BACKGROUND: Tuberous sclerosis is a complex disorder of hamartoma formation in many organs, particularly the skin, brain, eye, kidney, and heart. The characteristic skin lesions are angiofibromas (AF), the shagreen patch, periungual fibromas, and "ash-leaf" white macules. Treatment for AF has previously included electrocoagulation, electrodesiccation and curettage, dermabrasion, excision, cryosurgery, and oral 13-cis retinoic acid. This is the first report on the use of endoscopic argon photoirradiation to treat patients with low-grade ureteral tumors involving the distal ureter. Using a rigid ureteropyeloscope, a 300 mu or 600 mu quartz fiber, and 5 watts (W) of power in a continuous-wave mode, the lesions were ablated without complication. Patients remain free of ureteral recurrence after 5, 7, and 22 months. We conclude that the shallow (1 mm) depth of penetration and the ability to use the fiber in either a contact or noncontact mode provide the surgeon a large margin of safety and low risk of ureteral perforation.

We report six eyes in six patients in which corneal decompensation developed 18 or more months after argon laser iridectomy (ALI). In addition to the level of laser energy used, other risk factors included performing ALI during an attack, diabetes, and a cornea damaged by glaucomatous attack. The interval between the ALI and corneal decompensation ranged from 18 months to 3 1/2 years.

During the course of routine hospital surgical procedures, three patients lapsed into hypoxic cyanosis. Two expired immediately, another after four days of coma. Investigation of the hospital's central liquid oxygen tank revealed that it had been refilled recently and was labelled both "oxygen" and "argon." Mass spectrometric analysis of gas sampled from the questioned tank revealed a predominance of argon. A discussion of the sampling technique, method of analysis, role of the criminalist, and causes of this accident is presented.

Corneal argon laser photocoagulation (CALP) was used in 13 patients to treat deep stromal vascular ingrowth. Eight patients had undergone successful penetrating keratoplasty but had developed deep stromal vessels into the graft associated with signs of graft rejection, which did not improve with steroid treatment alone (group 1). The primary concerns in the development of a laser catheter for intravascular use are the potential hazards of vessel wall perforation and distal embolization. We present evidence, using technetium 99-labeled thrombi in two rabbit aortas and one human cadaver coronary artery, that distal embolization does not occur after argon laser recanalization. The application of the Argon laser in urology has proved to be effective in resecting recurrent, exophytic urothelial tumors of the bladder up to the size of a raspberry. In cases of wide spread bladder tumors we only perform a radiation of the resected area as local recurrence prophylaxis following transurethral resection. The urethroscopic Argon laser irradiation makes laser urethrotomy and evaporisation of urethral strictures possible. We performed argon laser photocoagulation on 31 eyes of 30 patients with presumed ocular histoplasmosis. Nineteen of 25 eyes (76%) with neovascular membranes 0.25 disk diameter or further from the fovea had 6/12 (20/40) or better vision after an average follow-up period of 20 months. Twenty-two of these eyes (88%) had the same or improved visual acuity during this period. When compared to a similar group of untreated eyes, the apparent advantage of photocoagulation in preserving visual acuity was not statistically significant.

Argon inhalation attenuates multiorgan failure (MOF) after experimental ischemic injury. Dieulafoy disease of the bronchus is a rare condition. A 28-y-old male patient with recurrent exacerbations of hemoptysis (producing up to 300 mL) over the previous 4 y was admitted to our hospital following an acute episode. A 3-mm sessile lesion covered with somewhat ulcerated smooth mucosa was detected at the distal end of the left main bronchus, at the entrance to the upper lobe bronchus. Chronic radiation proctitis calls for a disease of the rectum due to irradiation of pelvic malignancies. The results of treatment of 8 patients with hematochezia from chronic radiation proctitis are presented here. They were treated with argon plazmakoagulation during 1-4 sessions. Hematochezia has disappeared in all cases. Rectal pain was the reason for hospitalization in one case. After the treatment, improved quality of live was claimed by all patients.

PURPOSE: To better define the efficacy and safety of argon plasma coagulation (APC), specifically for brachytherapy-related proctitis, we reviewed the clinical course of 7 patients treated for persistent rectal bleeding. Approximately 2-10% of prostate cancer patients treated with 125I or 103Pd brachytherapy will develop radiation proctitis. The optimum treatment for patients with persistent bleeding is unclear from the paucity of available data. Prior reports lack specific dosimetric information, and patients with widely divergent forms of radiation were grouped together in the analyses.
