Wound botulism is an uncommon disorder that continues to be rarely reported in the United States. A 34-year-old intravenous heroin user was admitted to the Loma Linda, Calif., Veterans Administration hospital with multiple abscesses on his forearms. His clinical course was compatible with botulism, and his culture and serum were positive for Clostridium botulinum toxin type A. Early culture and/or serum identification can lead to prompt diagnosis, treatment, and improvement in the morbidity and mortality rates of this disease.

In brief: Previous reports of arm wrestling fractures have involved the distal third of the humerus. What makes this case unique is that the fracture occurred at the radial neck. The mechanism of injury probably involved extreme pronation where the radial head is locked into the capitellum-an area of potential weakness. Treatment was conservative, consisting of keeping the elbow flexed, 90° in a sling.

Myositis ossificans is a benign condition resulting from severe muscular contusion manifested by heterotrophic bone formation. The process is common in the anterior thigh but the literature dealing with myositis ossificans in the upper arm is limited. Ten cases of the condition in the upper arm were seen in our practice. A fragile site at the long arms (q21) of chromosome 16 was found in two persons, each of whom became the parent of a child with a de novo structural chromosome abnormality--a balanced autosomal translocation and an autosomal deletion. The question of an increased risk of structural chromosome abnormalities in the offspring of persons with fragile site long arm 16 is discussed.

This paper investigates the hidden yet persistent cost of conflict to birth weight outcomes for 53 developing countries experiencing conflict in the past three decades (1990-2018). Exploiting the variation across districts and conception months-years, we find that intrauterine exposure to armed conflict in the first trimester of pregnancy reduces child's weight at birth by 2.8% and raises the incidence of low birth weight by 3.2 percentage points. A 43-year-old man presented at our emergency department with a painful shoulder after a fall with his arm in abduction. Clinical presentation was typical for a luxatio erecta without evidence for neurovasculair damage. It is a rare but important diagnosis because of the high risk of plexus injury.

Cutaneous mucormycosis is an uncommon disease and occurs mainly in immunocompromised patients. We report an immunocompetent infant who developed primary cutaneous mucormycosis at an intravenous line site secured with an arm board and elastic bandage. The isolate was identified as the zygomycete, Rhizopus sp. High mortality rates are reported with invasive mucormycosis; however, early identification of the causative agent and antifungal therapy led to complete cure of the lesions in the reported case.
