Acute shortening and re-lengthening versus antibiotic calcium sulfate-loaded bone transport for the management of large segmental tibial defects after trauma.

BACKGROUND: The purpose of this paper was to compare the clinical effects of acute shortening and re-lengthening (ASR) technique with antibiotic calcium sulfate-loaded bone transport (ACSBT) technique for the management of large segmental tibial defects after trauma. METHODS: In this retrospective study, 68 patients with large segmental tibial defects were included and completely followed. The bone loss was 3-10 cm. ASR group included 32 patients, while ACSBT group contained 36. There was no significant difference in demographic information between the two groups. The external fixation time (

Autograft vs Allograft With Bone Marrow Aspirate Concentrate as an Osteotomy Gap Filler in Lateral Column Lengthening for Progressive Collapsing Flatfoot Deformity.

BACKGROUND: Progressive collapsing flatfoot deformity (PCFD) is a complex condition characterized by hindfoot valgus, midfoot varus, and forefoot abduction, leading to functional impairment and pain. Surgical correction often includes lateral column lengthening (LCL), which addresses structural deformity and restores alignment. Autografts remain the gold standard as an osteotomy gap filler for LCL despite donor site morbidity. Allografts augmented with patient's own bone marrow aspirate concentrate (BMAC) offer a potential alternative because of their osteogenic properties. This study compares

Acute Shortening and re-lengthening in the management of open tibia fractures with severe bone of 14 CMS or more and extensive soft tissue loss.

INTRODUCTION: A traumatic event to the tibia with more than 14 cm of bone and soft tissue loss represents a challenge to most orthopedic surgeons and is considered a limb-threatening condition. Few solutions are available in such clinical situations and among them is the acute shortening and re-lengthening of bone and soft tissue. MATERIALS AND METHODS: Our study presents the management of 7 patients with grade III B open fractures (according to the Gustillo-Anderson classification) of the tibia who underwent resection of all the devitalized tissues, acute limb shortening to close the defect,

[Bone distraction using an external fixator: a new mandibular lengthening technic. A preliminary study apropos of 2 cases of children with mandibular hypoplasia].

The authors expose a technical innovation concerning mandibular lengthening without any bone graft, by applying to the mandibular Ilizarov's principles about limb lengthening by osseous distraction. This surgical technique concerns children with mandibular hypoplasia, like the Hanhart's syndrome (aglossia-adactylia, first observation), or Hemifacial Microsomia (second observation). The purpose of this new technique is mandibular lengthening with functional and aesthetic correction of the mandibular growth deficiency, and minimal morbidity. A specialist staff is essential to realize a distracti

Confounding Association of Lower Limb Hypertrophy With Retarded Long Bone Growth in Mosaic Somatic Neurofibromatosis 1-A Genetic Review and Femoral Lengthening Treatment.

This presentation describes a 15-year-old adolescent boy with neurofibromatosis type 1 with regional somatic mosaicism in the right half of his body. The unique clinical features are highlighted by generalized hemihypertrophy of the lower extremity in association with progressive growth retardation of long bones, which results in limb shortening. The patient successfully underwent limb lengthening. To our knowledge, this is the first neurofibromatosis type 1 article to report of a limb-lengthening procedure in a patient with two major and opposite growth phenomena on the same limb caused by a

Therapeutic effect of intramedullary reaming and nailing for long bones lengthening in children with Ollier disease and Maffucci syndrome on enchondromas: multicentric retrospective case series.

INTRODUCTION: Leg length discrepancy (LLD) and malalignment of long bones are frequent orthopedic problems encountered in Maffucci syndrome and Ollier disease (OD). Orthopedic surgeons used historically external fixators to address the deformities. In this multicentric case series, we propose the use of motorized intramedullary nails. METHODS: We retrospectively reviewed for 9 years, in four different centers, patients with OD and Maffucci's syndrome that had lengthening nails for LLD with or without associated deformities. The minimum follow-up period was 24 months. We reported complications

All Internal Segmental Bone Transport and Optional Lengthening With a Newly Developed Universal Cylinder-Kombi-Tube Module for Motorized Nails-Description of a Surgical Technique.

Segmental transport is an effective method of treatment for segmental defects, but the need for external fixation during the transport phase is a disadvantage. To avoid external fixation, we have developed a Cylinder-Kombi-Tube Segmental Transport (CKTST) module for combination with a commercially available motorized lengthening nail. This CKTST module allows for an all-internal segmental bone transport and also allows for optional lengthening if needed. The concept and surgical technique of CKTST are described and illustrated with a clinical case.

