Permanent His bundle pacing: shaping the future of physiological ventricular pacing.

Conventional right ventricular (RV) pacing, particularly RV apical pacing, can have deleterious effects on cardiac function. Long-term RV apical pacing has been associated with increased risk of atrial fibrillation, hospitalization for heart failure, pacing-induced cardiomyopathy and associated death. His bundle pacing (HBP) results in physiological ventricular activation and has generated tremendous research interest and enthusiasm. By stimulating the His-Purkinje network directly, HBP results in synchronized ventricular activation, which might translate into improved clinical outcomes compar

Unmasking of left free wall ventricular preexcitation by His bundle ablation.

His bundle ablation was performed in a 48 year old man with drug refractory orthodromic atrioventricular reentrant tachycardia and paroxysmal atrial fibrillation. Reentry was caused by a left free wall concealed accessory pathway. Interruption of the His bundle by low energy direct current shock (25 J) was quickly followed by anterograde conduction via the accessory pathway, with various forms of advanced block. The combined effect of unfavourable anterograde conduction (prolonged anterograde effective refractory period and decremental conduction), the left lateral location of the accessory pa

Right-sided versus left-sided radiofrequency ablation of the His bundle.

UNLABELLED: Radiofrequency (RF) ablation of the His bundle was attempted in 30 consecutive patients with atrial flutter or fibrillation. A 7 French quadripolar catheter with a 4-mm distal electrode was advanced from the right femoral vein (21 patients), or subclavian vein (two patients) and positioned across the tricuspid valve. Adequate His-bundle potentials were obtained in all patients. However, in six patients atrioventricular (AV) block could not be obtained after multiple (mean = 8) applications of RF energy from the conventional right-sided approach. In these patients the same catheter

Electrophysiologic study of tachycardia-dependent paroxysmal His bundle block in man.

Electrophysiologic study was performed in a patient with tachycardia-dependent paroxysmal atrioventricular block. The site of block was within the His bundle. The effective refractory period of the His bundle was markedly prolonged and it was comparable to the critical atrial cycle length producing type II His bundle block. The most likely mechanism of paroxysmal atrioventricular block was repetitive concealed penetration of the blocking zone by nonconducted impulses that reached the proximal His bundle. Enhancing the blocking ratio at the atrioventricular nodal level resulted in improvement o

[Tachycardia-dependent paroxysmal block in the bundle of His. Electrophysiological demonstration].

The authors report the case of a patient who presented with angina of effort followed by transient loss of consciousness or syncope. Clinical examination yielded little information. The electrocardiogram showed signs of an old antero-septal infarct, and a slightly prolonged PR interval at 0.22 s. A recording of the activity of the bundle of His showed a double H potential, whose two components were separated by an interval of 80 ms. This interval increased progressively under the influence of atrial stimulation. At a critical frequency of 125/mn, a complete block below the bundle of His was pr…

Differential response of myocardium and His bundle to steroid therapy in a patient with cardiac sarcoidosis: A change of pacing threshold in His bundle pacing with cardiac sarcoidosis.

A patient with cardiac sarcoidosis who had introduced His bundle pacing and caused threshold deterioration was treated with steroid therapy, which resulted in lead threshold improvement and avoidance of lead replacement. Additionally, the thresholds of improvement with steroid therapy vary between the myocardium and His bundle in these patients.

Long-term outcomes of His bundle pacing in patients with heart failure with left bundle branch block.

OBJECTIVES: His bundle pacing (HBP) can potentially correct left bundle branch block (LBBB). We aimed to assess the efficacy of HBP to correct LBBB and long-term clinical outcomes with HBP in patients with heart failure (HF). METHODS: This is an observational study of patients with HF with typical LBBB who were indicated for pacing therapy and were consecutively enrolled from one centre. Permanent HBP leads were implanted if the LBBB correction threshold was <3.5V/0.5 ms or 3.0 V/1.0 ms. Pacing parameters, left ventricular ejection fraction (LVEF), left ventricular end-systolic volume (LVESV)

Application of His bundle recording in different rhythm disturbances of the heart.

The new intracardiac electrophysiological method: His bundle electrocardiography, combined with the electrophysiological examination used in routine clinical practice, has considerably improved our knowledge of the cardiac dysrhythmias. It helps to localize the focus of the atypical junctional and other supraventricular premature beats complicated with either organic or functional intraventricular conduction disturbance. The various forms of paroxysmal tachycardias can be differentiated more exactly from each other. In pre-excitation syndrome, it has become possible to detect the abnormal path

Comparison between his-bundle pacing and left bundle branch pacing in patients with atrioventricular block.

BACKGROUND: Pacing the cardiac conduction system has been explored in patients with conduction system disease, but comprehensive comparisons between different pacing modalities are not well investigated. OBJECTIVE: To compare pacing characteristics and ventricular synchrony between His-bundle pacing (HBP) and left bundle branch pacing (LBBP) in patients with atrioventricular block (AVB). METHODS: Fifty pacemaker-indicated patients with AVB were enrolled. Twenty-five patients underwent HBP, and another 25 patients underwent LBBP. Success rate, procedural and fluoroscopy duration, pacing param

Left Bundle Branch Block-Induced Cardiomyopathy in a Transplanted Heart Treated With His Bundle Pacing.

A 70-year-old male with prior orthotopic heart transplant developed left bundle branch block followed by new-onset left ventricular systolic dysfunction. He underwent His bundle pacing for cardiac resynchronization therapy with complete normalization of his ejection fraction. This is the first reported case of left bundle branch block-induced cardiomyopathy in a transplanted heart. (Level of Difficulty: Advanced.).
