Numerical investigation of mechanical effects caused by various fixation positions on a new radius intramedullary nail.

Fracture of the radius diaphysis is an unusual injury in adults. Open reduction and plate osteosynthesis has been recommended by most of the authors. However, this trend has started to change with the recent introduction of newly designed interlocking intramedullary (IM) nails. New generation of IM nails are developed in order to utilise the advantages of IM nails against plates. Because of its anatomical structure, the radius bone has a complex geometry. Therefore, the callus structure, which forms during the healing period, should not be affected from external effects, such as excessive load

A novel method for lateral callus distraction and its importance for the mechano-biology of bone formation.

We introduce a novel method of lateral callus distraction for bone formation, which avoids the conventional splitting and weakening of bones. At the medial aspect of the sheep tibia the periosteum was resected and small holes were drilled into the cortex to connect the bone surface with the marrow. A distraction device with a hydroxyapatite-coated titanium plate was fixed over the drilled area. After 10 days latency the plate was distracted perpendicular to the bone's long axis twice a day by 0.27 mm for 10 days. The newly formed tissue was then allowed 50 days of maturation. In a control grou

[Optimizing the degree of fixation stability based on the strain theory].

Elastic flexible internal fixation stabilizes the fracture using the principle of splinting. Foregoing absolute stability using compression serves to maintain the mechano-biological induction of fracture healing. It also serves to maintain the blood supply to soft tissues and bone. The healing pattern seen after flexible fixation corresponds to that of secondary bone healing. It starts early and requires less precision, which in turn helps to avoid iatrogenic damage during reduction and stabilization.Prognosis based alone on the degree of fracture mobility is inadequate because spontaneous hea

Good reduction of paresthesia and pain after excision of excessive callus formation in patients with malunited clavicular fractures.

BACKGROUND AND AIMS: Sequelae following non-surgical treatment of clavicular fractures are not uncommon. The aim with this study was to describe results after excision of excessive callus in patients with late onset of paresthesia/numbness. MATERIAL AND METHODS: Eight patients with hypertrophic callus after a malunited clavicular fracture were operated with removal of excessive callus and scar tissue, without stabilizing ostheosynthesis. The reason for surgery was paresthesia/numbness in the arm and hand. There was a delay between trauma and onset of symptoms on average 10 (6-18) months. Neur

Callus response to micromovement after elongation in the rabbit.

The purpose of this investigation was to determine whether induced micromovement could improve the consolidation of diaphyseal elongation by callus distraction. Two series of paired rabbit hindlimbs were studied. The surgical procedure, waiting period, and elongation period were identical. One hindlimb was then left under neutralization conditions, but the other limb was stimulated by axial micromovements. Reproducible tibial osteotomy and lengthening of the two tibiae were confirmed radiographically. The mineralized callus was quantified by dual-beam x-ray absorptiometry. The anteroposterior

Foreign-body reaction to absorbable fixation devices.

While it is evident that poly-p-dioxanon is less reactive than polyglycolide, recent studies, including the case above, have shown that Orthosorb pins are not exempt from the same type of foreign-body reaction as the Biofix products.

Digital lengthening. With emphasis on distraction osteogenesis in the upper limb.

Digital lengthening, commonly practiced for hand digits of segments, seldom for those of the foot, has been much enhanced by new applications of old techniques and some surprising physiologic responses to those techniques.

[Damage to the arm plexus caused by atypical callus formation following clavicular fracture].

Secondary brachial plexus paralysis do not occur very often as a consequence of collar bone fractures. It is difficult to detect the cause, especially as there can be quite a long interval between the trauma and occurrence of the first symptoms. The problem is discussed with reference to a case observed by the authors: atypical callus formation 7 weeks after a clavicular fracture that was conservatively treated and had healed had led to brachial plexus damage. The patient was treated 11 months later by a trauma surgeon. Once the callus had been removed she was finally free of symptoms.

[Bone healing and dynamic interferential current (DIC) (author's transl)].

