PURPOSE: To study the clinical features and bacteriology of canaliculitis in patients presenting to King Khaled Eye Specialist Hospital (KKESH), a major tertiary eye care center in the Middle East and compare the results to previous studies from other countries.
METHODS: In this retrospective study, a chart review was performed of 131 patients (135 eyes) diagnosed with lacrimal canaliculitis who underwent treatment between January 1983 and December 2012 at KKESH. Data were evaluated on demographics, presenting signs and symptoms, diagnostic studies, causative organisms, treatment rendered inc…

PURPOSE: To compare two distinct surgical techniques in the management of chronic Primary Lacrimal Canaliculitis (PLC) with clinically detectable concretions.
MATERIALS AND METHODS: Inclusion criteria: Patients presenting with symptoms consistent with the diagnosis of primary lacrimal canaliculitis (PLC) with ocular involvement were gathered. Gomori-Grocott Methenamine Silver test was used for concretion staining. Two groups were identified based on the surgical technique. Group A consisted of patients who underwent a punctum sparing canalicular curettage using a chalazion curette, while the …

Lacrimal fistulae are rare and not well known by radiologists. Four cases are illustrated, two congenital (a skin-canaliculous and a canaliculous-palpebral fistula) and two acquired (a rare case of sac-orbital fistula and a canaliculous-palpebral fistula arisen during dacryography). Fistulae are to be distinguished from pedunculated lacrimal diverticulous of which an interesting and rare case is presented. After a careful examination of the pathogenesis and symptomatology of lacrimal fistulae the authors agree that only macrodacryography carried out by means of sophisticated techniques (seriog

BACKGROUND: Dacryolithiasis can occur anywhere in the lacrimal drainage system and is frequently associated with microbial infections. The p … ed with dacryolithiasis at our hospital from January 2017 to December 2021. Patients diagnosed with canaliculitis were treated with canaliculotomy. Dacryocystitis complicated by canaliculitis was treated with endoscopic dacryocystorhinostomy (En-DCR) combined with canaliculotomy. Dacryocystitis accompanied by dacryoliths was treated with En-DCR. Canaliculitis is a rare disease of the lacrimal pathway, especially of the canaliculi. It is often not identified, therefore misdiagnosed and inadequately treated. It accounts 2 % of all lacrimal diseases. False diagnoses are usually conjunctivitis, blepharitis, dacryocystitis, hordeolum and chalazion. Besides viruses and fungi a variety of bacteria can cause a canaliculitis. Actinomyces is the most common pathogenic agent of canaliculitis. Its generic name was first described by Harz in 1877. In 1854 von Graefe as well as Kipp and others in 1883 identified actinomyces as the agent for intraca…

Analysis of the clinical details associated with 26 referred isolates of Propionibacterium propionicum revealed the marked propensity of this anaerobe to be involved with infections of the lacrimal apparatus; 18 (69%) of the 26 referred isolates were from cases of canaliculitis in which this organism was regarded as the significant pathogen. Further analysis of data showed a significant correlation with the sex of the patient. Of the 18 cases of canaliculitis, the sex of the patient was reported in 16; 14 (88%) of these 16 patients were females (P < .005). The age of 13 of the 18 patients with…

PURPOSE: To evaluate the complications and the treatment results of SmartPlug-related complications.
METHODS: Retrospective review of all patients from a single medical hospital who received SmartPlug (Medennium, Inc., Irvine, CA) insertions from October 2007 to February 2014. All patients who developed SmartPlug-related canaliculitis and pyogenic granuloma were analyzed. Lacrimal irrigation with antibiotics was performed in most patients.
RESULTS: Six hundred thirty-one eyes received SmartPlug insertion and 18 eyes developed SmartPlug-related complications (14 canaliculitis and 4 pyogenic g…

Actinomycotic conjunctivitis is usually unilateral and secondary to canaliculitis. We report an unusual case of bilateral actinomycotic blepharokeratoconjunctivitis in the absence of canaliculitis. Keratitis occurred without any preceding ocular trauma. Both eyes had remissions and relapses in response to the antibiotic-steroid drops. Diagnosis of A. israelii infection was made on the basis of Gram staining, culture and biochemical characteristics. Complete excision of the conjunctival ulcer along with penicillin therapy resulted in cure. This case highlights that microorganisms of the order A…

PURPOSE: To report the clinical presentations, complications, and outcomes of Sphingomonas paucimobilis-specific lacrimal drainage infection … hingomonas-specific lacrimal drainage infections of 11 patients were identified. Of these five were canaliculitis and seven were acute dacryocystitis. All the seven acute dacryocystitis presented in an advanced stage; five were with lacrimal abscess and two with orbital cellulitis. BACKGROUND: In clinical practice, ultrasound examination of the lacrimal drainage system is not able to represent the lacrimal canaliculi wi … , a good echographic visualisation of canaliculi structures was obtained from patients with chronic canaliculitis or dislocated punctum plugs. In chronic canaliculitis the ultrasonic images showed an ectatic canaliculus (diverticulum) and high-reflective structures (concrements).

Background. Lacrimal infections by Actinomyces are rare and commonly misdiagnosed for long periods of time. They account for 2% of all lacrimal diseases. Case Report. We report a case of a 70-year-old female patient suffering from a para-canalicular abscess in the medial canthus of the left eye, beside the lower punctum lacrimale, resembling a chalazion. Purulence exited from the punctum lacrimale due to inflammation of the inferior canaliculus (canaliculitis). When pressure was applied to the mass, a second exit of purulence was also observed under the palpebral conjunctiva below the lacrimal…

Actinomyces species are known to cause a variety of human infections. Ocular actinomycosis is a rare disease. We report an unusual case of bilateral actinomycotic blepharoconjunctivitis in the absence of canaliculitis that presented with forniceal masses in eye. The case report is discussed here along with Indian literature.

PURPOSE: Intracanalicular plugs are commonly used in the management of dry eyes. The authors report 3 cases of complications associated with intracanalicular plugs.
METHODS: Clinical findings and the management of these patients are presented.
RESULTS: The first patient (case 1) is a 47-year-old female with severe dry eyes. Six months after insertion of intracanalicular Form fit hydrogel plugs (OASIS Medical) she developed canaliculitis with Klebsiella oxytoca. The second patient (case 2), a 33-year-old female, who developed left canalicular abscess 5 months following the insertion of bilate…

Although relatively uncommon, fungi, atypical Mycobacteria, and Nocardia have been isolated from a variety of infections of eye including keratitis, scleritis, canaliculitis, dacryocystitis, endophthalmitis and orbital cellulites. The organisms typically cause a slowly progressive disease. The diseases caused by the organisms can pose both diagnostic and therapeutic challenges. In this manuscript we will describe updates on important aspects of the ocular infections caused by these organisms.
