BACKGROUND: Brucellosis has become a major medical problem in Israel particularly in the Muslim Arab population.
METHODS: Eighty-eight children with acute brucellosis are described. Sixty-seven were studied retrospectively during 1987 through 1988, and 21 children were studied prospectively during 1989 through 1992. Epidemiologic, clinical and laboratory features were evaluated, and the outcome of 4 antimicrobial regimens are compared.
RESULTS: Although the clinical manifestation varied, the classical triad of fever (91%), arthralgia or arthritis (83%) and hepato- and/or splenomegaly (63%) c…

Two hundred and sixty three patients with a diagnosis of brucellosis between January 1984 and December 1987 were studied prospectively. Sixty five patients (25%) developed osteoarticular complications. These patients had a more prolonged course than those with no complications. Spondylitis in 38 (58%) and sacroiliitis in 29 (45%) were the most prevalent. There were no significant laboratory, serological, or bacteriological differences between patients with and without osteoarticular complications. At diagnosis 47 patients (72%) showed radiographic abnormalities, commonly in axial sites but rar…

A case of induced pemphigus occurring in a patient affected with a brucellosis is reported with emphasis on the definitive remission of the pemphigus after antibiotic therapy against brucellosis and in the absence of corticosteroid treatment. Attention is also drawn to a possible autoimmune mechanism of the pemphigus triggered by brucellosis.

Eleven patients with brucellosis presented with neurological features closely simulating transient ischaemic attacks, cerebral infarction, acute confusional state, motor neuron disease, progressive multisystem degeneration, polyradiculoneuropathy, neuralgic amyotrophy, sciatica and cauda equina syndrome. Most patients improved quickly after adequate antibiotic treatment but chronic cases responded poorly. These protean neurological manifestations of brucellosis indicate that the underlying pathological mechanisms are diverse.

Cardiac involvement in childhood brucellosis is rare and when present mimics findings usually noted in acute rheumatic fever with carditis. We report five children aged 6 to 11 years. Echocardiography showed mitral valve vegetations in one, functional mitral valve incompetence in two, and sluggish myocardial function in one. All the patients presented with fever, arthralgia, and malaise, four of them had leucopenia, and all five showed relative lymphocytosis. Blood cultures grew Brucella melitensis, biotype 1 in four cases and type 2 in one. Treatment with oral tetracyclines over three weeks t…

106 patients diagnosed as having brucellosis were typed for HLA class I and class II antigens. The number of healthy controls was 272 and 102 for classes I and II, respectively. 17 HLA-A, 31 HLA-B, 7 HLA-C and 8 HLA-DR specificities were studied. The most important finding was a highly significant decrease of the HLA-Cw2 antigen frequency in the patient group compared with the controls. The corrected p value was p less than 0.0001 (Fisher's test). This finding is compatible with the idea that HLA-Cw2 antigen could be a protective factor. No other positive or negative association was observed. …

INTRODUCTION: and Importance: Brucellosis is a common prevalent zoonotic disease in developing countries including Somalia. Brucellosis may affect many organs. However involvement in the spine and paravertebral muscles is common and may lead to diagnostic challenges since it presents with non-specific symptoms.
CASE PRESENTATION: Here we report 18-year old male patient who presented with low back pain, low grade fever, difficult voiding and progressive lower extremity weakness for 5 months. The pain was localized to the lumbar vertebra (tender on palpation) and radiating to both lower limbs. …

Brucellosis is being increasingly recognized after solid organ transplantation but has not been reported after orthotopic heart transplantation. We present the case of a 51-year-old farmer who underwent orthotopic heart transplantation and was readmitted after 3 months in a severely immunosuppressed state with significant nonspecific complaints. He posed a diagnostic and management dilemma to all disciplines, but finally turned out to be harboring Brucella infection. He responded well to medical management and was discharged in a stable clinical status. Although rare, brucellosis should be inc…

Brucella suis is a significant zoonotic species that is present in domestic livestock and wildlife in many countries worldwide. Transmission from animal reservoirs is the source of human infection as human-to-human transmission is very rare. Although swine brucellosis causes economic losses in domestic livestock, preventing human infection is the primary reason for its emphasis in disease control programs. Although disease prevalence varies worldwide, in areas outside of Europe, swine brucellosis is predominantly caused by B. suis biovars 1 and 3. In Europe, swine are predominantly infected wi…

Brucellosis is a systemic infection with multiple presentations. Despite its oral transmission and gastrointestinal pathogenesis, systemic symptoms are usually more prominent than gastrointestinal complaints. We report a patient with enteric fever caused by Brucella melitensis.

OBJECTIVES: To describe haemorrhagic pleural effusion as a rare complication of brucellosis that finally needed lung decortication.
CLINICAL PRESENTATION AND INTERVENTION: A 37-year-old female presented with a 1-week history of fever, dry cough and pleuritic chest pain. Physical examination showed signs of right pleural effusion and hepatosplenomegaly. Complete blood count showed pancytopenia, white blood cells 2.9/mm3, haemoglobin 10 g/dl, platelets 131/mm3. Chest X-ray confirmed a moderate right pleural effusion, that was found to be exudative biochemically. Culture of pleural fluid and blo…
