Estimates of lifetime bulimia nervosa (BN) range from 4% to 6.7% across studies. There has been a decrease in the presentation of BN in primary care but an increase in disordered eating not meeting full diagnostic criteria. Regardless of diagnostic status, disordered eating is associated with long-term significant impairment to both physical and mental quality of life, and BN is associated with a significantly higher likelihood of self-harm, suicide, and death. Assessment should adopt a motivationally enhancing stance given the high level of ambivalence associated with BN. Cognitive behavior t…

Cluster analyses of eating disorder patients have yielded a "dietary-depressive" subtype, typified by greater negative affect, and a "dietary" subtype, typified by dietary restraint. This study aimed to replicate these findings in an adolescent sample with bulimia nervosa (BN) from a randomized controlled trial and to examine the validity and reliability of this methodology. In the sample of BN adolescents (N=80), cluster analysis revealed a "dietary-depressive" subtype (37.5%) and a "dietary" subtype (62.5%) using the Beck Depression Inventory, Rosenberg Self-Esteem Scale and Eating Disorder …

OBJECTIVE: To examine whether having a baby following treatment for bulimia nervosa places women at increased risk for continuing or relapsing eating disorders or major depression.
METHODS: Subjects were women who had participated in a large randomized controlled trial evaluating cognitive behavior therapy for bulimia nervosa, who were prospectively followed-up over 5 years. At follow-up assessments (at least yearly), life charts were completed with patients and childbirth was recorded. The presence of eating disorders and major depressive disorder was assessed using the Structured Interview …

Bulimia nervosa (BN) is a prevalent illness. There are multiple different medical complications that impact the overall treatment plan and prognosis of these patients. Practitioners should be alert for medical complications that are a direct result of the mode of purging behavior utilized by the bulimic patient. The treatment will proceed most smoothly if the primary care physician and the mental health professional work collaboratively and have clear and frequent communication.

OBJECTIVE: The purpose of this retrospective study was to replicate and to extend previous work indicating that bulimia nervosa is associated with elevated serum levels of cholesterol and to examine the association between cholesterol and other clinical and serum measures.
METHODS: Clinical characteristics and laboratory data including nonfasting cholesterol and thyroid indices were collected from charts of 119 women with bulimia nervosa and 42 female controls.
RESULTS: There was no significant difference between the two groups in mean age or body mass index. The mean serum cholesterol level…

Bulimia nervosa (BN) is an eating disorder frquently accompanied by changes of the hard and soft tissues of the oral cavity and of the salivary glands. Routine dental examination in a 25-year-old woman disclosed oral signs and symptoms characteristic of BN and she was referred for psychiatric evaluation. Oral evaluation of patients suspected of BN is recommended to prevent irreversible damage to the oral and perioral structures.

More common than anorexia and less easily detected, bulimia typically begins as an effort to control weight. Although the prognosis is better than that for anorexia nervosa, a number of medical complications can arise related to the mode of purging. Some of these complications can have profound deleterious sequelae if not diagnosed and treated.

OBJECTIVE: Despite literature citing the frequency of abnormal eating behavior in persons with schizophrenia, little attention has been paid to the comorbidity of eating disorders and schizophrenia. This case review explores the comorbidity of bulimia nervosa and schizophrenia and its possible clinical implications.
METHOD: The authors present four case reports of women with rigorously diagnosed schizophrenia who have eating-disordered behavior.
RESULTS: The first case describes a woman whose bulimia nervosa clearly preceded the onset of her schizophrenia. The second and third cases describe…

More common than anorexia and less easily detected, bulimia typically begins as an attempt to control weight. The prognosis is better than in anorexia, however; more than 80% of patients recover. Most complications relate to the method of purging. Self-induced vomiting may have oral and esophageal sequelae, and abuse of diuretics, laxatives, or diet pills can induce profound intestinal and systemic effects.

Degree of eating pathology in bulimia nervosa was assessed using the Eating Behavior Rating Scale (EBRS) across three videotaped eating sessions. Twelve bulimic and 12 non-eating disordered subjects participated in this study. All subjects were filmed during a baseline meal (ad libitum), a high-calorie meal, and a low-calorie meal (both fixed amounts). Mean EBRS scores were significantly higher for bulimics (12.6 +/- 1.7) than for controls (3.9 +/- 0.9), suggesting a higher degree of eating pathology in these patients. Affect during eating was significantly more negative in the bulimic patient…

The histories and psychological profiles of more than 500 patients meeting DSM-III-R criteria for bulimia nervosa were reviewed. A total of 310 patients demonstrated the most characteristic pattern of bulimia, with finger-induced purging and occasional diet pill, diuretic, or laxative abuse. Seventeen patients reported binge eating with no self-induced vomiting but with severe laxative abuse (i.e., greater than or equal to 50 laxatives daily). A total of 126 patients reported bulimia with finger-induced purging and regular mild (i.e., 2-3 daily) laxative abuse. Eight patients reported bulimia …

Twenty-two normal weight women with bulimia nervosa (BN) were studied (mean age, 25 +/- 5 yr; body mass index, 20.2 +/- 2.6 kg/m2). Sixteen of them reported menstrual cycles in the range of 21-42 days, and 6 had experienced absence of menstruation for at least 3 months. Twenty-one healthy women with regular menstrual cycles (mean age, 23 +/- 2 yr; body mass index, 20.7 +/- 1.4) served as the control subjects. Frequent morning blood samples for estradiol (E2) and progesterone (P4) determinations were obtained for the duration of 1 menstrual cycle or for 6 weeks in the case of amenorrhea. LH, FS…
