Towards a standardised assessment procedure for developmental apraxia of speech.

This study addresses the assessment of developmental apraxia of speech (DAS) in children. For this, 11 children with a clear diagnosis of DAS were selected, based on documented speech history and perceptual evaluation of speech. The children with DAS, as well as 11 normal-speaking children, produced singleton real word and nonsense word imitations elicited in a standardised way. Phonetic transcriptions were analysed and errors in consonants classified. The results showed, firstly, that the children with DAS produced similar types of consonant errors as has been reported in the literature, whic

Maximal opposition approach to phonological treatment.

The purpose of this paper was to evaluate a phonological treatment program of maximal rather than minimal feature contrasts by charting the course of learning in a child displaying a systematic error pattern involving the nonoccurrence of word-initial consonants. Generalization data indicated that the child learned 16 word-initial consonants following treatment of only three sets of maximal opposition contrasts. Overgeneralization data indicated that the child restructured his phonological system based on a larger concept of "word initialness." Basic components of and differences between vario

Dynamical changes of occlusion and articulation during treatment of mandibular angle fractures.

Outcomes of mandibular fractures were stated as frequent reason of temporomandibular joint dysfunctions. The objective of this study was to evaluate dynamical changes of occlusion and articulation during treatment of mandibular angle fractures. 12 subjects with mandibular angle fractures underwent analysis of occlusion and articulation in all stages of healing. Findings of investigations were presented in Posselt's and intercondylar axis diagrams, occlusiograms and relative force vs. time graphs. Comparison of results in first day, 1, 3 and 6 months after trauma showed that despite typical pro

Cerebral Hemodynamics in Speech-Related Cortical Areas: Articulation Learning Involves the Inferior Frontal Gyrus, Ventral Sensory-Motor Cortex, and Parietal-Temporal Sylvian Area.

Although motor training programs have been applied to childhood apraxia of speech (AOS), the neural mechanisms of articulation learning are not well understood. To this aim, we recorded cerebral hemodynamic activity in the left hemisphere of healthy subjects (n = 15) during articulation learning. We used near-infrared spectroscopy (NIRS) while articulated voices were recorded and analyzed using spectrograms. The study consisted of two experimental sessions (modified and control sessions) in which participants were asked to repeat the articulation of the syllables "i-chi-ni" with and without an

Evaluation, treatment, and analysis of a rare case of motor speech systems dyscoordination syndrome.

This report describes an unusual presentation of a voice disorder arising from inability to coordinate the three components of motor speech: respiration, phonation, and articulation. These systems were individually intact, as demonstrated by laryngoscopy, motor speech examination, and treatment methods achieving success under controlled conditions. Following initial programming of his deep brain stimulation (DBS) device, a 62-year-old male, diagnosed with Parkinson's disease (PD) 14 years previously, abruptly experienced a vocal disorder characterized by pressed, very low frequency creaky voic

Surgical speech disorders.

Most speech disorders of childhood are treated with speech therapy. However, two conditions, ankyloglossia and velopharyngeal dysfunction, may be amenable to surgical intervention. It is important for surgeons to work with experienced speech language pathologists to diagnose the speech disorder. Children with articulation disorders related to ankyloglossia may benefit from frenuloplasty. Children with velopharyngeal dysfunction should have standardized clinical evaluation and instrumental asseessment of velopharyngeal function. Surgeons should develop a treatment protocol to optimize speech ou…

Reliability and Validity of the Mandarin Articulation and Phonological Test: Scale Development and Psychometric Properties for Preschool Children.

PURPOSE: This study aimed to (a) develop preliminary reference data for the Mandarin Articulation and Phonological Test (MAPT) and (b) examine its reliability and validity for assessing speech abilities in Mandarin-speaking preschool children aged 3-6 years. The study responded to the need for speech assessment tools with systematic psychometric documentation in Mandarin, where standardized instruments with comprehensive scoring systems remain limited. METHOD: Participants included 148 typically developing children (ages 3;0-6;11 [years;months]) and 76 children with speech sound disorders rec

The long-term speech outcome in Flemish young adults after two different types of palatoplasty.

OBJECTIVE: The success of cleft palate surgery is specifically determined by the subsequent speech characteristics. There are several types … palatoplasty. The authors hypothesized that a decreased overall intelligibility, more compensatory articulation disorders, higher nasalance values and more nasality disorders would occur in the two-stage Furlow palatoplasty. Moreover, an increased risk for dysphonic symptoms, caused by the more intensive vocal tract activities, were expected in subjects with higher nas …

The effect of ankyloglossia on speech in children.

OBJECTIVE: We wanted to determine whether ankyloglossia is associated with articulation problems and the effect of frenuloplasty on speech and tongue mobility. STUDY DESIGN: We conducted a prospective study of 30 children aged 1 to 12 years with ankyloglossia undergoing frenuloplasty. Outcomes were assessed by measurements of tongue mobility, speech evaluation, and parent questionnaires. RESULTS: Mean tongue protrusion improved from 14.2 mm preoperatively to 25.8 mm postoperatively (P < 0.01). Similarly, mean tongue elevation improved from 5.2 to 22 mm (P < 0.01). Impact of anterior single-tooth implants on quality of life, articulation and oromyofunctional behaviour: a pilot study.

The main purpose of this study is to determine the impact (one and a half year after implantation) of a single-tooth implant on articulation and oromyofunctional behaviour. This information is important for dentists, orthodontists or stomatologists who treat professional speakers. Objective (acoustic analysis) as well as subjective (questionnaire, phonetic inventory, phonetic analysis, oromyofunctional assessment of lip and tongue function, blowing, sucking and swallowing) assessment techniques were used to determine the quality of life, articulation and oromyofunctional behaviour. Fourteen su

Acoustic patterns of apraxia of speech.

Apraxia of speech (or verbal apraxia) is a controversial disorder, considered by some to be an impairment of the motor programming of speech. Because the disorder is characterized by "higher order" errors such as metathesis and segment addition as well as by errors of apparent dyscoordination of articulation, it seems to reflect a relatively high level of damage to the nervous system. This report presents acoustic descriptions of the speech of seven persons diagnosed as having apraxia of speech but without severe aphasic impairment, especially agrammatism. The acoustic results indicate a varie

Intra-articular Neoplasms and Masslike Lesions of the Knee: Emphasis on MR Imaging.

Intra-articular masses affecting the knee joint are uncommon lesions that encompass a range of neoplastic and nonneoplastic disorders. A joint mass limited to a single articulation is most commonly related to neoplastic or focal proliferative disease, whereas masses affecting multiple articulations are typically caused by underlying inflammatory arthritides, metabolic abnormalities, or systemic deposition disorders. This article focuses on those masses that present in a monoarticular fashion, emphasizing the lesions that most commonly affect the knee joint. MR imaging is the modality of choice
