Why do almost all mammals have seven cervical vertebrae? Developmental constraints, Hox genes, and cancer.

Mammals have seven cervical vertebrae, a number that remains remarkably constant. I propose that the lack of variation is caused by developmental constraints: to wit, changes in Hox gene expression, which lead to changes in the number of cervical vertebrae, are associated with neural problems and with an increased susceptibility to early childhood cancer and stillbirths. In vertebrates, Hox genes are involved in the development of the skeletal axis and the nervous system, among other things. In humans and mice, Hox genes have been shown also to be involved in the normal and abnormal (cancer) p

[Sports injuries of the cervical spine (author's transl)].

The article reports on injuries of the cervical spine occurring during sports activities. An attempt is made to reconstruct the movements which led to the cervical spine injuries in question. In two cases of accidents occurring during bathing, one football accident and a toboggan accident, the injuries concerned point to hyperextension of the cervical spine as cause of the injury. In another football accident and a riding accident, the changes observed allow us to conclude that the movement leading to the injury must have been a hyperflexion. One accident occurring while jumping on the trampol

Splitting of the semispinalis capitis muscle as a less invasive approach for atlantoaxial fusion - A technical note.

PURPOSE: Atlantoxial fusion with screw rod constructs traditionally requires subperiostal dissection of the C1/C2 muscle attachments, which may lead to postoperative axial neck pain and intraoperative blood loss. We evaluate a potentially less invasive, muscle splitting approach for atlantoaxial fusion in a cadaver model. METHODS: A technical feasibility study was performed in a human cadaveric specimen with standard surgical instruments and an operating microscope. Surgically relevant anatomical structures as well as final exposure of the C1, C1/C2 and C2 screw entry points are demonstrated.

Transoral-transpharyngeal approach to the upper cervical vertebrae.

The classical operative approaches to the cervical spine include the posterior one and the anterior exposure along the sternomastoid muscle. However, neither of these are helpful in exposing the upper cervical vertebrae, especially the odontoid process, atlas, and axis. We have used the transoral-transpharyngeal exposure for lesions of the odontoid process and upper (first to third) cervical vertebrae in six patients. The pathologic processes included rheumatoid disease and fracture of the cervical vertebrae, suspected tumor with compression of the spinal cord, basilar invagination, and compre

C2 over C3 spondyloptosis in a case with absent posterior elements. Report of an unusual case and analysis of treatment options.

We report an extremely rare anomaly in a 31-year-old male patient, where the posterior elements of the axis were almost completely absent and the anterior elements were dislocated anterior to the body of the C3 vertebra. The body of C3 caused severe compression of the high cervical cord. A radical transoral decompression and plate and screw fixation resulted in sustained clinical improvement. The relevant literature is briefly reviewed and the treatment options in such a case are analyzed.

Cervical vertebral anomalies in cleft lip and palate.

A survey was made of the upper cervical vertebrae in children with cleft lip and palate in order to determine the prevalence of cervical vertebral anomalies. The cleft sample consisted of 105 patients attending for orthodontic treatment. It was subdivided into cleft lip (CL), cleft palate (CP), unilateral cleft lip and palate (UCLP) and bilateral cleft lip and palate (BCLP) subgroups. A control was comprised of 120 orthodontic patients. The cervical vertebral anomalies were classified into two types, posterior arch deficiency (PAD) and fusion anomalies (FUS). The results confirmed that the cer

Surgical Management Of Irreducible Atlanto-Axial Dislocation With OS Odontoideum And Klippel-Feil Syndrome.

Klippel-Feil syndrome (KFS) is the congenital fusion of two or more cervical vertebrae which is often associated with various other abnormalities in the cervical spine. Involvement the upper cervical segments leads to atlanto-axial instability which manifests as progressive neurological symptoms due to compression on the spinal cord. These cases pose a surgical challenge due the abnormal and unique anatomy of each patient. A 37-year-old patient presented with neck pain and cervical myelopathy due to a posterior subluxation of C2-3 fused segment over C4-6 fused segment. The patient had an os od

Evaluation of the efficiency of cervical orthoses on cervical fracture: A review of literature.

BACKGROUND: Various methods have been used to stabilize the vertebra in cervical fractures, including the use of various orthoses and surgery. However, it is not cleared which type of orthosis is more suitable for the subjects with cervical fractures to best immobilize the vertebra and to decrease the associated side effects. Therefore, the aim of this study was to evaluate the efficiency of various orthoses based on the available literature. MATERIALS AND METHODS: A search was done in some databases include PubMed, ISI Web of Knowledge, EBSCO, Embasco, and Google Scholar. The search was done

Axis fractures: a comprehensive review of management and treatment in 107 cases.

The combination of movement, location, and anatomy of the axis predisposes it to multiple and varied fracture/dislocations distinct from other vertebrae. We examine all forms of axis fractures and address the appropriate treatment for each specific fracture type. In a retrospective review of 625 cervical spine fractures during an 8-year period, we found 107 axis fractures. There were 25 hangman's fractures (23%), 59 odontoid fractures (55%), and 23 miscellaneous fractures (22%). Each case was characterized by age, sex, the presence of associated injuries, presenting symptoms and findings, init

Review of the tetrapod skull-neck boundary: implications for the evolution of the atlas-axis complex.

This review describes variation in modern and fossil occiput-atlas-axis complex anatomy of total group Tetrapoda with the aim of documenting the range of structural variation throughout their evolutionary history to establish grounds for comparison of the complex between tetrapod clades. This review reveals that every modern tetrapod has an atlas with morphology unique to its vertebral column that articulates to the skull, composed, typically, of paired neural arch halves and an intercentrum. Maximally complex tetrapod atlantes articulate to paired proatlas halves and are composed of paired ne

La maladie de Grisel: a rare occurrence of "spontaneous" atlanto-axial subluxation after pharyngoplasty.

A review of the literature, with a short discussion of the possible aetiology, diagnosis and treatment of "la maladie de Grisel" is presented with a case report of atlanto-axial subluxation occurring in a child after pharyngoplasty. The occurrence of a "spontaneous" subluxation after pharyngoplasty has not been previously reported. The possible scale of neurological complications which may range from relatively minor nerve root compression to a potentially lethal major cord compression emphasises the importance of this condition especially as a minor trauma may precipitate a major disaster. We

Reiter disease with atlanto-axial subluxation.

The authors report the first known case of atlanto-axial subluxation in Reiter arthritis. The type of analysis, including radiographic and laboratory examinations, required for a diagnosis of atypical Reiter disease is an important feature of the case.
