Despite marvelous advances in repair for acute type A aortic dissection over past decades, it remains challenging to repair the aortic root when aortic dissection extended to the sinuses causes the fragile root because of its thinner layers, which are susceptible to suture trauma. Here, we describe a modified Florida sleeve technique to strengthen the weakened aortic root. After mobilization of the aortic root and the coronary arteries, a designed Dacron tube graft was wrapped outside the sinuses as neo-adventitia to reinforce the dissected weakened wall. During surgery for aortic dissection,

OBJECTIVES: To present initial experience with a new modular transfemoral multibranched stent graft for treating aortic arch aneurysms.
METHODS: Six patients, considered high risk for open surgery, were treated with custom made branched stent grafts. All patients had a staged left carotid subclavian bypass before the endovascular procedure. Each branched graft had a 12 mm side branch for the innominate artery and an 8 mm side branch for the left common carotid artery.
RESULTS: Four patients out of six had uneventful placement of the prostheses, with successful exclusion of their aneurysms. O

Endovascular aortic arch surgery is an evolving field that offers less invasive alternatives to open surgery. The emergence of new devices and their implantation imply new considerations in the management by the anaesthesiologist during the procedure. An example of this is the NEXUS TRE™ stent, designed to preserve the patency of the supra-aortic trunks without the need for extra-anatomic bypass. We present the case of a 75-year-old man diagnosed with an aortic arch aneurysm with accelerated growth and risk of rupture, who underwent endovascular repair of the aortic arch and its three branches

BACKGROUND: Postoperative kinking of a stent graft following total arch replacement using a frozen elephant trunk technique (TARFET) is a rare but significant complication. Such kinking can lead to serious issues, including heart failure, hemolysis, graft thrombosis, and significant upper and lower limb blood pressure difference. Although several cases involving various frozen elephant trunk (FET) devices have been documented, this report presents a potentially novel instance of stent graft kinking following implantation of an E-Vita Open Neo prosthesis.

Arterial pseudoaneurysms represent an uncommon complication of acute pancreatic inflammation or chronic pancreatitis. We describe a contained rupture of a suprarenal abdominal aortic pseudoaneurysm. An aorto-uni-iliac stent-graft was adopted as the aortic main body and was combined with two chimneys and two periscope stents for celiac/superior mesenteric artery and renal arteries, respectively. The procedure was complicated by the entrapment of the celiac sheath into the barbs of the aortic stent-graft and the attempts to remove the sheath resulted in an upward migration of the stent-grafts. A

OBJECTIVES: The aim of this study was to evaluate the feasibility and safety of a non-customized modular inner branched stent graft for total endovascular aortic arch repair in a porcine model.
METHODS: The modular inner branched stent graft system with a split main body design included 1 proximal main component, 1 distal main component, and 1 branched covered stent. The gutter in the proximal main component was sealed with sutured membrane. Fatigue testing was performed to evaluate the durability of the stent graft. Fifteen pigs were used in this study. In each pig, a stent graft was deliver

BACKGROUND: Endovascular aortic aneurysm repair (EVAR) with stent-grafts is used widely for the treatment of thoracic aortic aneurysms (TAA). Inappropriate design of stent-grafts may lead to complications such as endoleak, stent-graft migration and new entries, causes of which may be inappropriate radial support force or insufficient longitudinal flexibility of the stent-grafts.
MATERIAL AND METHODS: To improve the mechanical performance of the stent-grafts, a type of non-equal-strut stent hoops was proposed, and the influence of structural parameters on the mechanical performance was studied

A 31-year-old man with a history of multiple strokes of unknown etiology presented with multiple asymptomatic visceral artery aneurysms including a 3.3-cm bilobed splenic artery aneurysm and a 3.1-cm fusiform superior mesenteric artery aneurysm. He had no family history of aneurysms, and there was no evidence of collagen disease. He underwent open splenic artery aneurysm resection without splenectomy and had an uneventful recovery. Seven months later, he had open repair of his 14.6-cm-long fusiform superior mesenteric artery aneurysm with a 15-cm-long collagen-coated polyester interposition gr

Endovascular repair of thoracic and thoraco-abdominal aortic aneurysms became apparent as an alternative to open repair. When the distal landing zone proximal to celiac artery is inadequate, a traditional open surgical approach with thoracoabdominal aortic replacement concomitant with visceral and renal bypasses is necessary. Alternatively, either an abdominal hybrid procedure with debranching of the visceral vessels with subsequent thoracic stent graft placement or complete endovascular aneurysm exclusion with branched stent grafts is required. Extending the distal landing zone might be possi

Although open aneurysm repair remains the standard of care, the advent of endoluminal techniques and stent grafts has significant potential to improve treatment of aneurysms of the innominate and subclavian arteries. Regardless of the modality chosen, therapy must be individualized and treatments tailored on the basis of anatomic and etiologic considerations. Occasionally, definitive treatment of an aneurysm with endoluminal stent-graft placement is possible for focal aneurysms or pseudoaneurysms of the innominate or subclavian arteries. In other situations, endoluminal grafting may be combine