The aim of this investigation was to clarify the influence of dynamic interferential current (DIC). Two sinusoidal currents of medium frequency are superimposed within the body to determine the reactive changes of bone healing after osteotomy. An osteotomy was performed on the radius and ulna of the right foreleg of 34 'black-head breeding sheep'. The radius shaft was deperiostalized and unstably fixed with a four-hole AO-plate leaving a gap of at least 1 mm width. DIC of different mA-values were not treated 24 sheep 3 times per week for 10 mins. The other 10 animals were not treated with DIC

Deferoxamine restores callus size, mineralization, and mechanical strength in fracture healing after radiotherapy.

BACKGROUND: Therapeutic augmentation of fracture-site angiogenesis with deferoxamine has proven to increase vascularity, callus size, and mineralization in long-bone fracture models. The authors posit that the addition of deferoxamine would enhance pathologic fracture healing in the setting of radiotherapy in a model where nonunions are the most common outcome. METHODS: Thirty-five Sprague-Dawley rats were divided into three groups. Fracture, irradiated fracture, and irradiated fracture plus deferoxamine. The irradiated fracture and irradiated fracture plus deferoxamine groups received a huma

Impaired fracture healing with high non-union rates remains irreversible after traumatic brain injury in leptin-deficient mice.

Patients with traumatic brain injury (TBI) and long-bone fractures can show increased callus formation. This effect has already been reproduced in wild-type (wt) mice. However, the mechanisms remain poorly understood. Leptin is significantly increased following TBI, while its role in bone healing remains unclear. The aim of this study was to evaluate fracture healing in leptin-deficient ob/ob mice and to measure any possible impact of TBI on callus formation. 138 female, 12 weeks old, ob/ob mice were divided into four groups: Control, fracture, TBI and combined trauma. Osteotomies were stabili

Serum MicroRNA signatures associated with hypertrophic callus formation in polytrauma patients with traumatic brain injury.

Long-bone fractures occasionally develop excessive callus formation in the presence of traumatic brain injury, a clinically relevant but poorly understood injury response. Although this phenomenon is known for decades, the causal factors are still underexplored, and no systemic biomarkers are currently available to predict the exuberant bone-mass-formation at an early timepoint after injury. In this study, we used small-RNA-seq, bioinformatic analyses, and in vitro assays to identify a set of micro-RNAs in sera of hypertrophic callus patients that could serve as potential biomarkers. The ident

Mechanosensitive Piezo1 is crucial for periosteal stem cell-mediated fracture healing.

The biomechanical environment plays a dominant role in fracture healing, and Piezo1 is regarded as a major mechanosensor in bone homeostasis. However, the role of Piezo1 in fracture healing is not yet well characterized. In this study, we first delineated that Piezo1 is highly expressed in periosteal stem cells (PSCs) and their derived osteoblastic lineage cells and chondrocytes. Furthermore, downregulation of Piezo1 in callus leads to impaired fracture healing, while activation by its specific agonist promotes fracture healing through stimulation of PSC-modulated chondrogenesis and osteogenes

[An unusual cause of shoulder pain: stress fracture of the clavicle].

We report a case of stress fracture of the clavicle in which diagnosis was delayed due to the unusual localization of pain and absence of predisposing risk factors. A 34-year-old woman presented with severe right-sided pain in the shoulder, arm, neck, and hemithorax, and numbness in her right upper extremity. Systemic radiographs, cervical and brachial plexus magnetic resonance imaging (MRI) and thorax computed tomography (CT) did not show any pathology. In a retrospective review of the shoulder MRI sections, an edematous appearance was noted in the right clavicle and adjacent soft tissues, su

Colchicine inhibits fracture union and reduces bone strength--in vivo study.

INTRODUCTION: Recent studies have demonstrated that Colchicine (CO) prevents heterotopic ossification (HO) after total hip replacement in patients suffering from familial Mediterranean fever (FMF). Other investigators have proved that CO is an in vitro inhibitor of proliferation of osteoblasts and osteosarcoma cells, and is a non-selective mitosis inhibitor and selective inhibitor of mineralization. METHODS: A double blind prospective study comprised four groups of adult rats. The left posterior tibia in each rat was fractured except in one of the control groups. The study groups were treated
