Scalp autografts and hair transfer to the face in the burned child.

Hair transfer from split-thickness skin grafts harvested from the scalp is not a widely reported problem. The authors present their experience with hair transfer from scalp autografts in a pediatric burn population, with particular emphasis on hair transfer to the face. They retrospectively reviewed 3307 acute and reconstructive pediatric burn admissions over a 4-year period at a single institution and identified 109 cases in which the scalp had been used as a donor site and in which the patient survived the acute burn period. Data from 73 male and 36 female patients were analyzed with respect…

Burns (Part 2). Tops and flops using cultured epithelial autografts in children.

The goal of this article is to review the status of cultured epithelial autografts in clinical practice with particular focus on the pediatric subset of patients. The current indications include massive deep burns (>60 - 70% total body surface area), resurfacing-type postburn scar revisions, and skin defect coverage following excision of large skin lesions like giant nevi. Although this method can be lifesaving for massively burned patients, and although excellent functional and cosmetic results may be obtained under ideal circumstances, formidable problems continue to exist. Take is inconsist…

Long-term duct-occluded segmental pancreatic autografts: absence of microvascular diabetic complications.

BACKGROUND: Current insulin therapies for control of glucose metabolism in patients with type I diabetes mellitus prevent major metabolic consequences of insulin deficiency, but none prevents or arrests long-term complications. In experimental models of canine diabetes, retinopathy, neuropathy, and nephropathy have been shown to develop within 5 years. The aim of this study was to determine in a canine model whether glucose control provided by segmental duct-occluded pancreas autografts could prevent the long-term complications of diabetes. METHODS: Thirty-five outbred mongrel dogs underwent …

Human glabrous skin autografts partially reinnervated without sensory corpuscles. An immunohistochemical study.

The reinnervation of human glabrous skin autografts was investigated in biopsy specimens obtained four weeks to 15 months after transplantation. The grafted skin was taken from the volar aspect of the wrist and transplanted to the fingers. Immunohistochemical methods were used to detect the presence of nerve fibres and sensory corpuscles, using monoclonal antibodies against neurofilament proteins and S-100 protein. In normal skin, immunoreactivity of neurofilament proteins was localised in the axons of nerves and sensory corpuscles, while S-100 protein immunoreactivity was found in Schwann cel…

Long-term follow-up of nonvascularized fibular autografts for distal radial reconstruction.

Three patients with giant cell tumors of the distal radius had en bloc excision of the distal radius and replacement with the ipsilateral fibula. Two of the patients were followed for 16 years and one for 14 1/2 years. Forearm rotation, as well as wrist motion, was limited in all three patients, yet they remained functional and pain free except during periods of prolonged or excessive use. All three patients were pleased with the results. Bone union occurred primarily in all three patients without a supplemental bone graft or microvascular anastomosis. This method has a definite place in the h

Interfascicular suture with nerve autografts for median, ulnar and radial nerve lesions.

Interfascicular nerve suture with autografts is the operation of choice for repairing peripheral nerve injuries because it ensures more precise alignment of the fasciculi and so better chances of reinnervation of the sectioned nerve. The procedure as described by Millesi et al has been used at the Istituto Neurologico di Milano in 30 patients with traumatic lesions of the median, ulnar and radial nerves. All have been followed up for 2 to 7 years since operation. The results obtained are compared with those of other series obtained with interfascicular suture and with epineural suture. Microsu…

Preliminary report on experimental free autografts of skeletal muscle to the myocardium.

In three greyhounds, free grafts of pronator teres muscle from the foreleg were applied to the left ventricular myocardium, and showed successful gross and microscopic survival of the graft six months later. The free grafts of skeletal muscle were transplanted as complete muscle bellies two weeks after preliminary denervation by median nerve neurectomy, and at the time of suture to the myocardium, the central end of the divided left phrenic nerve was implanted into the graft. The animals were normally active and preserved normal electrocardiograms after grafting. The use of such grafts in clin

Comparison of Postoperative Outcomes of Rotational Acetabular Osteotomy Using Autografts and Allografts.

BACKGROUND: Developmental dysplasia of the hip (DDH) contributes to the progression of hip osteoarthritis (OA), particularly in younger individuals. To delay OA development, joint-preserving procedures such as rotational acetabular osteotomy (RAO) improve hip stability by repositioning the acetabulum. Originally introduced by Ninomiya and Tagawa, RAO involves filling the osteotomy gap with autografts or allografts. Since 2013, our institution has transitioned from autografts to allografts. This study compares postoperative outcomes between these graft materials, as no prior studies have direct…

Graft fixation influences revision risk after ACL reconstruction with hamstring tendon autografts.

Background and purpose - A large number of fixation methods of hamstring tendon autograft (HT) are available for anterior cruciate ligament reconstruction (ACLR). Some studies report an association between fixation method and the risk of revision ACLR. We compared the risk of revision of various femoral and tibial fixation methods used for HT in Scandinavia 2004-2011. Materials and methods - A register-based study of 38,666 patients undergoing primary ACLRs with HT, with 1,042 revision ACLRs. The overall median follow-up time was 2.8 (0-8) years. Fixation devices used in a small number of pati

Anterior Cruciate Ligament Revision Surgery: Ipsilateral Quadriceps Versus Contralateral Semitendinosus-Gracilis Autografts.

PURPOSE: To evaluate the subjective outcomes, knee stability, and donor-site morbidity after revision ACL reconstruction using either autologous ipsilateral quadriceps tendon or contralateral semitendinosus-gracilis tendon. METHODS: A sample size calculation suggested that we needed 25 patients in each group to detect equality between both groups. Therefore, we evaluated 30 consecutive patients who underwent an ACL revision surgery with ipsilateral bone-quadriceps tendon grafts and 30 consecutive patients with the contralateral semitendinosus-gracilis grafts between January 2010 and December

Intramuscular injection of parathyroid autografts is a viable option after total parathyroidectomy.

INTRODUCTION: Surgical transplantation of parathyroid gland into muscle is an established technique after total parathyroidectomy for renal hyperparathyroidism. However, no study has examined the role of injecting parathyroid tissue in these patients. We compared the outcome of surgical transplantation of parathyroid glands by implantation ("implant") versus that of intramuscular injection ("inject"). METHODS: Patients who had total parathyroidectomy for tertiary hyperparathyroidism due to chronic renal failure from 2001-2008 are included in this study. For the implant group, a parathyroid gl

Optimal size and location for corneal rotational autografts: a simplified mathematical model.

OBJECTIVE: To calculate clinical guidelines for the optimal location and size of a rotational autokeratoplasty. METHODS: The ideal graft size and trephine decentration for a rotational autograft were calculated based on scar location using geometric models. Mathematical variables were set to maximize postoperative visual acuity and for generalization of the geometric model. This model was used in a rotational autokeratoplasty of a patient with a history of a corneal scar and diplopia. An 8-mm autograft was decentered 0.5 mm superiorly and rotated 180 degrees to relocate the scar to the superi

"Flypaper technique" a modified expansion method for preparation of postage stamp autografts.

The gold standard for management of extensive burn has been early excision, temporary allografting and final autografting. However, "lack of donor skin" is a challenge condition when autografting in the treatment of extensive burns. Designing an efficient and easy to apply expansion method may improve burn care quality and shorten the hospital stay period. From December 1998 to May 2004, we have performed fly paper technique postage stamp skin autografting for eight major burn patients in the Kaohsiung Medical University Hospital. By using a quick cutting plate, chessboard tray and petrolatum
