Large inter- and intra-individual variations exist in breastmilk composition, yet factors associated with these variations in the short-term are not well understood. In this study, the effects of breastfeeding on breastmilk cellular and biochemical content were examined. Serial breastmilk samples (∼5 mL) were collected from both breasts of breastfeeding women before and immediately after the first morning breastfeed, and then at 30-minute intervals for up to 3 hours post-feed on 2-4 mornings per participant. The infant fed from one breast only at each feed. Effects of pump versus hand expressi

Breast milk has been reported to ameliorate the severity and outcome of neonatal abstinence syndrome (NAS). The mechanism of this beneficial effect of breast milk on NAS remains unclear, as the negligible amount of methadone transmitted via breast milk is unlikely to have an impact on NAS. The aim of this study was to compare the impact of different feeding modalities on the onset of NAS. A retrospective medical record review was conducted on 194 methadone-maintained mother/infant dyads. Infants were categorized on the first 2 days of life as predominantly breastfed, fed expressed human breast

INTRODUCTION: With the prevalence of infertility increasing worldwide, many are seeking adoption to fulfill the need to start or expand their family. However, one of the challenges mothers face is the lack of the early maternal bond with the adopted infant, which typically starts during pregnancy, and then continues after birth, while providing care and nourishment to the infant. Breastfeeding is proven to strengthen the maternal-infant bond and provides numerous benefits to the dyad. Reports of induced lactation in non-biological mothers are uncommon, they are even more uncommon to find in wo

The physical and emotional condition of the mother delivering a premature or low birth weight infant may be quite different than that of the mother of a healthy term infant when initiating breast-feeding. Despite this difference, incidence and duration of lactation among mothers of pre-term or low birth weight infants was found to be quite good compared with that of mothers of term infants. Considerable problems, however, are encountered by premature or low birth weight infants when breast-feeding, including delayed first suckling, poor or no suction, need for daily supplemental bottle feeding

OBJECTIVE: This study aimed to compare the effectiveness of two different breast pump suction patterns (BPSP) during the initiation of lactation in mothers of term, late preterm and preterm infants.
METHODS: Breast pump-dependent mothers (n=130) of term (n=19), late preterm (n=44) and preterm (n=67) infants were assigned to either a standard or irregular-BPSP after birth until the onset of secretory activation. Both groups used the same standard maintenance BPSP thereafter. Time to secretory activation, time to full milk production and daily milk output were compared between the standard and

Critical malnutrition and hypernatremic dehydration resulting from inadequate breast-feeding have previously been reported; the finding of elevated sodium concentration in expressed samples of mother's milk from these infants is found concurrently. This is the first case report describing successful relactation following management of hypernatremic dehydration in a breast-fed infant. Mother's milk was of inadequate volume and had a high sodium content. Treatment of the primary cause of this disorder, insufficient lactation, was effective in allowing successful relactation; a nursing supplement

BACKGROUND: Despite the benefits of breastfeeding to both infant and mother, many mothers find breastfeeding difficult secondary to many complications such as pain, breast engorgement, mastitis, and clogged milk ducts. The latter is typically treated conservatively with techniques such as gentle massage, breast pumping, and compresses. When conservative therapies are unsuccessful, more invasive options are considered to ensure the continuation of breastfeeding.
CASE DESCRIPTION: A 34-year-old lactating female presented with a 3-month history of worsening left breast and nipple pain radiating

Melamine (MEL) and cyanuric acid (CYA) are contaminants that can enter the human body through dietary sources, raising significant toxicological concerns, particularly for infants. MEL undergoes minimal biotransformation in the human body. While breast milk is widely recognized as the optimal source of nutrition for infants, it may also serve as a potential route for the transmission of contaminants. This study aimed to quantify the concentrations of MEL and CYA in breast milk and assess the associated non-carcinogenic toxicity risks for infants aged 0-6 months. A total of 100 mothers from Ham

BACKGROUND: Hypoxic-ischemic encephalopathy (HIE) remains a major cause of neonatal mortality and long-term neurodevelopmental disability. Intranasally delivered mesenchymal stem cells and neurotrophic factors from breast milk have been detected in the central nervous system and may have a role in neuroregenerative processes. F-NEO-BRIGHT (Feasibility- NEOnatal- intranasal BReast milk, Impact on brain Growth in HIE Therapy) is the first clinical study aimed to assess the feasibility and safety of intranasal fresh breast milk (iFBM) in neonates with HIE in both inpatient and home settings.

