapy intervention program in patients with non-fluent aphasia due to stroke in language tasks related to verbal fluency in semantic and phonological categories. METHODS: Patients with aphasia due to stroke were selected to take part in this study. Two groups were formed: diagnosed patients with Broca/transcortical motor aphasia (GA), and a control group (healthy individuals). GA took a fluency verbal task (FAS, other complementary categories: phonological /p/ /l/ and semantic: "fruits" and "names"). These patients were

Association of ictal aphasia with hypoperfusion in language areas in temporal lobe epilepsy patients.

Ictal clinical phenomenology, including aphasia, is usually associated with increased regional cerebral perfusion. We present an unusual pattern of ictal cerebral perfusion in three patients with pharmacoresistant, lesional temporal lobe epilepsy and ictal/postictal aphasia studied with prolonged video-EEG, ictal, and interictal SPECT and MRI for pre-surgical evaluation. Subtraction of ictal-interictal SPECT images co-registered with MRI (SISCOM) showed ictal hyperperfusion in the temporal epileptogenic area in all patients. In addition, hypoperfusion of Broca's area in one case, Wernicke's ar

The neuroanatomy of pure apraxia of speech in stroke.

The left insula or Broca's area have been proposed as the neuroanatomical correlate for apraxia of speech (AOS) based on studies of patients with both AOS and aphasia due to stroke. Studies of neurodegenerative AOS suggest the premotor area and the supplementary motor areas as the anatomical correlates. The study objective was to determine the common infarction area in patients with pure AOS due to stroke. Patients with AOS and no or equivocal aphasia due to ischemic stroke were identified through a pre-existing database. Seven subjects were identified. Five had pure AOS, and two had equivocal

Does Right-Sided Language Lateralization on BOLD-fMRI Affect Postoperative Language Outcome for AVM Patients?

AIM: For cerebral arteriovenous malformations (AVMs) involving language areas, right-sided language lateralization on functional magnetic resonance imaging (fMRI) has been reported, which is regarded as language cortex reorganization. The authors attempted to study if this right-sided language lateralization affects postoperative language outcome. MATERIAL AND METHODS: Clinical and imaging data of 43 right-handed AVM patients who underwent preoperative fMRI were retrospectively reviewed. All lesions involved the language cortex, with the Broca area involved in 13 patients and the Wernicke are

[Possible effects of drawing on the language and re-education of an aphasic patient (author's transl)].

Various studies have demonstrated that the ability to draw may be unaffected in motor aphasia, and even in some cases of sensory aphasia when there is no associated constructive apraxia. An artist, who was both a satirist and a caricaturist, was followed up for three years during re-educational therapy following the onset of an overall aphasia, and this raises the problem of the possible effects of drawing on recuperation of language, and the role of the right hemisphere in this recuperation. The question arises as to whether drawing should be restricted to certain particular cases during re-e

Damage to Broca's area does not contribute to long-term speech production outcome after stroke.

Broca's area in the posterior half of the left inferior frontal gyrus has long been thought to be critical for speech production. The current view is that long-term speech production outcome in patients with Broca's area damage is best explained by the combination of damage to Broca's area and neighbouring regions including the underlying white matter, which was also damaged in Paul Broca's two historic cases. Here, we dissociate the effect of damage to Broca's area from the effect of damage to surrounding areas by studying long-term speech production outcome in 134 stroke survivors with relat

Diffusion tensor imaging magnetic resonance imaging (DTI-MRI) helps to tailor speech therapy: A case report with a short narrative review.

BACKGROUND: Stroke has functional sequelae, including motor weakness, spasticity, dysphagia, and neurogenic bladder deteriorating activities of daily living. Speech therapy is more often an essential part of the rehabilitation program. Studies in aphasia have primarily focused on two major pathways: the arcuate fasciculus and the superior longitudinal fasciculus. The arcuate fasciculus is a major fiber bundle connecting Broca's area (associated with language production) and Wernicke's area (associated with language comprehension). Damage to this pathway can result in different types of aphasia

Study of two cases of aphasia by infarction of the left thalamus, without cortical lesion.

