Lithogenesis and bile metabolism.

Gallstone disease exacts a considerable financial and social burden worldwide leading to frequent physician visits and hospitalizations. Based on their composition, gallstones are categorized as cholesterol, black pigment, and brown pigment, with each category having a unique structural, epidemiologic, and risk factor profile. Cholesterol crystal formation requires the presence of one or more of the following: (a) cholesterol supersaturation, (b) accelerated nucleation, or (c) gallbladder hypomotility/bile stasis. Some risk factors for cholesterol stones include age, gender, genetics, obesity,…

The microflora of bile in Ghanaians.

Bile was obtained from the gall bladder of 104 patients undergoing cholecystectomy for gall stone disease. Bile was also obtained from the common bile duct and T-tube of 17 patients who also had exploration of common bile duct. During the same period, 148 cholecystectomies were performed. The specimens were sent for culture and sensitivity and 32.7% of the specimens grew bacteria. The factors that were associated with positive culture were emergency cholecystectomy for acute cholecystitis and empyema of gall bladder, carcinoma of gall bladder and obstructive jaundice. The commonest organisms w…

[Papillomatosis of the bile ducts].

In patients with signs of obstructive jaundice also rare diseases such as biliary papillomatosis should be taken into consideration. These primarily benign epithelial tumours show a tendency of recurrence and malignant degeneration and, therefore, have to be regarded as precancerous. An early preferably preoperative diagnosis is essential, because extensive surgical treatment is required. Today this diagnosis can be made by ultrasound, ERCP or PTC. This study describes 2 cases of progressive biliary papillomatosis observed in 1978 and 1985. (One female 48 years, one male 53 years). The first c

[Proximal bile duct cholangiocarcinomas].

AIM: The aim of this retrospective study was to report a series of 12 patients with hilar cholangiocarcinoma treated by curative resection. PATIENTS AND METHODS: Between 1980 and 1998, curative resection was performed in 12 patients out of 33 patients with a Klatskin's tumor. Preoperative biliary drainage was performed in 6 cases. Resection was limited to the extrahepatic bile duct in one case only. Resection was extended to the liver in 11 cases including segmentectomy I (n = 9), left hepatectomy (n = 7), right hepatectomy (n = 3) and segmentectomy IV (n = 1). Postoperative brachytherapy was…

Postoperative bile duct strictures.

Bile duct strictures are an uncommon but serious complication of primary operations on the gallbladder or biliary tree. Most strictures occur as a result of injury to the bile duct during cholecystectomy. In addition, strictures can occur at the site of previous biliary anastomoses for reconstruction of the biliary tree. Most patients with benign bile duct strictures present soon after their initial operation; however, in some cases, presentation is delayed for years. Cholangiography is essential for defining the anatomy of the biliary tree prior to management. In many cases, nonoperative bili…