The provision of breast pumps is a potential strategy to increase breastfeeding duration. This trial compared the effectiveness and acceptability of two breast pumps in mothers exclusively breastfeeding (EBF) their healthy term infant. It also tested whether provision of pumps versus vouchers of equivalent value influenced breastfeeding or attainment of mothers' goals at 3 and 6 months. Mothers were randomised at 3- to 4-week post-partum (Beijing [n = 30], Moscow [n = 34], London [n = 45], New York [n = 3]) to groups A (Philips single-electric pump, Natural bottle), B (Medela Swing single-elec

BACKGROUND: The impact of breast milk feeding on susceptibility to asthma in childhood is highly controversial, due in part to failure of the majority of studies in the area to adequately account for key confounders exemplified by respiratory infection history, plus the effects of recall bias.
METHODS: As part of a prospective cohort study on the role of respiratory infections in asthma development in high-risk children, we measured the concentration of a panel of anti-infective proteins in maternal milk samples and analyzed associations between these and subsequent atopy-, infection-, and as

Multifactorial diseases cannot be fully explained by genetic variation alone, highlighting the importance of epigenetic mechanisms that integrate early-life environmental exposures with long-term health outcomes. Within the Developmental Origins of Health and Disease (DOHaD) framework, human breast milk has emerged as a critical regulator of early-life epigenetic programming. Breast milk-derived bioactive components, including non-coding RNAs, extracellular vesicles, immune factors, hormones, and human milk oligosaccharides, actively modulate DNA methylation, chromatin structure, and gene expr

Human T-cell leukemia virus type I (HTLV-I), the causative agent of adult T-cell leukemia/lymphoma, is transmitted vertically by breast milk and sexually by semen. The transforming growth factor-beta (TGF-beta), a pleiotropic cytokine that is abundant in breast milk and semen, facilitates replication of HTLV-I in lymphocytes derived from asymptomatic HTLV-I carriers and transmission to cord blood lymphocytes in vitro. Transient expression assays revealed that TGF-beta can transactivate HTLV-I long terminal repeat promoter. These results suggest that TGF-beta may play a role in replication and

Human cytomegalovirus (HCMV) is shed into breast milk in nearly every seropositive woman during lactation. This reactivation shows mostly a self-limited, unimodal course. The dynamics and functional role of HCMV-specific-IgG in breast milk and in plasma during reactivation are unknown. Milk whey viral loads were monitored with real-time PCR in 18 HCMV-seropositive mothers over two months postpartum. HCMV-antibody binding assays (ECLIA) and antigen-specific immunoblotting were performed from plasma and corresponding milk samples. Epithelial-cell-specific neutralization was used to analyze funct

This review explores the relationship between maternal obesity and alterations in the expression of microRNAs (miRNAs) in breast milk, highlighting how these changes may influence the developmental programming of the infant. Evidence suggests that maternal obesity can affect the bioactive composition of breast milk, including miRNA profiles, which are key regulators of metabolic and immune pathways in early life. Specific miRNAs, such as miR-148a and miR-30b, have been identified as modulators of metabolic processes, potentially impacting offspring growth, energy balance, and long-term health

BACKGROUND: Serratia marcescens, a gram-negative bacterium once considered non-pathogenic, is now recognized as a cause of hospital-acquired infections, particularly in immunocompromised patients with outbreaks reported in neonatal intensive care units. In newborns, it can result in severe illness, including sepsis and meningitis. It is well recognized by the production of pink pigment found on drains and other equipment, including breast pumps.
CASE REPORT: We report a case of a two-month-old previously healthy infant with upper gastrointestinal bleeding, whose mother had pink-stained expres

There is a paucity of data on the effect of preterm birth on the immunological composition of breast milk throughout the different stages of lactation. We aimed to characterise the effects of preterm birth on the levels of immune factors in milk during the 1st month postpartum, to determine whether preterm milk is deficient in antimicrobial factors. Colostrum (days 2-5 postpartum), transitional milk (days 8-12) and mature milk (days 26-30) were collected from mothers of extremely preterm (<28 weeks of gestation, n 15), very preterm (28-<32 weeks of gestation, n 15), moderately preterm (32-<37

INTRODUCTION: Exclusive breastfeeding for the initial six months of life is crucial and it is recommended . Breast milk jaundice is an innocuous condition that occurs in some healthy, breastfed infants. However, the potential dangers of jaundice in the neonate such as bilirubin induced neuronal pathology, mandates a better understanding of the pathophysiology of breast milk jaundice and the impact of breastfeeding during jaundice. In this context , advice on continued breastfeeding must consider both the benefits of breastfeeding and the possible disadvantages of the jaundice.
METHODS: Review
