Acute Guillain-Barre syndrome (GBS) is characterized by an acute onset of limb weakness and areflexia. There are a few rare variants that have been described and one of them is the pharyngeal-cervical-brachial (PCB) variant (oropharynx, neck, and proximal upper limb muscles). However, in this patient, the only presentation was bulbar involvement with fast recovery within days. This is likely to be the milder form of PCB that has rarely been described before. A 19-year-old Malay lady presented with progressive dysphagia associated with nasal voice for one week duration. There was no limb weakne

BACKGROUND: Virtual reality (VR) is showing increasing promise for assessing, understanding, and treating mental health difficulties. Virtual humans (VHs) represent a key aspect within many VR mental health applications. While VHs can play diverse roles and display varied characteristics, their design and influence have rarely been the primary focus of mental health research.
OBJECTIVE: We aimed to carry out a systematic review of how VHs in immersive VR have been used in applications for mental health, focusing on their roles and interaction types, and the human characteristics being tested.

Clinical studies and case reports have shown that nortriptyline and imipramine exert type 1 antiarrhythmic properties when plasma concentrations are therapeutic, indicating that clinical reservation against their use in cardiac patients may be overstated. The clinical cardiac effects of amitriptyline have not been extensively reported. The case I have reported details the electrophysiologic record of the beneficial and safe use of amitriptyline in a patient with severe cardiac arrhythmias and organic affective disorder.

An 11-year-old girl developed a severe brainstem encephalopathy thought to be due to multiple sclerosis. She has been left with a severe long-standing bulbar palsy and a permanent tracheostomy. A new operation of laryngeal closure is described which gave relief from recurrent aspiration pneumonia and cardiorespiratory failure.

There have been periodic case reports of sudden death due to aspiration or asphyxiation in patients receiving phenothiazines. The mechanism of death is unknown. The author reports a case of a drug-induced bulbar palsy-like syndrome in a man receiving fluphenazine enanthate and reviews the literature on sudden respiratory death and phenothiazines. He postulates that phenothiazine-induced bulbar palsy may cause laryngeal and/or pharyngeal dysfunctions that are responsible for aspiration.

A case with wide-spread subcortical neurofibrillary changes, cell loss and gliosis in a 81 year old man is described. The most affected areas were both pallida, Luys subthalamic bodies, the substantia nigra and the denate nuclei of the cerebellum. The cerebral cortex contained only a few senile plaques. We interpreted this case as a variety of progressive supranuclear palsy despite the lack of the typical supranuclear ophtalmoplegia and of superior colliculi involvement. In the subthalamic body and in the substantia nigra fibrillary tangles were investigated ultrastructurally and found to cons

We report a 51-year-old woman with polymyositis accompanied by a high titer of antiacetylcholine receptor antibody. The patient presented with weakness of grip strength followed by rapidly progressive dyspnea, which required mechanical ventilation. She was treated with a glucocorticoid and came off the respirator one week later. Antiacetylcholine receptor antibody activity was elevated in the acute phase and decreased during recovery, although other signs of myasthenia gravis were negative. This patient suggested that in cases of rapidly progressive bulbar palsy and limb muscle weakness, it is

We herein report a case of Human T-lymphotropic virus type-I (HTLV-I)-associated myelopathy with bulbar palsy-type amyotrophic lateral sclerosis-like symptoms. A 52-year-old woman developed dyslalia at approximately 40 years of age, which slowly progressed. She presented with muscular atrophy and increased tendon reflexes of the extremities as well as bulbar palsy, from which motor neuron disease was suspected. Cerebrospinal fluid (CSF) testing revealed no abnormalities except for an elevated neopterin concentration at 143.17 pmol/mL (normal ≤30 pmol/mL). Her serum and CSF anti-HTLV-I antibody