Two patients suffering from a left thalamus infarct were studied over a long period. The precise location of the lesion has been demonstrated by the C.T. scan. In spite of the absence of any cortical lesion, both presented with aphasia and other neuropsychological disorders (left-right confusion, dyscalculia and possible finger agnosia). Their speech disorders although different, had several features in common. The observations were compared with the other cases reported in the literature.

Anatomical evidence of an indirect pathway for word repetition.

OBJECTIVE: To combine MRI-based cortical morphometry and diffusion white matter tractography to describe the anatomical correlates of repetition deficits in patients with primary progressive aphasia (PPA). METHODS: The traditional anatomical model of language identifies a network for word repetition that includes Wernicke and Broca regions directly connected via the arcuate fasciculus. Recent tractography findings of an indirect pathway between Wernicke and Broca regions suggest a critical role of the inferior parietal lobe for repetition. To test whether repetition deficits are associated wi

Interhemispheric compensation: a hypothesis of TMS-induced effects on language-related areas.

Repetitive transcranial magnetic stimulation (rTMS) applied over brain regions responsible for language processing is used to curtail potentially auditory hallucinations in schizophrenia patients and to investigate the functional organisation of language-related areas. Variability of effects is, however, marked across studies and between subjects. Furthermore, the mechanisms of action of rTMS are poorly understood. Here, we reviewed different factors related to the structural and functional organisation of the brain that might influence rTMS-induced effects. Then, by analogy with aphasia studi

Multimodal measures of sentence comprehension in agrammatism.

Agrammatic or asyntactic comprehension is a common language impairment in aphasia. We considered three possible hypotheses about the underlying cause of this deficit, namely problems in syntactic processing, over-reliance on semantics, and a deficit in cognitive control. We tested four individuals showing asyntactic comprehension on their comprehension of syntax-semantics conflict sentences (e.g., The robber handcuffed the cop), where semantic cues pushed towards a different interpretation from syntax. Two of the four participants performed above chance on such sentences indicating that not al

Neural basis of speech and grammar symptoms in non-fluent variant primary progressive aphasia spectrum.

The non-fluent/agrammatic variant of primary progressive aphasia (nfvPPA) is a neurodegenerative syndrome primarily defined by the presence of apraxia of speech (AoS) and/or expressive agrammatism. In addition, many patients exhibit dysarthria and/or receptive agrammatism. This leads to substantial phenotypic variation within the speech-language domain across individuals and time, in terms of both the specific combination of symptoms as well as their severity. How to resolve such phenotypic heterogeneity in nfvPPA is a matter of debate. 'Splitting' views propose separate clinical entities: 'pr

The perversion of language: Jules Baillarger on aphasia, the lateralization of speech, and the Baillarger-Jackson principle.

Jules Baillarger was one of the foremost figures of nineteenth-century neurobiology. He is remembered today for his discovery that the human cerebral cortex is composed of six intercalated layers. Even today, two horizontal fiber bundles within cortical layers IV and V are referred to as the outer and inner bands of Baillarger. It is a measure of the importance of Baillarger's discovery that his findings were elaborated upon with advances in microscopy and the development of methods for staining neurons and myelin by, notably, Theodor Meynert and Santiago Ramon y Cajal. Furthermore, Baillarger

The on-line processing of verb-phrase ellipsis in aphasia.

We investigate the on-line processing of verb-phrase ellipsis (VPE) constructions in two brain injured populations: Broca's and Anomic aphasics. VPE constructions are built from two simple clauses; the first is the antecedent clause and the second is the ellipsis clause. The ellipsis clause is missing its verb and object (i.e., its verb phrase (VP)), which receives its reference from the fully specified VP in the antecedent clause. VPE constructions are unlike other sentence types that require displacement of an argument NP; these latter constructions (e.g., object-relatives, wh-questions) yie
