Zinc burns: a rare burn injury.

A patient was presented with significant burns resulting from a workplace accident in a zinc production unit. This occurred as a result of the spontaneous combustion of zinc bleed under high pressure. The patient sustained burns to the face, body, and hands and suffered significant injury to the left cornea. Computed imaging revealed solid particles in the ethmoid sinus and also in the right nasal fossa, dissecting the right lacrimal duct. Photographic documentation is presented. This injury was potentially preventable and resulted from poor observance of safety procedures.

Epidemiology of burns. The burn-prone patient.

Predisposition to burning was identified by history, by conversation with the family, or by physical examination. Factors that decreased the patient's ability to respond appropriately were considered as predisposing. A consecutive series of 155 hospitalized, burned, adult patients was reviewed. Approximately 50 per cent of the entire series showed predisposition to burning; among the more severe burns, this fraction was 57 percent. Among women, predisposition was more prominent in all categories than among men. Among women, those predisposed to burning had larger burns and a greater likelihood…

"Burning tongue" and "burning tip": the diagnostic challenge of the burning mouth syndrome.

OBJECTIVE: To investigate the clinical features of burning mouth syndrome (BMS) in a large cohort of patients and to correlate them with the results of tongue biopsy. METHODS: We screened 98 patients complaining of oral burning pain for at least 6 months. Forty-two patients were excluded after screening for contact sensitivity to dental materials, food allergies, tongue injuries, malignancies, connective tissue and metabolic disorders, oral infectious diseases, vitamin deficiencies, and other systemic diseases known to cause neuropathy. Fifty-six patients underwent neurologic examination and

HORMONAL PROFILE IN SEVERE BURNS.

Acute or chronic burn injuries require hormonal responses that significantly influence patient prognosis. Elevated cortisol, catecholamines, and glucagon levels, lead to important metabolic changes, such as hyperglycaemia, insulin resistance, protein catabolism, free fatty acids oxidation, and secondary metabolic acidosis. These alterations impair immune function and wound healing and trigger a systemic inflammatory response. A multidisciplinary approach is needed in order to correctly manage the aforementioned endocrine and metabolic changes. During the acute phase, glucose monitoring, cortic

Mucormycosis in Burns: A Review.

Mucormycosis is a rare fungal infection with a high mortality rate. It presents with scattered black/necrotic ulcers, white fungal elements, and progression of wounds despite seemingly adequate debridement. Diagnosis is confirmed on wound histology; however, this is often delayed. There is currently no comprehensive review of burn-related mucormycosis within the literature, making this the first article to provide evidence-based treatment guidance. We performed a review of publications from 1946 to the present. There were 151 cases of mucormycosis complicating burns. The mortality rate was 54.…

Topical treatment for facial burns.

BACKGROUND: Burn injuries are an important health problem. They occur frequently in the head and neck region. The face is the area central to a person's identity that provides our most expressive means of communication. Topical interventions are currently the cornerstone of treatment of burns to the face. OBJECTIVES: To assess the effects of topical interventions on wound healing in people with facial burns of any depth. SEARCH METHODS: In December 2019 we searched the Cochrane Wounds Specialised Register; the Cochrane Central Register of Controlled Trials (CENTRAL); Ovid MEDLINE (including …

[Skin engineering for severe burns].

Severe burned patients need definitive and efficient wound coverage. Outcome of massive burns has been improved by using cultured epithelial autografts (CEA). Despite fragility, percentages of success take, cost of treatment and long-term tendency to contracture, this surgical technique has been developed in few burn centres. First improvements were to combine CEA and dermis-like substitute. Cultured skin substitutes provide earlier skin closure and satisfying functional result. These methods have been used successfully in massive burns. Second improvement was to allow skin regeneration by usi…

Perceived fatigue following pediatric burns.

PURPOSE: Fatigue is a common consequence of numerous pediatric health conditions. In adult burn survivors, fatigue was found to be a major problem. The current cross-sectional study is aimed at determining the levels of perceived fatigue in pediatric burn survivors. METHODS: Perceived fatigue was assessed in 23 children and adolescents (15 boys and 8 girls, aged 6-18 years, with burns covering 10-46% of the total body surface area, 1-5 years post burn) using both child self- and parent proxy reports of the Pediatric Quality of Life Inventory Multidimensional Fatigue Scale. Outcomes were compa…

Chlorhexidine burns after shoulder arthroscopy.

Chlorhexidine is an antiseptic and disinfectant commonly used for surgical site preparation and cleansing. It is active against a broad spectrum of bacteria, viruses, mycobacteria, and fungi. We report 3 cases of patients with superficial partial thickness burns immediately following shoulder arthroscopic surgery with the use of a Chloraprep 26 mL applicator (2% chlorhexidine gluconate and 70% isopropyl alcohol; CareFusion, Leawood, Kansas). All 3 patients reported pain as the anesthetic waned at a localized area on the anterior arm near the axilla. Erythema and blistering were noticeable. The…

Iatrogenic burns: beware of microwaves!

(1) The traditional hot-water bottle now faces competition from a variety of similar devices, such as microwave-heated compresses and gel packs; (2) These devices can cause severe burns; (3) Microwave-heated gel packs can be harmful for two main reasons. First, microwave ovens heat deeply and unevenly and dangerous temperatures can quickly be reached. In addition, gels retain heat longer than other materials such as cotton compresses or towels; (4) Burns are sometimes caused by lengthy contact with an object that is not hot enough to cause pain or even discomfort. The heat perceived by the use…

Plaster cast burns: the reality.

An in vivo experimental study was performed of the temperatures produced by a setting plaster cast using hot and cold water. The results confirmed a statistical difference in the temperatures reached using hot and cold water. Forearm and below knee plasters did not represent a burn risk. Lower limb cylinder casts reached sufficient temperatures to cause burns with hot water but did not remain at these temperatures long enough to pose a clinical risk.

Microsurgical reconstruction in children's burns.

There have been few reports on microsurgical procedures performed on children who have been left with disability and deformity as a result of burn injury. The reality is that whilst there are many children who could benefit from such procedures they do not have access to the resources that can provide them. Where these resources are available there has been a reluctance to use them partially due to a misconception that such procedures are technically challenging and carry significant risk. This paper presents a series of 21 children in whom 29 microsurgical procedures were performed. There wer

[Acid burns in the oesophagus].

BACKGROUND: In the literature are different ways to treat patients with acid burns in the oesophagus. PATIENTS: Between 1989 and 1995, 169 patients were examined in the Ear, Nose and Throat Clinic at the Olga Hospital, Stuttgart, with suspected acid burns in the oesophagus. The patients examined included 168 children under 13 years and one adult. If possible a rigid oesophagoscope was used to make the diagnosis. There were no complications. 17 patients had first degree acid burns, 37 second degree acid burns, 20 third degree acid burns. The patients with acid burns were treated with cortison …

[Systemic complications of extended burns].

Burns covering more than 10% of the total body surface area (TBSA) are responsible for systemic perturbations which, in very severe cases, can represent a vital risk and, in all cases, affect the wound evolution. Among these general perturbations, fluid volume and electrolyte changes, leading eventually to burn shock, have the most dramatic consequences. Burn shock is, still to day, a vital risk and can also, in case of inadequate early fluid resuscitation, results in secondary morbidity and mortality. Fluid replacement during the very first hours after injury represents certainly a key point …
