Bronchial-biliary fistula secondary to cholangiocarcinoma: long-term efficacy of biliary self-expandable metal stent.

A 77-year-old female with previous medical history of non-cirrhotic chronic hepatitis B and hepatocellular carcinoma treated with sequential … ented with new-onset dyspnea and bilioptysis and abdominal computed tomography revealed a bronchial-biliary fistula. ERCP was performed to place a self-expandable metal stent in the biliary tract, which resulted in rapid clinical improvement. The patient has been followed for 2 years and remains globally stable with two episodes of worsening of bilioptysis second …

Multiple Hepatic Hydatid Cysts With Cysto-Biliary Communication and Broncho-Biliary Fistula: A Pediatric Case Report.

Hydatid cyst disease is a health problem in some developing countries occurring in groups with permanent contact with dogs. It is a parasitic disease caused by Echinococcus Granulosus. The liver is the most common organ infected. If not treated appropriately; it may lead to developing life-threatening complications. Its rupture into bile ducts is the most frequent one, while hepatopulmonary fistula is a rare complication. We discuss the case of a child presenting those 2 complications as we emphasize the importance of early diagnosis and the role of the radiologist.

Biliary Colonic Fistula Formation in the Setting of Obstructive Cholelithiasis.

Biliary enteric fistulas are rare form of internal biliary fistula encountered in <1% of patients with biliary disease while cholecystocolic fistulas are even rarer with reported incidence of 0.06%-0.14% in patients undergoing cholecystectomy. We present a case of biliary colonic fistula that developed in the context of obstructive cholelithiasis, emphasizing a pathological process involving chronic inflammation due to gallstone with a gradual erosion into the surrounding structure. A high index of suspicion is required for diagnosis, given the presentation with nonspecific symptoms. Given the…

Percutaneous embolization of persistent biliary and enteric fistulas with Histoacryl.

PURPOSE: To describe our experience with transcatheter n-butyl-2-cyanoacrylate (NBCA) embolization of refractory enteric or biliary fistulas. MATERIALS AND METHODS: Between March and December 2009, a retrospective analysis of patients with enteric or biliary fistulas revealed 11 cases unresponsive to drainage and treatment with NBCA (seven men; age range, 36-81 y). Fistula output ranged from 100 to 300 mL per day. Embolization was performed 20-55 days (mean, 32 d) after a fistula was noted on a radiograph of the abscess performed through a previously placed percutaneous drainage catheter. Dig…

[Ultrasonographic diagnosis of a biliary-digestive fistula with gallstone ileus].

HISTORY AND CLINICAL FINDINGS: A 72-year-old woman was admitted to the hospital with upper abdominal pain, nausea, vomiting, and obstipation. INVESTIGATIONS: Percutaneous ultrasound found aerobilia, a biliary-digestive fistula, and an ileus with gallstones within the small bowel. The findings were confirmed at computed tomography. TREATMENT AND COURSE: A gallstone ileus on the basis of a biliary-digestive fistula was diagnosed. The patient was treated by surgery and with antibiotics. CONCLUSION: This case report demonstrates that percutaneous ultrasonography should be used as the first imag

Surgical experience in a baby with congenital broncho-biliary fistula.

Congenital tracheobiliary and bronchobiliary fistulae are rare malformations in which patent communications exist between the respiratory system and biliary tract, respectively. We present a newborn who was admitted with respiratory distress and bilious tracheal discharge. Investigation revealed a bronchobiliary fistula originating from the left main bronchus, as well as biliary atresia. Excision of the bronchobiliary fistula was successful and the connection between biliary tract and gastrointestinal system was established by performance of a Roux-en-Y cholecysto-jejunostomy. Diagnostic tools…

[Management of biliary fistulas in surgery for hepatic hydatid cyst].

One of the most important problems of the surgical treatment of the hydatid disease of the liver is the remaining cavity of the cyst. Its evolution is mainly decided by the existence and the debit of a biliary fistula. During 10 years (1997-2006), 138 patients with 166 hydatid cysts of the liver were admitted and operated in our clinic. Among them, 57 cysts had a biliary fistula, found during the operation, while in 11 other patients the fistula became obvious 1 or 2 days after the procedure. The incidence of biliary fistulas (68 of 166 cases, which means 41%) is higher, due to the location of…

Diagnosis and management of hepatic hydatidosis complicated with biliary fistula.

A total of 1,204 patients with liver hydatidosis were treated by operations in our hospital from 1953 to 1990. Of these 74 had biliary fistulae. Growth of echinococcus cyst causes displacement, distortion and stenosis of the hepatic ductules with impaired bile drainage. Biliary effusion may occur between the endo- and ecto-cyst walls. Long term compression renders the hepatic ductule atrophic, and liable to rupture, forming a hydatid cyst-biliary fistula. The hydatid cyst can rupture into the biliary tract, and cyst fluid escapes into the biliary tract with daughter cysts discharged into the c…

Percutaneous transhepatic biliary drainage for bile leaks and fistulas.

Percutaneous transhepatic cholangiography and biliary drainage were performed in 12 patients with major injuries to the bile ducts manifested by biliary leaks and fistulas. Eleven of the 12 patients had had inadvertent biliary trauma during surgery. In six patients, the biliary leaks sealed with percutaneous drainage. In other patients requiring definitive surgical procedures on the biliary tract, initial percutaneous drainage allowed these procedures to be delayed until the patients' clinical condition improved. Percutaneous biliary drainage is an important adjunct to the management of patien

Spontaneous external biliary fistula in a patient with heroin addiction.

Spontaneous external biliary fistula, once common, is now a rare clinical entity. A patient with this lesion is described and attention is drawn to the fact that this rare condition is still being seen in certain patients with neglected cholecystitis. Heroin addiction was thought to have modified his response to gallbladder disease and contributed to the perforation. Diagnosis was suspected on physical examination and confirmed by spontaneous discharge of stones and fistulogram.

Utility of stent-grafts in treatment of porto-biliary fistula.

A porto-biliary fistula causing hemobilia is a known complication of percutaneous transhepatic biliary drainage (PTBD). We present two patients with hemobilia secondary to porto-biliary fistula, treated successfully by percutaneous placement of stent-grafts. In one case, the stent-graft was placed in the bile duct, and in the other case, it was placed in the intrahepatic portal vein branch. Hemobilia stopped and there were no complications except a small area of hepatic infarction, distal to the stent-graft in the portal vein.

[Methods of treatment for suppurative biliary fistulas of amebic etiology].

Experience in the treatment of 43 patients with pyobiliary fistulas which formed after abscessotomy in amebic abscess of the liver is generalized. The fistulas occurred after open drainage in 29 patients, spontaneous opening of the abscess on the external surface of the body in 8, and after operation for transdiaphragmatic opening of a hepatic abscess in 6 patients. Two groups of patients were distinguished according to the method of completion of the operation. Among the 24 patients of the first clinical group 9 were subjected to drainage of the residual cavity with excision of the fistula, b…