Brown-Vialetto-Van Laere syndrome (BVVLS) is a rare type of motor neuron disease, characterized by bilateral deafness, and multiple cranial nerve involvement. The disease is more frequent in females, but rapid deterioration was attributed to male gender or early onset. Herein, we report a man with BVVLS who had a longer than expected interval between onset of deafness and other motor symptoms that culminated to diagnosis. Six years later, he had gradual worsening and death. Along with potential new developing therapeutic options, this case emphasizes considering the diagnosis of BVVLS especial

AIM: Worster-Drought syndrome (WDS), or congenital suprabulbar paresis, is a permanent movement disorder of the bulbar muscles causing persistent difficulties with swallowing, feeding, speech, and saliva control owing to a non-progressive disturbance in early brain development. As such, it falls within the cerebral palsies. The aim of this study was to describe the physical and neuropsychological profiles of children with WDS.
METHOD: Forty-two children with WDS (26 males, 16 females; mean age 7y 10mo, SD 3y 1mo; range 2y 6mo to 16y 5mo) were studied prospectively using a standard protocol.

A variety of plants and seeds, some of which are used for therapeutic and nutritional purposes, can cause neurotoxic symptoms. The ingestion of the green or ripe fruit of the Karwinskia humboldtiana (buckthorn), a bush known in Mexico as coyotillo or tullidora, causes a flaccid, symmetric, progressive, and ascending palsy of the lower limbs, which, in severe cases, can cause bulbar palsy and death. The neurologic symptoms are similar to those of poliomyelitis, Guillain-Barré syndrome, and other polyradiculoneuritis syndromes, for which it is often mistaken. The clinical diagnosis of intoxicate

Colistin, a polymyxin antibiotic, is increasingly used to manage infections caused by multidrug-resistant gram-negative bacteria. This case report documents the occurrence of colistin-induced neurotoxicity manifesting as bulbar palsy in a 38-year-old male with a complex medical history. The patient, presenting with septic shock secondary to a diabetic foot ulcer, was administered colistin and meropenem, leading to a progressive onset of neurological symptoms. The timely discontinuation of colistin resulted in the complete resolution of neurotoxicity.

The incidence of wound botulism in injection drug users has increased since the introduction of black tar heroin. Many species of the Clostridium genus, most commonly Clostridium botulinum, Clostridium baratii, and Clostridium butyricum, have been associated with wound botulism. Patients often present with progressive bulbar weakness, including dysphagia, cranial nerve palsies, and loss of speech, in addition to symmetrical descending weakness of the upper extremities that may progress to the chest and lower extremities. In this article, we present 3 cases of wound botulism, in which the patie

RATIONALE: Brown-Vialetto-Van Laere syndrome, currently recognized within the spectrum of riboflavin transporter deficiency type 3 (RTD3), is a rare inherited neurometabolic disorder associated with pathogenic variants in SLC52A3. It typically presents with progressive sensorineural hearing loss, pontobulbar dysfunction, peripheral neuropathy, and respiratory involvement. Diagnosis may be delayed because routine metabolic and neuromuscular investigations can be unrevealing despite a characteristic clinical phenotype.
PATIENT CONCERNS: A young Asian man presented with several years of progress

Brown-Vialetto-Van Laere syndrome (BVVLS) is a rare neurodegenerative disorder associated with progressive cranial and spinal motor neuron involvement. While typically managed conservatively, select patients may present with focal peripheral deficits amenable to surgical intervention. We report the first known case of intrinsic hand dysfunction in BVVLS treated using a supercharged end-to-side (SETS) anterior interosseous-to-ulnar motor nerve transfer. Electroneuromyography revealed isolated postganglionic ulnar motor dysfunction with preservation of the median nerve, supporting surgical candi

The typical presentation of multifocal motor neuropathy (MMN) is progressive asymmetric limb weakness. Cranial neuropathy is rare. We report a 28-year-old woman with cranial and bulbar palsies but with typical electrophysiological features of MMN by multifocal motor conduction blocks and serological markers of anti-ganglioside GM1 antibodies. The previous consensus on the treatment of MMN is intravenous immunoglobulins, but our patient responded to oral steroids and had clinical and electrophysiological improvement under continuous low-dose prednisolone treatment. In summary, MMN is a treatabl