Pelvic Support Osteotomy; Salvage Procedure in Chronically Dislocated Hips Case Report: Technical Note on the On-lay Bone Positioning of a Lengthening Nail and Review of the Literature.

BACKGROUND: Young patients with a acquired dislocation of the hip with concomitant shortening of the lower limb is a difficult diagnostic problem with few definite treatment options. METHODS: We used the technique, originally described by Ilizarov, with a double femur osteotomy and lengthening the femur. Lengthening was done with a sub muscular placed on-lay femur distraction rod. Distraction of the rod was done by fluid mechanics. After lengthening, the distraction was secured by a parked plate by locking the distal part of the plate. RESULTS: This patient needed a lengthening of 60 mm, dis

[Primary shortening--secondary lengthening. A new treatment concept for reconstruction of extensive soft tissue and bone injuries after 3rd degree open fracture and amputation of the lower leg].

The main problem in major limb replantation--especially of the lower extremity--is an extensive bone- and soft-tissue loss. The traditional replantation concept tries to preserve the initial limb length; only a small shortening is accepted. To avoid a more extensive shortening, often insufficient debridement at the time of replantation is carried out. After successful revascularisation, bone and soft-tissue defects will be reconstructed according to the principles of staged reconstruction. Especially segmental nerve defects of more than one major peripheral nerve and severe skin and muscle los

Leg-lengthening procedures. A historical review.

While there are new techniques for lengthening long bones, there are, nevertheless, contraindications and preoperative considerations for lengthening the femur or tibia, such as the presence of acetabular dysplasia, fixed deformity around joints, peripheral vascular and metabolic bone disease, and the age of the patient. Lengthening a long bone may be associated with many complications, especially in a congenitally short bone. It is important to anticipate and prevent complications, especially nonunion, subluxation of joints, muscle contractures, and neurologic complications. Lengthening of th

Posttraumatic limb shortening: treatment with the Ilizarov distraction device.

BACKGROUND AND AIMS: Limb length inequality is not only a cosmetic problem, it is also a functional concern. The aim of the study was to analyse the cases of limb lengthening after their posttraumatic shortening. The study design is descriptive and comparative. MATERIAL AND METHODS: During the years 1979-1994 14 patients between 4-18 years of age were treated. Limb shortening was the result of premature closure of physeal plate and severe traumas with large bone defects. Ilizarov external devices for limb lengthening were used. The lengthening index was counted. The patients were divided into

Use of ultrasonography in evaluation of new bone formation in patients treated by the method of Ilizarov.

BACKGROUND: Lengthening of bones by gradually distracting bone fragments using an external apparatus by Ilizarov, is a long process with numerous complications. The greatest threats in limb lengthening are poor new bone formation as well as premature consolidation of the newly generated bone. The purpose of this study was to determine the importance of ultrasonography in evaluation of bone formation in limb lengthening. PATIENTS AND METHODS: The study involved 31 patients, in whom 52 long bones were lengthened by the Ilizarov method at the University Clinic for Orthopaedic Surgery in Skopje f

Limb lengthening and correction of deformity in the lower limbs of children with osteogenesis imperfecta.

We performed limb lengthening and correction of deformity of nine long bones of the lower limb in six children (mean age, 14.7 years) with osteogenesis imperfecta (OI). All had femoral lengthening and three also had ipsilateral tibial lengthening. Angular deformities were corrected simultaneously. Five limb segments were treated using a monolateral external fixator and four with the Ilizarov frame. In three children, lengthening was done over previously inserted femoral intramedullary rods. The mean lengthening achieved was 6.26 cm (mean healing index, 33.25 days/cm). Significant complications

Remission of idiopathic thrombocytopenic purpura after femoral lengthening. Clinical case followed for 5 years.

We report on a patient with chronic idiopathic thrombocytopenic purpura (ITP) who went into remission after femoral lengthening. Although it is possible that spontaneous remission (frequency 5%-10%) of chronic ITP coincided with the femoral lengthening, limb lengthening could also have caused the thrombocytosis. This case suggests a close relationship between osteogenesis and hematopoiesis during regenerate bone formation. Limb lengthening can therefore be defined as the formation not only of bone and soft tissue but also of hematopoietic tissue.

Repeated intramedullary stabilization following failed telescopic nail lengthening - An appropriate treatment strategy.

Externally magnetic-controlled intramedullary telescopic nails for bone lengthening have recently gained popularity due to many advantages when compared to more traditional lengthening methods. Patients requiring lengthening often present with a clinical history of previous multiple surgeries increasing the risk for complications of further procedures. However, studies regarding the treatment of complications following implantation of these devices remain scarce in literature. Therefore, we report our experience with revision surgery after lengthening with a telescopic intramedullary lengtheni…
