Nontuberculous mycobacterial bursitis and arthritis of the shoulder.

A 56-year-old woman with systemic lupus erythematosus developed septic arthritis and bursitis of the left shoulder due to an atypical mycobacterium, M. xenopi. Plain radiography, ultrasound (US), and MRI were performed. Articular disease by tuberculous and nontuberculous mycobacteria have similar presentations, clinically as well as radiologically, and have to be differentiated from other chronic bacterial or fungal infections, pigmented villonodular synovitis, rheumatoid arthritis, gout, hemophilia, and synovial chondromatosis. Although atypical mycobacterial involvement of the skeleton and s…

Septic bursitis: experience in a community practice.

We reviewed 47 episodes of septic bursitis occurring in a private community medical practice. Most patients were male (85%), and roughly half (49%) the cases were related to recreational or occupational trauma. About 72% of cases were located in the olecranon bursa, while the remaining cases were prepatellar. Prepatellar bursitis patients were more likely to be hospitalized. Staphylococcus aureus was isolated from 70% of bursal fluid aspirations; other etiologic organisms included gram negative bacteria and Mycobacterium marinum. The majority of patients were able to be treated as outpatients …

Management and outcome of infective prepatellar bursitis.

Forty seven cases of prepatellar bursitis are reported. Twenty one patients had sustained a recent injury with a break in the skin which had caused the infection and seventeen patients were employed in jobs which involved kneeling. Oral antibiotics proved to be inadequate treatment in many cases. Splintage and intravenous antibiotics with or without aspiration of the bursa were usually successful in treating the condition, although nine patients required surgical drainage of the bursa. Twelve patients continued to have symptoms months or years after the infection, particularly those with preex…

The surgical approach to refractory trochanteric bursitis.

This article identifies and describes a small group of patients with trochanteric bursitis for whom all modes of conservative therapy have failed. They are treated surgically by release of the ileotibial band, removal of trochanteric osteophytes and debridement of the gluteus maximus bursae. In this series all of the patients had satisfactory results and returned to near-normal function without significant complications.

Pediatric Infectious Prepatellar Bursitis with Kingella kingae.

We present the first reported case of septic prepatellar bursitis with Kingella kingae in a 2-year-old female. Although it is a well-established cause of osteoarticular infections in the pediatric population, K. kingae has never been reported as the etiology for septic bursitis. A high index of suspicion is required for the diagnosis given that this organism is difficult to culture and isolate using standard laboratory methods. Our diagnosis was established through bursal fluid analysis, though oropharyngeal polymerase chain reaction (PCR) may be also be considered. Our case also builds upon p…

Protothecal olecranon bursitis: an unusual algal infection.

Prototheca is an achlorophyllic alga which rarely causes infections in humans and protothecal olecranon bursitis is remarkably rare. We report a case of a 76-year-old immunocompetent man presenting with pain and swelling of the right elbow secondary to protothecal infection. Initial cultures of the olecranon bursal aspirate revealed no growth; however, repeat aspiration after 2 months grew prototheca species on culture. Prototheca wickerhamii and Prototheca zopfii are the only 2 protothecal species known to cause human infections. Protothecal infection can manifest as skin infections, extremit…

[Osteomyelitis after septic trochanteric bursitis - clinical case].

We report the clinical case of a 76 years-old woman with a subacute trochanteric inflammatory pain with low-grade fever and laboratory markers of acute inflammation, associated with the ultrasonographic evidence of bursitis and radiologic evidence of femoral erosions, that resolved after intravenous antibiotherapy. Although rare, the infectious etiology should be considered in patients with clinical manifestations of bursitis and signs of systemic involvement.

Chronic Bilateral Olecranon Bursitis: A Case Report.

Olecranon bursitis is a common condition that primarily affects men between the ages of 30 and 60. Although the conservative treatment of acutely inflamed olecranon bursitis is relatively straightforward, managing chronic olecranon bursitis can be challenging. In this publication, we report a case of rare bilateral chronic olecranon bursitis and discuss the rationale for choosing the best treatment option.

Prepatellar Morel-Lavallée Effusion Mimicking a Bursitis.

Morel-Lavallée lesions are traumatic abnormalities characterized by the accumulation of hemolymphatic fluid collection following disruption of subcutaneous fat tissue from the underlying deep fascia. Here, we present the case of a 63-year-old woman with a prepatellar Morel-Lavallée lesion, an unusual location for this pathology initially misdiagnosed as prepatellar bursitis. An MRI was performed allowing the correction of the diagnosis, highlighting the essential role of imaging in confirming the diagnosis and ruling out differentials such as prepatellar bursitis or neoplastic origins. In our …
